Basophils in vernal conjunctivitis in humans: an electron microscopic study.

The histopathological changes of vernal conjunctivitis comprise stromal infiltration by lymphocytes, plasma cells, and neutrophilic, eosinophilic, and basophilic leukocytes and epithelial invasion by mast cells and eosinophils. Blood vessels show swelling and death of endothelial cells and increased permeability with associated extravasation of erythrocytes and fibrin. The presence of basophils in ocular tissue has not been reported previously, and their occurrence in conjunction with the other pathological changes enables vernal conjunctivitis to be compared with, and classified as a manifestation of, delayed-type hypersensitivity of the cutaneous basophil type. Thus the mechanism is probably a mixture of both delayed and immediate immunological responses.

Plasma cell content of main and accessory lacrimal glands and conjunctiva.

The size, weight, and plasma cell content of main and accessory lacrimal glands were examined and compared to the number of plasma cells in the conjunctiva. There were from four to 35 accessory lacrimal glands in the upper conjunctiva and a rare accessory gland in the lower conjunctiva. The total accessory gland mass was approximately one tenth of the lacrimal gland mass although the number of plasma cells was approximately 1/20 of the mass. The number of plasma-cells in the lacrimal gland was estimated to be 3,200,000; in the conjunctiva, 2,100,000; and for the accessory glands, 180,000. Because of the many plasma cells in the relatively inaccessible lacrimal gland, cells of the lymphatic series may home to the lacrimal gland by non-antigenic mechanisms.

Tissue, tear, and serum IgE concentrations in vernal conjunctivitis.

Abnormal upper tarsal tissue from four patients with vernal conjunctivitis stained immunofluorescently contained abundant IgA-, IgD-, and IgE-forming plasma cells in two of four patients. These were absent in normal upper tarsal conjunctiva. The IgE levels of paired tear-serum samples of 11 patients with vernal conjunctivitis and ten control subjects were 130 ng/ml in patients that did nor differ significantly from the normal tear IgE value of 61 ng/ml. The serum IgE means of 1,031 ng/ml for patients and 201 ng/ml for normal subjects differed significantly (P less than .05). Tear and serum IgE values correlated significantly for both groups (P less than .01 for patients, P less than .05 for normal subjects). These findings are consistent with a hyperplasia of IgA, IgD, and IgE antibody-forming cells in tarsal conjunctiva of some patients with vernal conjunctivitis, and tear IgE levels being a function of serum IgE levels.

Induced ocular pseudopemphigoid.

Two patients with a clinical picture identical to idiopathic ocular cicatricial pemphigoid had received long-term (six to nine years) echothiophate iodide treatment for control of glaucoma in the affected eyes after cataract extraction. Basement membrane zone staining for IgG was seen in the conjunctiva of one affected eye. Decreased or absent goblet cells, epidermalization of the conjunctiva, fibroses, and abnormal numbers of inflammatory cells were seen in both affected eyes.

The role of tissue typing in corneal transplantation.

The ABO relationships of 150 corneal transplant recipients and their donors were evaluated as were IgG and IgM isoagglutinins in recipients receiving grafts from ABO-incompatible donors. The HL-A antigens of 43 corneal transplant recipients and their donors were determined as were the cross-matches of donor lymphocytes in selected cases. No relationships of significance between mismatched donors and recipients or presence of antibody in recipients against antigens of the donor for either ABO or HL-A systems were found. It was concluded that in this series histocompatibility mismatches could not be shown to be a factor in corneal transplant failure.

ABO Blood groups and corneal transplantation.

A study of 150 corneal donor-recipient grafts, examined for ABO type, revealed that 25% of the grafts were from corneas from incompatible donors. Recipients with histocompatible sensitization exposures such as prior corneal transplant, one or more pregnancies, or one or more blood transfusions were no more likely to experience graft failure if the graft were from an ABO incompatible donor than from a compatible donor. Of 13 graft failures (8%), three of the failed grafts were from AB donors, a significant result. No detrimental effect of transplanting across the ABO barrier could be proven with assurance for any parameter.

Immunoglobulins in the human cornea.

We examined 20 corneas of ten subjects to determine the amounts and distribution of immunoglobulins and albumin. Immunoglobulins IgG and IgA were found in all corneas, were at the same level centrally and peripherally, and were at one-half and one-fifth the serum levels respectively. The amount of cornea IgG correlated with serum IgG, but not with IgA. Immunoglobulin IgM was present in five of the ten sets of eyes. No IgM was detected in the central buttons of four of the five sets where IgM occurred in the corneal periphery. Albumin was present in all corneas, but less centrally than peripherally. The cornea contains high levels of antibody proteins.

Penetrating keratoplasty in a patient with 75% third degree burns.

A 25-year-old woman was severely burned in a gasoline explosion in her home. One month after her admission to the hospital a mycotic corneal ulcer, caused by Candida albicans, developed and progressed to a descemetocele in spite of medical therapy. A therapeutic penetrating keratoplasty was performed and the graft remained clear until the present time (10 months of follow-up). The one episode of clinical corneal graft rejection occurred at the same time the first evidence of return of immune competence was seen in her skin grafts. The corneal rejection episode was successfully treated by topical corticosteroids.
