Chlorhexidine with an Anti Discoloration System. A comparative study.

UNLABELLED: Correct oral hygiene is believed to be the basis of primary and secondary prevention. Sometimes, using a toothbrush or other mechanical instruments for oral hygiene may be difficult and it may become necessary to use an antiseptic. Chlorhexidine is an essential component in many available preparations on sale, because of its marked antiseptic qualities. One of the most frequent side-effects is the appearance of stains on the teeth and mucous membranes, which particularly disturbs the patient. A new mouthwash containing chlorhexidine has recently become available, besides maintaining its antiseptic qualities, also avoids the side-effect of staining. OBJECTIVES: The aim of this study was to check the capacity of the new mouthwash, which contains chlorhexidine and Anti Discoloration System (ADS), not only to prevent plaque formation like the other mouthwashes containing chlorhexidine but also to avoid staining that is one of the most frequent side-effects. STUDY DESIGN: The comparative study was carried out on a sample of 15 patients treated with two mouthwashes both containing 0.2% chlorhexidine, but different in that the first does not contain ADS, which is instead present in the second, a new product. The results obtained show that in the 15 patients treated, there is no statistically significant difference in the ability of the mouthwash to prevent bacterial plaque, however evidence of the stain was much less with the new mouthwash.

[Genetic and microbiologic tests in periodontal disease].

AIM: The presence of genetic polymorphisms IL-1A and IL-1B was demonstrated to be associated to an increased clinical severity of Periodontitis. The aim of our study was to verify the existence of a relationship between such genetic polymorphisms and pathogenic variations of Actinobacillus actinomycetemcomitans (Aa), Prevotella intermedia (Pi) and Porphyromonans gingivalis (Pg), in patients with periodontitis before and after active therapy. METHODS: Twenty-five patients were randomly selected among those attending the Department of Periodontology, University of Bologna, on the basis of radiographic and clinical data. At the end of active therapy, subjects had to undergo "PST" test. RESULTS: The "PST" was positive in 32% (8 out of 25) of patients; the majority of them (4 out of 8) were classified ADA IV, 3 were ADA III and 1 was ADA I (p > 0.05). All subjects with good hygiene therapy (13 out of 24 patients) have shown a reduced bacterial count after active clinical therapy (Fisher test p = 0.03). CONCLUSION: No statistically significant association was observed between positivity to "PST" and severity of periodontal disease.

Dietary intake and nutritional status in Italian elderly subjects.

Dietary intake and nutritional status was assessed in a random sample of 190 Italians (70-75 years of age) participating in the Survey in Europe on Nutrition and the Elderly (EURONUT-SENECA). The daily energy intake as assessed by a Modified Dietary History, was 2208+/-562 Kcalories in men and 1742+/-527 Kcalories in women. The alcohol intake was significantly higher in men than in women (35. 9+/-32.5 g/day vs 14.7+/-15.4 g/day; p<0.0001). As for calcium, there was a high percentage of men (77%) and women (86%) with a lower intake than the recommended values. The subjects underweight (BMI<20) were only 4.1% men and 9.7% women, while the great part was normal (BMI= 20-24.9) and overweight (BMI=25-29.9). The body composition parameters showed a significant difference between two genders. Men had a Total Body Water (56.5+/-4.5% vs 51.3+/-5.4%; p<0. 001) and Fat-Free Mass (80.4+/-5.2% vs 70.9+/-6.8%; p<0.001) higher than women. Few subjects were at high risk of deficiency with regard to plasma levels of vitamins, haemoglobin and albumin. If we analyse the composition of the diet consumed, we can remark the characteristics of a typical Mediterranean diet. We conclude that the general nutritional status of our sample was fairly good.

Gingival recession and toothbrushing in an Italian School of Dentistry: a pilot study.

The aim of this pilot study was to evaluate the prevalence of gingival recession at buccal tooth surfaces in a student population of Bologna University Dental School. A total of 55 subjects, attending the 1st and 5th year of the course were examined. The clinical examination involved assessment of plaque, calculus, width of keratinized gingiva, buccal probing depth and buccal gingival recession. Information about toothbrushing behavior was collected in an interview. The multiple regression analysis showed that level of education (p=0.002), toothbrushing technique (p=0.013) and toothbrushing frequency (p=0.016) are significant contributors to gingival recession. Notwithstanding the limits of a preliminary study, a higher prevalence of buccal recession is observed in the final year students, which have also a significantly lower % of buccal surfaces with calculus (p=0.014). In addition, it clearly appears that there is the necessity to investigate other factors connected with toothbrushing (pressure, time, toothpaste quantity) in order to provide an oral hygiene education without undesired effects.

Antibiotic therapy in impacted third molar surgery.

The use of routine antibiotic therapy in patients undergoing surgical third molar extraction is controversial. The efficacy of antibiotic therapy in preventing postoperative complications following surgical third molar extractions was evaluated in 141 patients. In the test group (66 patients), the protocol utilized a regimen of 2 g of amoxicillin orally daily for 5 d postoperatively, starting at the completion of surgery. In the control group (75 patients), no antibiotic therapy was given. No significant difference was found between the test group and the control group in the incidence of postoperative sequelae, i.e. fever, pain, swelling and alveolar osteitis. A statistically significant association between smoking, habitual drinking and increased postoperative pain and fever was found. Patient age > or = 18 yr was positively correlated with an increased incidence of alveolar osteitis. Swelling was found to be gender-related, in that female patients experienced more swelling than male patients. No correlation was found between the time required for surgery or difficulty of extraction and post-operative pain. In conclusion, no difference was found between patients receiving postoperative amoxicillin and the control group in the incidence of postoperative sequelae.

Use of gloves and prevalence of glove-related reactions in a sample of general dental practitioners in Italy.

OBJECTIVE: The purpose of this study was to evaluate the frequency of the use of gloves and to assess the prevalence of glove-related symptoms in a sample of Italian dentists with a high occupational exposure to latex gloves. METHOD AND MATERIALS: A 31-question instrument was mailed to all dentists (n = 550) practicing in the town of Bologna during 1994 to determine the type of procedures for which gloves were worn by dentists and the prevalence of self-reported glove-related symptoms. RESULTS: Of the dentists who replied to the items (n = 160), 94% reported that they usually wear nonsterile latex examination gloves; and 46% reported that they wear surgical gloves, even if not exclusively. About 98% reported changing gloves after contact with each patient, and 14% reported using disinfectants to treat examination gloves. Of the 106 respondents claiming to use examination gloves for surgical activities, 79 (74%) reported wearing them for extractions. Sterile surgical gloves were reported to be mostly worn for implant placement (47%) and periodontal surgery (42%). There were 46 (29%) responses indicating glove-related problems; of those subjects, 14 (30%) reported having other allergies. CONCLUSION: The use of gloves is common in the sample of Italian dentists, and the choice between nonsterile examination gloves and sterile surgical gloves seems to be made appropriately.

A population based study of Helicobacter pylori infection in a European country: the San Marino Study. Relations with gastrointestinal diseases.

Helicobacter pylori is present worldwide but few large population studies exist on the epidemiology of the infection. A random cross sectional study was performed of H pylori infection in the adult population of San Marino, a European country with high gastric cancer rate, to assess its prevalence and to evaluate its relations with gastrointestinal disease. In 2237 subjects (77% of the initial sample) H pylori IgG antibodies were detected with enzyme linked immunosorbent assay (ELISA) and immunoblotting. A questionnaire including questions about occupation, place of birth, and smoking was given to all subjects. Dyspepsia, peptic ulcer, and gastric cancer in the subjects, relatives, and partners as well as use of drug, dental treatment/prostheses, and gastrointestinal endoscopies, were evaluated by multivariate analysis. H pylori prevalence was of 51%, increased with age from 23% (20-29 years) to 68% (> or = 70 years), and was higher among manual workers. H pylori was independently associated with ulcer (OR = 1.63, 95% confidence intervals (CI) = 1.16 to 2.27), H2 antagonists (OR = 1.94, 95% CI = 1.21 to 3.10), and benzodiazepines (OR = 1.57, 95% CI = 1.02 to 2.42), dental prostheses (OR = 1.25, 95% CI = 1.05 to 1.49), gastroscopy in the past five years (OR = 1.50, 95% CI = 1.05 to 2.14), peptic ulcer in siblings (OR = 1.52, 95% CI = 1.09 to 2.12), gastric cancer in father (OR = 1.61, 95% CI = 1.02 to 2.52). The association of seropositivity with history of ulcer, gastric cancer in family, gastroscopy, and H2 antagonists suggests that H pylori is an epidemiological key factor in the pathogenesis of gastroduodenal diseases in this area.

Descriptive analysis of the prevalence of anemia in a randomly selected sample of elderly people living at home: some results of an Italian multicentric study.

We studied hematological indexes (RBC, HB, HT, MCV), serum iron and serum ferritin values in 1784 randomly selected subjects aged 65 and over (725 males and 1059 females) divided into five age groups (65-69, 70-74, 75-79, 80-84, > or = 85 years). The subjects were classified as anemic and normochromic according to the criteria for a "geriatric" level of anemia (HB < or = 12 g/dL in both sexes) as well as "W.H.O." levels for anemia (HB < 13 g/dL in males and < 12 g/dL in females). Macrocytosis (MCV > 100 fl) and low serum ferritin level (< or = 12 ng/dL) were classified according to MCV and serum ferritin values. Mean HB values in males were 14.85 +/- 1.33; 14.82 +/- 1.40; 14.77 +/- 1.43; 14.59 +/- 1.47 and 13.83 +/- 1.13 in the five age groups (65-69, 70-74, 75-79, 80-84 and > or = 85 years) respectively; in females, they were 13.77 +/- 1.15; 13.75 +/- 1.27; 13.44 +/- 1.39; 13.44 +/- 1.52 and 13.34 +/- 1.61, respectively. There was a low frequency of anemia in the entire sample: 2.9% in males and 9.9% in females according to the "geriatric" level, and 9.4% in males and 8.8% in females according to the "W.H.O." level. There was a higher prevalence of macrocytosis in males (6.3%) than in females (3.3%). We conclude that red cell parameters tend to decrease in aging, and further investigations are needed that exclude persons with existing chronic conditions, and incorporate data on nutritional status.

Patient compliance with maintenance therapy in an Italian periodontal practice.

A sample of 414 patients, treated between 1985 and 1988 by an Italian periodontal practice for supportive periodontal therapy, was studied to determine compliance with recommended maintenance programmes. Patients included in the study had attended for supportive periodontal therapy for at least 1 year prior to the close of data collection in 1989. Based on their compliance with the suggested maintenance schedule, patients were classified as complete attending (100% of programmed visits), partial (at least 50% of programmed visits), or insufficient (less than 50% of programmed visits). Only 30% of the initial patient sample was found to be compliant at the end of 1989. Complete compliance decreased as the number of years after active therapy increased, from 38% at 1 year to 20% at 4 years. The % of patients with insufficient compliance increased from 37% to 46% from the 1st to the 2nd year, then remained stable over the following years, indicating that the 1st year represented the critical period when subjects decided whether to follow recommended maintenance therapy. No significant relationships were found between degree of compliance and patient gender, recall schedule or type of treatment procedure performed. However, complete compliant subjects were younger than the other groups (p < 0.03), and compliance increased with the number of surgeries (p < 0.002).

Body weight, fat distribution and the menopausal status in women. The VMH Collaborative Group.

Since sex hormones are involved in the regulation of body composition and adipose tissue metabolism, in this study we investigated whether menopause may alter body weight and fat distribution in women. Data were obtained from the Virgilio-Menopause-Health Study, which is a longitudinal epidemiological project aimed at investigating the relationship between menopause and related plasma hormonal concentrations, body weight, fat distribution and health. Only data from the first cross-sectional examination are presented. Out of the 952 women living in the town of Virgilio, Mantua (Italy) and born between January 1st, 1932 and December 31st, 1946, 596 (62.6%) participated in the study. Since incomplete data were present in 19 women, 577 was the final sample size available for statistical analysis. The protocol included a collection of blood for hormones and biochemistry, a full clinical history with socioeconomic and personal information, drug use, smoking, dietary and physical activity habits, and several anthropometric measurements. According to the clinical and hormonal status, 160 women were classified as pre-menopausal, 124 as peri-menopausal and 293 as post-menopausal. After adjusting for age, we found that body mass index (BMI) was significantly higher in peri-menopausal than in pre-menopausal women and remained slightly, but again significantly, higher in post-menopausal women. Although waist-to-hip and waist-to-thigh ratios increased significantly and progressively from pre- to post-menopause, any difference was not significant after adjusting for age. Diet, physical activity and smoking habits did not explain the difference in BMI values. No difference was found in the use of estro-progestagen compounds between the groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Development of a scoring system to assess disability in an Italian elderly population. Evaluation of a disability questionnaire.

Functional disability was evaluated in a cross-section sample of 671 persons, aged > or = 60 years attending the Local Health Care Unit of Borgo San Dalmazzo, Cuneo, Italy, by the OECD (Organization for Economic Co-operation and Development) questionnaire. The unidimensionality of the item set and grading order of severity were studied by Multiple Correspondence Analysis; cumulativeness was examined by Guttman scale-analysis. The OECD set measured two dimensions of disability, physical and sensorial. By excluding the six items that caused the greatest loss of homogeneity, a reduced set of ten items was obtained that was cumulative (Reproducibility coefficient = 0.934 and Scalability coefficient = 0.778) and also produced a simpler score (0-10 points scale). Based on this score, 40% of the subjects presented no disability in running 100 metres (the most difficult task), and males were less disabled than females (p = 0.001). The level of functional disability was slightly decreased in males, and nearly uniform in females up to 70-74 years, after which a clear decrease appeared (p = 0.001). The more disabled aged state a need for a higher number of health services (injection and physical therapy, nursing hospital, nursing home, physician), but knowledge and use of these services are not clearly related to physical function. Based on our results, the choice of scaling appears irrelevant if the homogeneity of the item set is checked; if the cumulative criterion is identified, the use of a simplified cumulative score can be proposed as a sensitive measure of disability, and indirectly, the amount of personal assistance needed.

Relationship between low lead exposure and somatic growth in adolescents.

The possible effects of moderate lead exposure on human growth and development are not well defined. This study was carried out in order to evaluate the relationship between somatic growth and lead exposure. 418 subjects (202 males and 216 females) aged 11-13 years in good health and living in an area of the northern region of Italy were submitted to the following determinations: blood lead levels (PbB), serum concentrations of gonadotropins (LH, FSH) and sex-steroid hormones (DHA-S, T, E2) and anthropometric measurements (stature and weight). The mean blood levels were 85.39 and 70.08 micrograms/1, respectively, for males and females. Significant and negative relationships were found between blood lead levels and stature in 13 years old males and 12 years old females (p = 0.009 and p = 0.006, respectively). Negative relationships between lead in blood and LH and FSH were found only in males with lead levels higher than 90 micrograms/1. Our findings seem to suggest that even for low lead exposure this metal may affect stature growth and gonadotropins levels.

Relationship between zinc in serum and hair and some hormones during sexual maturation in humans.

We have measured the levels of zinc in serum (Zn-S) and in hair (Zn-H) in 391 adolescents, in good health, aged between 11 and 14 years. To evaluate the relationship between zinc status and sexual maturation we have analyzed, in the same subjects, the serum concentrations of the following hormones: luteinizing hormone (LH), follicle stimulating hormone (FSH), dehydroepiandrosterone-sulphate (DHA-S), testosterone (T) and estradiol (E2). No significant relationship between zinc in the biological samples and the hormones measured in all subjects was observed; only in prepuberal females was a positive correlation found between Zn-S and E2. A significant relationship between Zn-S and LH was observed only for males with short stature or low weight (less than 25th percentile) (r = 0.359, p = 0.010; r = 0.47, p = 0.008, respectively). When prepuberal males with short stature were considered, a significant association between Zn-S and T appeared (r = 0.399, p = 0.006). In females with short stature (less than 25th percentile), partial correlation coefficients showed a significant association between FSH and zinc in hair (r = 0.435, p = 0.004), while in girls with low weight, FSH appeared positively related to zinc in hair (r = 0.470, p = 0.003) and negatively related to zinc in serum (r = -0.320, p = 0.050). Our results suggest that zinc plays an important role in the metabolism of hormones linked to sexual maturation.

Epidemiology of obesity in the elderly: CNR multicentric study in Italy.

This study includes anthropometric measurements (Body Mass Index, triceps and subscapular skinfolds, % Body Fat and Mid Arm Muscle circumference) of a cross-sectional sample of 1247 elderly representative of the Italian population between 65-95 yr (522 males and 725 females). BMI at the 50th percentile is 26 for males and 27.7 for females, at the 90th it is 31.1 and 34.7 for males and females, respectively. Compared with the data of Master et al. (1960), 13% (males) and 28% (females) of the elderly Italian subjects were overweight in 1985.

Ageing and small-bowel mucosa: a morphometric study.

A direct effect of ageing in reducing mucosal surface area of the small intestine is still disputed. The aim of this study was to morphometrically evaluate a panel of jejunal biopsy specimens obtained from 16 elderly patients and 22 younger controls, all complaining of abdominal symptoms but without evidence of malabsorption and/or malnutrition in either group. Quantitative histology was performed by calculating the surface area to volume ratio of jejunal mucosa and the mean enterocyte height. The mean of surface to volume ratios in geriatric patients (97.6 +/- 18), although slightly reduced, did not significantly differ from that of younger controls (107.6 +/- 26) and no significant correlation was found between surface to volume ratio and age in years. The mean enterocyte height in the elderly group (30.1 +/- 1.2 microns) did not significantly differ from that of younger controls (31.3 +/- 2.3 microns). In conclusion, the frequent finding of malnutrition due to malabsorption in the elderly should not be attributed to a reduction of jejunal surface area as a mere consequence of age.

Effects of cold stimulus of the chest wall on bronchial resistance.

We studied the possibility of inducing a bronchospasm by a cold stimulus (0 degrees C) on the chest wall of normal subjects and those with bronchial lability. The stimulus is applied on areas in metameric connection with the bronchial tree. Modifications of MEF25, MEF50 and FEV1.0 before and after stimulation are surveyed. Positive bronchospasm indexes are pointed out in a large number of subjects with bronchial lability, while no obstructive bronchial response is found in normal subjects. The cold stimulus on the chest wall can therefore be considered as an aspecific stimulus producing bronchospasm. Some physiopathogenetic interpretations are suggested.

[Comparison of three methods for the rapid determination of body composition].

The study of body composition is becoming increasingly important in the field of nutritional medicine. The possibility of assessing body composition easily and economically has therefore raised considerable interest. Using straightforward correlations, we have compared the data obtained from the assessment of BF% using plicometry, impedance analysis and the interactivity of close-infrared rays; in addition, we studied the possible influence of the distribution of fatty tissue, expressed as W/H, on the results obtained using these methods. One hundred and forty-eight subjects (51 M and 97 F) were studied aged between 13 and 76 years old, with BMI ranging between 16.8 and 56.4; subjects were divided into three groups according to BMI and W/H. The three methods proved to be well correlated in patients with BMI less than 30 (r between 0.92 and 0.74), whereas correlations were less marked and less significant in groups with BMI greater than 30. W/H seemed capable of influencing results obtained using these methods and this was more evident in results obtained using impedance analysis (p less than 0.0001). The three methods studied were comparable in normal weight and slightly overweight patients, whereas contrasting findings were obtained in patients with moderate and severe obesity. Extreme caution must therefore be used in interpreting the data obtained in these subjects.
