Intracranial tumours rarely metastasize outside the central nervous system (CNS). Meningeal haemangiopericytoma is an exception. We report the case of a 41-year-old female who initially presented in 1984 with what was thought to be a posterior fossa meningioma which was excised, as were two subsequent recurrences in 1987 and 1988. In October 1990 she represented, having discovered a mass in her abdomen. This proved to be a metastatic haemangiopericytoma in the liver which had similar histology to the original intracranial tumour. This metastasis, and another found at operation, have been excised, and at the time of this report, she is alive and well seven months after surgery without any evidence of other metastasis or recurrence. Hemangiopericytomas are unusual in their propensity to metastasize outside the CNS and successful resection of hepatic metastases from this tumour has not been reported before.
Financing accounts receivables is becoming a popular strategy in the healthcare industry. Factoring and securitization are two financing methods available to hospitals. Patient accounts managers who understand the programs' structures, incentives, and costs will be able to achieve the maximum benefit for their hospitals when choosing one of these transactions.
Two receivables financing approaches, factoring and asset-backed securitization, offer an initial cash flow boost and a predictable source for continual cash flow. In a typical receivables factoring program, a healthcare organization receives advance funding from its receivables and reduces collection and follow-up efforts required of its staff. In exchange, the organization: Sells receivables at a discount between 5 percent and 10 percent off face value; and Pays a factoring fee of up to 20 percent of sold receivables. In a typical asset-backed securitization: Proceeds generated from the sale of A1-rated commercial paper are used to purchase receivables from a hospital; Accounts receivable eligible for sale are advance-funded at a level between 80 and 90 percent, with the unfunded portion remaining an asset of the hospital; The hospital is responsible for collection and follow-up activities; and An asset manager maintains cash collections to retire commercial paper notes and pay administrative costs. A healthcare organization interested in receivables financing should review each option's structure and benefits to assess advance funding provided, costs, a seller's level of control, and program eligibility requirements.
Two hundred and ten patients with coeliac disease previously reported from this unit were reviewed at the end of 1985 after a further 11 years of follow up. The initial review at the end of 1974 could not demonstrate that a gluten free diet (GFD) prevented these complications, probably because the time on diet was relatively short. The same series has therefore been kept under surveillance with the particular aim of assessing the effects of diet on malignancy after a further prolonged follow up period. Twelve new cancers have occurred: of which one was a carcinoma of the oesophagus and two lymphomas. Thirty nine cancers developed in 38 patients and of 69 deaths, 33 were the result of malignancy. A two-fold relative risk (RR) of cancer was found and was because of an increased risk of cancer of the mouth and pharynx (RR = 9.7, p less than 0.01, 95% confidence interval (CI) = 2.0-28.3), oesophagus (RR = 12.3, p less than 0.01, CI = 2.5-36.5), and of non-Hodgkin's lymphoma (RR = 42.7, p less than 0.001, CI = 19.6-81.4). The results indicate that for coeliac patients who have taken a GFD for five years or more the risk of developing cancer over all sites is not increased when compared with the general population. The risk is increased, however, in those taking a reduced gluten, or a normal diet, with an excess of cancers of the mouth, pharynx and oesophagus (RR = 22.7, p < 0.001), and also of lymphoma (RR = 77.8, p < 0.001). A significant decreasing trend in the excess morbidity rate over increasing use of a GFD was found. The results are suggestive of a protective role for a GFD against malignancy in coeliac disease and give further support for advising all patients to adhere to a strict GFD for life.

562 patients whose duodenal ulcers had healed in a series of double-blind controlled trials of various medical treatments were enrolled in a long-term endoscopic follow-up. 436 were followed for up to four years or until relapse. Relapse rates did not differ between the groups treated with cimetidine, ranitidine, pirenzepine, or placebo. At six and twelve months but not subsequently, there was a significant advantage for tripotassium dicitrato-bismuthate. Relapse rates were at all times higher in smokers than in non-smokers.
Lymphocytes from 48 IBD patients including nine with HLA-B27 were examined for their capacity to absorb Y.enterocolitica 0:9 antiserum. They possessed a range of absorption ability which was unconnected with HLA-B27 status. Samples of lymphocytes from 28 AS patients, 10 RA patients and 28 normal control subjects were included for comparison. The significance of the findings is discussed.
Six bacteria believed to be associated with IBD and/or AS and ReA were investigated for antigenic cross-reactivity using an ELISA. Considerable degrees of cross-reactivity were detected and it is suggested that if bacterial antigens are involved aetiologically in the diseases, no specific bacterium is concerned.
Two patients with neurofibromatosis and somatostatinoma are described, one patient in addition having a parathyroid adenoma diagnosed post mortem. The other patient had a partial somatostatinoma syndrome with diabetes, abdominal pain and cholelithiasis. The tumour was diagnosed preoperatively and metabolic studies demonstrated mild diabetes mellitus apparently due to suppression of insulin secretion by somatostatin, since oral glucose tolerance returned to normal post-operatively despite hemipancreatectomy. The tumour also secreted gastrin. There are now 18 reported cases of neurofibromatosis and duodenal carcinoid tumours which makes a genuine association between these two conditions very likely. With the present two cases, seven of the carcinoid tumours in this group have been positively identified as somatostatinomas. The histological finding of psammoma bodies is important in the diagnosis of duodenal somatostatinomas.
A community survey was undertaken in order to determine the seroepidemiologic pattern of acute viral hepatitis in Auckland. As hospital records and Health Department notifications underestimate the problem, all patients with a serum alanine transpeptidase (ALT) level of greater than 200 mu/l (X 5 normal) were investigated for viral liver disease. Over a four month period a total of 303 cases of acute viral hepatitis were identified, 49 (16.2%) were hepatitis A, 11 with coincident hepatitis B, 88 (29%) were hepatitis B, 80 (26.4%) nonA nonB hepatitis, 81 (26.7%) Epstein Barr virus hepatitis and 5 (1.6%) cytomegalovirus hepatitis. Hepatitis A and hepatitis B occurred with increased frequency among Maoris and Polynesians, while Epstein Barr virus, cytomegalovirus and nonA nonB hepatitis occurred more frequently among Europeans. The incidence of acute symptomatic viral hepatitis (excluding cytomegalovirus and Epstein Barr virus infections) was 78 cases/10(5)/per year for the Auckland region in this survey.
On the basis of experiments performed some years ago, it has been generally accepted that mammalian sperm metabolic activity, as determined by manometric measurement of O2 uptake, increases during capacitation. However, when mouse sperm suspensions were incubated for a total of 120 min in glucose-containing medium which promotes both capacitation and fertilization, O2 consumption, measured with an O2 electrode at 30 and 120 min, consistently declined with time. This same pattern was also observed when pyruvate was the sole exogenous energy substrate, despite the fact that these cells, although capacitated, cannot fertilize eggs until supplied with glucose. Since mouse spermatozoa require a suitable glycolysable substrate to express full functional ability, these results suggest that a shift in metabolic activity from oxidative phosphorylation to glycolysis normally accompanies the onset of fertilizing ability in this species. It was further demonstrated that this pattern, in the presence of glucose, can be markedly modified by alterations in the medium composition which have been shown to modulate fertilizing ability. All conditions chosen inhibit fertilization while retarding (Ca2+-free), accelerating (hyperosmotic high Na+) or nor affecting (iso-osmotic high K+) the rate of capacitation. When suspensions were assessed in these media, O2 consumption increased rather than decreased over the 120 min, despite the continuous presence of glucose. When these same suspensions were assessed at 30 and 120 min in media altered to permit expression of fertilizing ability, the pattern observed was one of declining O2 consumption.(ABSTRACT TRUNCATED AT 250 WORDS)

We prospectively analysed the liver histology and clinical data of 45 patients with a clinical diagnosis of chronic hepatitis. There was more chronic active hepatitis than chronic persistent hepatitis. In both, there were more men than women except in the subgroup of lupoid hepatitis, where all were women. As a group, chronic persistent hepatitis patients tended to have less severe abnormalities in biochemical liver function tests. Chronic hepatitis B infection accounted for 38% (17/45) of all patients. Of these, 53% (9/17) were Maori or Polynesian, although they only account for approximately 1/5 of the European population in Auckland. This correlated with the known high hepatitis B surface antigen carrier frequency in the Maori and Polynesian and the high incidence of primary hepatocellular carcinoma in this ethnic group. The present study also showed there are relatively few chronic active hepatitis patients, those with immunological abnormalities (lupoid hepatitis, 5/45), who are likely to respond to steroid treatment.
Globally, liver abscess caused by Entamoeba histolytica is a common condition. In New Zealand, however, it occurs infrequently. Eight cases were diagnosed in Auckland between 1972 and 1982, each patient having travelled through an endemic area within two years of presentation. Management often was not ideal, with both delays in diagnosis and inappropriate treatment. There is a need for increased awareness of possible amoebic abscesses in those who have been in endemic areas because with prompt use of effective treatment complications can be avoided.
