Improvement of skin flap survival by local injections of Regitine and propranolol. An experimental study in the rat.

An increase in the surviving area of skin flaps was achieved by local injections of alpha-adrenergic and beta-adrenergic blocking agents. Dorsal skin flaps of albino rats were injected postoperatively at their bases with Regitine (Ciba-Geigy, Switzerland), propranolol and a combination of these two drugs, two or three times daily for 6 days. All the pharmacologically treated animals showed a significantly better flap survival rate than did the control group (P less than 0.001). More frequent injections gave increasing flap survival areas. However, a combination of the above-mentioned drugs did not improve the results when compared with propranolol administered alone, and was less effective than Regitine administered alone.

Silicone ectoprostheses for correction of facial defects.

Twenty-two patients with congenital and acquired defects, rehabilitated with silicone ectoprostheses, were followed up for 6 mo to 4 yr. The simplicity of the procedure, its good results and the early physical and social rehabilitation attained, favor its use when adequately indicated.

Territorial flooding, or perfusion takeover--an experimental study in abdominal flaps in the pig.

The arterial skin flap in the pig with 7 to 8 vascular pedicles, as described by Milton, was employed in a modified version by leaving it based on only one pedicle. Circulation through the base was shown to flow radially into the flap. The area thus supplied is considerably larger than that in the traditional rectangular longitudinal flap., Following initial distal necrosis, narrowing of the flap's base by severing 7 of its 8 pedicles caused slight marginal necrosis, while most of the flap survived on its single pedicle. A reasonable explanation for this phenomenon may be that the abdominal segmental arteries belong to one particular vascular system, which creates "territorial flooding" or "perfusion takeover" when circulation is impaired.

Electrical stimulation in the treatment of the painful hand.

Fifty patients with painful hands due to inflammation, trauma, or surgery were treated by means of a Neurogar nerve stimulator. In 82% of the cases the results were satisfactory. Early physiotherapy was possible, and yielded best recovery and healing.

Juvenile leg ulceration in beta-thalassemia major and intermedia.

A series of 12 patients with beta-thalassemia who developed juvenile leg ulcers is described. The trophic skin changes and leg ulcers occurred after the age of 15 years. Usually the ulcer occurred after trauma over the predisposed medial aspect of the leg. In four out of seven females, the trophic changes deteriorated during or following pregnancy. The development of trophic changes and subsequent leg ulcers is not related to the severity of the anemia or to the patients' transfusion requirements, but may be related to high fetal hemoglobin concentration. The ulcers are slow to heal and tend to recur. Multiple skin-grafting operations are required in order to achieve satisfactory coverage of the ulcers. Since tissue hypoxia may be the underlying pathophysiology, the use of topical hyperbaric oxygen chamber may be beneficial in the treatment of thalassemic leg ulcers.
