Existence of uncoupling protein-2 antigen in isolated mitochondria from various tissues.

Antibodies against Escherichia coli-expressed uncoupling protein-2 (UCP2) and uncoupling protein-3 (UCP3) were raised by operating the blotted proteins into the spleen of minipigs. The antisera reacted more intensively with the recombinant UCP2 and UCP3 than with uncoupling protein-1 (UCP1) isolated from brown adipose tissue. Moreover, anti-UCP2 and cross-reacting anti-UCP3 antibodies identified the presence of the UCP2/3 antigen in isolated mitochondria from rat heart, rat kidney, rat brain, rabbit epididymal white adipose tissue, hamster brown adipose tissue, and rabbit skeletal muscle. It has been concluded that UCP2 is expressed in these tissues (UCP3 in skeletal muscle); however their existence in mitochondria had not previously been demonstrated.

[Benign tumors of the pancreas].

The contemporary incidence of the tumours of the pancreas is approximately 3.5% in the Czech Republic. Benign tumours represent only about 2% of them. We have found 7 such patients (2.3%) in our population of 303 patients operated for pancreatic tumours. This paper summarizes our experience with the diagnostics and surgical treatment of these patients.

[Complications of esophageal stents used in benign esophageal stenoses].

Oesophageal stents are used in the treatment of stenosis of the oesophagus in the last 10 years. The application of the stent in benign stenosis should be exceptional because it is associated with high morbidity and mortality. Two cases with severe complications treated by operation are demonstrated. Types of stents for the treatment of the oesophageal stenosis are presented.

[Gangliocytic paraganglioma of the duodenum].

Gangliocytic Paraganglioma of the duodenum is rare neuroendocrine tumor. This type of the tumor can cause bleeding into the gastrointestinal tract and epigastric pain in this region. It can be difficult to differentiate this tumor from the primary tumor of the head of the pancreas growing secondary to the duodenum. The authors describe one such misdiagnosed case treated finally successfully by hemipancreatodecodenectomy.

Photoaffinity labelling of the mitochondrial uncoupling protein by [3H]azido fatty acid affects the anion channel.

Brown adipose tissue (BAT) mitochondria were incubated with the azido derivative of fatty acid (hexadecanoic) containing four tritium atoms, [3H]AzHA, and among all mitochondrial proteins only a few proteins were photolabelled after irradiation with UV. It suggests the existence of specific fatty acid binding sites on mitochondrial proteins. It was also possible to label with [3H]AzHA the isolated uncoupling protein (UcP) of BAT mitochondria with a low stoichiometry--lower than one AzHA per dimeric UcP. These results together with the observed competition (i.e. prevention of photolabelling) of various UcP anionic substrates with [3H]AzHA and its dodecanoic acid analogue, suggest the existence of the specific fatty acid binding site on UcP identical with the anion channel or anion translocating site.

[Retroperitoneoscopy--a new era].

Retroperitoneoscopy is a minimal invasive surgical technique enabling the diagnostics of the disease, as well as operative treatment of retroperitoneal organs. It is a very promising method especially for urologist, but of interest for general, vascular surgeon or gynecologist. The method is known since the end of sixties. Its routine introduction was enabled by technical advancement of laparoscopic surgery.

[Retroperitoneoscopic lumbar sympathectomy].

The development of revascularization techniques of the distal lower extremity dramatically diminished the need of lumbar sympathectomy. Which is indicated when patient with rest pain or with a profound gangrene of toes (which does not exceed the line of metatarsal articulation) could not be treated by vascular reconstruction. Lumbar sympathectomy could be done surgically or chemically, the efficiency of which is about 50%. The second to the fourth lumbar sympathetic ganglion could be resected with minimal invasivity, retroperitoneoscopically. This approach to lumbar sympathectomy opens further possibilities for operating in retroperitoneal space. Retroperitoneoscopy is well tolerated by patients.

[External pancreatic fistulae].

External pancreatic fistulas are feared complications of pancreatic surgery or to trauma of the pancreas. In our paper we report on 2 patients suffering of external pancreatic fistulas successfully treated by operation. We demonstrate the examinations suitable for preoperative mapping of the fistula, the possibilities of conservative treatment and the strategy of surgical management of the fistulas. During the follow-up time the results of the operation are excellent.

[Renal hemodynamics and albuminuria in patients with arterial hypertension].

BACKGROUND: The cause of hyperalbuminuria in hypertonic patients can be functional or irreversible structural changes. The objective of the present investigation was an attempt to differentiate these two possibilities by comparing data of hypertonic patients with normal albuminuria (albumin excretion < 20 micrograms/min) and those with microalbuminuria in patients with renovascular hypertension by comparing findings after successful percutaneous transluminal angioplasty of the renal arteries (n = 8). METHODS AND RESULTS: The authors examined 20 patients with normal albuminuria (12 men and 8 women, mean age 46 years) and 20 patients with microalbuminuria (12 men and 8 women, mean age 49 years). Of these patients 8 were examined repeatedly after angioplasty. The examination was made in the morning, renal haemodynamics were assessed by the infusion technique (clearance of polyfructosan and paraaminohippuric acid), in blood and urine the albumin and IgG concentration (bromcresol purpur-Lachema Brno and immunoturbidometrically-Tina-quant Boheringer Mannheim). In hypertonic patients with microalbuminuria a significantly higher (p < 0.001) IgG excretion was found (14.1 as compared with 5.12 micrograms/min.) and a reduced ratio of IgG clearance to albumin clearance (p < 0.001), 0.73 as compared with 1.60. After successful angioplasty of the renal arteries there was a significant drop of the blood pressure (p < 0.05) and of the renal vascular resistance as well as a significant decrease of albuminuria (p < 0.05) on average from 137 to 48 micrograms/min. The quantitative improvement of albuminuria, however, did not go beyond the discrimination value for microalbuminuria. CONCLUSIONS: The results suggest that microalbuminuria in hypertensive patients is as a rule a manifestation of structural renal changes, while also functional and reversible changes participate. The asset of treatment of hypertension by angioplasty of the renal arteries was manifested not only in the renal haemodynamics but also by reduced albuminuria.

[Pancreatic head resection with preservation of the duodenum--the optimal surgical procedure in chronic pancreatitis].

Surgical procedure appears as an optimal curative method for the treatment of advanced stages of all 4 types of chronic pancreatitis. Progress in diagnostic methods and defining of classification with more precision in last years led us to the idea to optimalize also surgical procedure for the treatment of chronic pancreatitis. Since 1985 we have been performing duodenum preserving pancreas head resection followed by pancreatogastroanastomosis in symptomatic form of chronic pancreatitis. Checking up of 28 patients operated on by this procedure confirm best results of all used methods. With regard to the character of the disease we are not able to improve external secretory function of the pancreas by this operation. External secretory insufficiency has to be substituted permanently by medicaments. Substantial contribution of this operation is in improvement of the quality of the life of the people suffering from chronic pancreatitis.

[Preoperative drainage of the biliary tract in patients with obstructive jaundice].

The authors present their experience with preoperative external-internal percutaneous transhepatic drainage of the biliary pathways (PTD) in 143 patients. From this group in the final stage 53 patients were operated. The authors discuss the contemporary position and importance of preoperative PTD and compare the advantages of external-internal PTD and external drainage of the biliary pathways. A rapidly performed and correctly indicated external-internal PTD reduces effectively within several days the patient's bilirubin level, the bile passes into the duodenum, dehydration of the organism does not occur and vitamin K absorption is not impaired. All this has an impact on the success of surgery. Moreover, the external-internal drain modified in a suitable way surgical tactics and ensured effective drainage of the biliary pathways during the early postoperative period. If radical surgery is impossible, it is feasible as a suitable alternative procedure of palliative surgery to insert by the percutaneous route an expansible metal stent or leave the external-internal PTD drain in situ for a prolonged period. The authors discuss the criteria for indication, analyze the main problems of this procedure and submit their results.

Modelling tissue behaviour based on hyperelasticity theory.

The tissues are during their physiological function, e.g., in the course of growth, adolescence, and aging, subjected to a cyclic mechanical loading and to large displacements and rotations as well. A tissue free of all external tractions is in a state that minimizes its internal power. In the course of aging of the tissues, for instance in the wall of the aorta, the vein, and also in the myocardium or heart valves, the decrease of the water content and increase of the collagen content occurs; while in compact and trabecular bone the contents of both mineral substances and collagen, undergo reduction. In accordance with it, the strain energy function and the constitutive equations of living tissue based on the hyperelasticity theory using rotationless strain were studied. On the base of the proposed eigenvalue decomposition of the rotationless strain tensor and hyperelasticity the strain energy function was formulated as depending on biological time of tissue. The quantity of strain energy function per unit of the biological time, which essentially characterizes the velocity of change of mechanical response of tissue in the course of its aging, was also defined. The coefficient of tissue aging is the further diagnostic parameter, which is independent of the rotationless strain tensor and expresses the relative change of mechanical response of tissue during the biological time. The corresponding constitutive equation of tissue depending on the biological time is also determined. On the base of the regression analysis the theoretical stress-strain curves for myocardium and blood vessels were determined. The numerical results reveal that the coefficient of aging progressively increases in hardening tissues (coronary artery, vena cava inferior) whereas at the softening tissues it has a relatively slow increase at the dependence on tissue aging.

[Albuminuria after acute oral administration of proteins in patients with renovascular hypertension].

In a group of 19 patients with renovascular hypertension the effect of a morning snack comprising meat (1 g protein per 1 kg body weight) on urinary albumin excretion was assessed. Concurrently the plasma creatinine concentration (Pcr) was examined which varied between normal and 260 mumol/l and the creatinine clearance (Ccr). After administration of an acute protein load the mean Ccr value increased by 23%. The albumin excretion, however, did not change substantially, as compared with the previous collection period (mean 17 and 18 micrograms/min). Microalbuminuria was recorded in 31% of the patients and its prevalence was directly related to the increasing Pcr value. The results revealed that an acute protein load did not increase albuminuria although the rise of Ccr was significant. The variability of albumin excretion in the course of the day is, however, influenced also by other factors and for assessment of microalbuminuria therefore examination of 24-hour urine samples should be preferred.

Urinary albumin excretion in patients with renovascular hypertension.

Twenty-four hour urinary excretion of albumin (UEalb), IgG and beta-2 microglobulin was investigated at a 3 hour-interval in a control group (C) of healthy subjects, in 30 patients with renovascular hypertension (RVH), and in 16 patients with essential hypertension (EH). Mean UEalb in RVH was significantly higher than in C. A significant direct correlation was demonstrated between diastolic blood pressure and UEalb (p < 0.01). Microalbuminuria (MA) > or = 30 micrograms.min-1 was found in about 18% of RVH patients; it was higher than 16.7 micrograms.min-1 in approx. 31%. These results did not substantially differ from those obtained in patients with EH. The cause for increased UEalb in hypertensive patients may be functional, haemodynamic changes, or structural ones. In either case, MA indicates renal injury, and these patients should be given increased attention when monitoring their blood pressure and when selecting antihypertensive drugs.
