Double cortical line in the acetabular roof: a sign of disuse osteoporosis.

A double cortical line (DCL) was observed in the acetabular roofs during interpretation of the abdominal radiographs and intravenous pyelograms of patients with paraplegia and quadriplegia. This was investigated by review of the radiographs of 40 paralyzed patients, by postmortem radiographic and histologic examination of the acetabula of one quadriplegic and three nonparalyzed patients, and by observation of the subchondral cortices of various long bones on radiographs of paralyzed and nonparalyzed patients. The DCL resulted from intracortical bone loss caused by severe disuse osteoporosis. It was most prominent in the acetabulum and most frequently found in patients with paraplegia or quadriplegia. The DCL also occurred in other bones, however, and in other conditions associated with severe disuse osteoporosis.

Should all pregnant women be screened for hepatitis B?

To assess the sensitivity of historical risk factors for identification for hepatitis B surface antigen (HBsAg)-positive parturients, 4399 pregnant women were consecutively screened for HBsAg. Information regarding risk for hepatitis B infection was obtained from each HBsAg-positive parturient. Twenty-three HBsAg-positive subjects were identified (5.2/1000 deliveries). The HBsAg carrier rate (18/2231, or 8.1/1000 deliveries) was significantly higher in women of black, Asian, or Hispanic origin than in the remaining ethnic groups (non-Hispanic whites plus all others, 5/2168, or 2.3/1000 deliveries) (chi square, 5.95; p = 0.016). Risk factors for identification of HBsAg-positive women were present in 10 of 22 asymptomatic subjects (sensitivity, 45%; 95% confidence interval, 24% to 68%). Much of the information required to assess one of these risk factors, previous infection, involved detailed questioning and is unlikely to be obtained in the context of conventional obstetrical care. Routine maternal HBsAg screening programs may be needed if transmission of hepatitis B from mother to infant is to be prevented.

Acute monocytic leukemia presenting in the eyelid. An immunohistochemical and electron microscopic study.

A 28-year-old woman was evaluated for a firm subcutaneous nodule of the left upper eyelid. Results from the examination showed multiple red to brown subcutaneous nodules on her upper extremities and thorax. An excisional biopsy specimen showed the nodules to be cutaneous involvement of acute monocytic leukemia. Acute monocytic leukemia should be considered in the differential diagnosis of a subcutaneous nodule in the eyelid.

Involvement of the mandibular canal: early sign of osteogenic sarcoma of the mandible.

A case of osteogenic sarcoma of the mandible is presented. The most striking radiologic finding in this case was irregular widening of the mandibular canal, with areas of narrowing and loss of fine parallel cortical margins of the walls of the canal. These changes were accompanied by widening of the periodontal membrane of the adjoining teeth, an early sign of osteogenic sarcoma described by Garrington and colleagues, and loss of lamina dura of the same teeth and one additional adjacent tooth.

Acid ingestion in an experimental model.

Following acid ingestion, the mortality of operative intervention is clearly related to late recognition of perforation. Experimentally, fiber-optic endoscopy can be used to accurately predict the depth of gastric injury. Operative intervention can be undertaken before gross perforation. Flexible endoscopy should be performed upon admission and repeated at intervals of 12 to 24 hours until one is satisfied that the injury is stable, reversible and will not progress. The risk of gastric perforation is minimal if the endoscopic procedure is performed by a competent individual. Continued careful medical follow-up examination and nutritional support is indicated when major damage to the stomach occurs to ensure that the patient will tolerate anticipated operative reconstruction.
