[The preventive minitracheotomy as support in difficult intubations: a proposition. Description of a clinical case].

The authors suggest preventive minitracheotomy as a support technique in difficult endotracheal intubation, and report their experience in a case of surgical operation to remove a bony neoformation of C1 and C2 posterior arcs. Endotracheal intubation, difficult because no part of the glottis could be seen, was made possible by preventive minitracheotomy. In this way sedation, myorelaxation and excellent oxygenation during following manoeuvre that resulted atraumatic, were obtained. The small cannula of minitracheotomy was maintained closed during the operation and open for 24 hours thereafter to prevent obstructive complications. It was then removed.

[Estrogen receptors in the lower uterine segment].

The presence and quantity of specific cytoplasmatic receptors for E2 were analysed in the smooth muscle cells of the lower uterine segment in 14 patients (mean age 29.5 years, mean parity 2.9) who had a normal pregnancy and delivered at term. Myometrial tissue (greater than 150 mg) was taken from the median zone of the front wall of the lower uterus during cesarian section with a transversal incision on the lower segment at the level of the vescico-uterine fold. Cesarian section was electively chosen by 10 patients before the onset of labour and was used in 4 cases due to complications arising during labour. In order to carry out the receptor assay 100 ml of cytosolic fraction were incubated for 24 h with increasing concentrations of 0.38 microCu/ml to 2.5 microCu/ml of E2 tired in presence or absence of approximately 100-fold higher concentrations of unlabelled E2. Recorded cpm were compared to a standard reference curve. There was no significant difference between the radioactivity levels of the cytosolic fraction measured as total bound or with non-specific bound. Results are fully discussed in the paper.

[Morbidity-mortality related to domestic accidents in Belgium: Epidemiology and strategy of prevention].

Home accidents, especially those occurring in children in the younger age group, are a considerable concern to the public health authorities. In addition to the informations registered on the death certificates, the EHLASS-program, i.e. European Health and Leisure Accident Surveillance System, was organized since 1986 by the European Community in order to collect data on the morbidity related to this type of accidents. In Belgium, the collection of data started in 1987, and actually four hospitals provide information. Each year, several thousands of cases are reported. This poster will summarize the Belgian situation from the two sources of data. General strategies for prevention of home accidents are proposed, and some types of intervention are suggested.

Heating reactivation of rats aminolevulinic acid dehydratase in lead poisoning.

Heat reactivation (60 degrees C for 5 minutes) of the red blood cells aminolevulinic acid dehydratase activity was studied in lead treated rats (0.25 to 5.0 mg/kg b.w., i.p., for 4 weeks). Complete enzyme reactivation occurs with lead blood concentration up to about 70 mcg/dl. At higher blood lead concentrations, only a part of the enzyme can be restored by heating. The different mechanisms of aminolevulinic acid dehydratase activity inhibition by lead are discussed.

In vivo interaction of lead with aminolevulinic acid dehydratase and induction of a thermolabile factor: an experimental model.

Aminolevulinic acid dehydratase (ALA-D) activity of male albino Wistar rats was used as an experimental model for a study on the interaction of lead with biological systems. Lead at 1 mg/kg was administered i.p. and the rats were killed immediately, and at 30 min, 1, 2, 3, and 4 h after treatment. It was shown that lead (Pb) interacted directly with the enzyme molecule immediately after treatment, first on the active site of zinc (Zn) and then on the thiolic groups. Induction of the so-called thermolabile factor (TF) seemed to occur later, i.e., it may only be shown from the 2nd-3rd h after treatment. The long-term persistence of lead-induced TF in the acute phase of intoxication may be the key to the interpretation of some chronic toxic effects.

[A case of pulmonary eosinophilia with protracted and benign course].

A case of corticosteroid-sensitive pulmonary eosinophilia with protracted course, followed up for more than years, is reported. After highlighting the diagnostic difficulties of such clinical pictures and their as yet ill-defined nosological contours, the comparative benignity of most of such forms is stressed.

[New aspects of the relation between work and pulmonary tuberculosis].

Two new aspects of the relation between work and T.B. are considered: T.B. secondary to psychologically stressing work, and as the result of contagion due to the presence of subjects with poorly marked T.B. symptoms in the work environment. Notes on preventive medicine are offered in the light of a personal series.

[Determinants of depression in 111 Italian patients with systemic sclerosis].

BACKGROUND: A high prevalence of depressive symptoms has been described in systemic sclerosis (SSc), but no clear association with organ involvement or objective indices of disease severity has been depicted. To date, no effort has been made to determine the prevalence of depressive symptoms in Italian patients with SSc or to clarify their cause. METHODS: One-hundred-eleven SSc patients were asked to fill in the Beck Depression Inventory (BDI) questionnaire, the scleroderma Health Assessment Questionnaire (sHAQ) and two additional questions assessing the patient's familiar support and the social consequences of the patient's change in physical appearnace. RESULTS: Thirty-seven subjects (33.4%) presented mild to severe depressive symptoms (BDI >/=17). On univariate analysis the diffuse cutaneous form of the disease (p=0.019), higher pulmonary systolic pressures on echocardiogram (p=0.016), lower FVC percentage of predicted values (p=0.022), higher sHAQ values (p<0.001) or higher VAS values for pain (p=0.007), lung involvement (p=0.02), Raynaud's phenomenon severity (p=0.002), ulcers severity (p=0.006) or disease severity (p<0.001), were associated with the presence of pathologic depressive symptoms. On multivariate analysis only the VAS for disease severity relevant to BDI scores (p=0.016). Social behaviour changes due to SSc-related physical involvement were reported in 14 patients (38%) with depressive symptoms (p=0,006) and were more likely to be observed in younger patients (p=0.001) with a more severe Raynauds's phenomenon (p=0.013). CONCLUSIONS: Mild to severe depressive symptoms are common in SSc patients especially in those with a worse perception of disease severity, these patients should be carefully monitored and a psychological assistance counselled whenever necessary.

[Carpal tunnel syndrome among cashiers in commercial businesses].

The Check-out assistants represent a working sector at risk of Work Related Musculo Skeletal Disorders (WMSDs). The aim of our study is to evaluate carpal tunnel syndrome's incidence in check out assistants. Our search was carried out on a sample of 695 female check-out assistants,after a specific risk's valutazion witch "chec-list application", during health surveillance. Subjects with an pand's painful symptoms or paraesthesias along territory of median were submitted to tests thath revial compression of median at wrist: Phalen and Tinel tests. Who were positive to painful symptoms or paraesthesias in both tests, were subjected to electromyographia (E.M.G.), gold standard for carpal tunnel Syndrome diagnosis. This sample was compared with a control group no-exposed to specific risk (all teachers) of equal number, age, sex and working age of our sample. Our results point out that: our study sample show more symptoms and carpal tunnel Syndrome than the control group; tere aren't statistically meaningful differences between test's positive subjects amd EMG positive subjects. We demonstrated that a careful anamnesis and objective exam can replace EMG.
