The flow rate of whole and submandibular/sublingual gland saliva in patients receiving replacement complete dentures.

Provision of complete dentures to a new denture wearer increases the salivary flow rate is well known. The new dentures act as an additional mechanical stimulus to the salivary reflexes, thus increasing the flow rate. The purpose of this study was to determine whether replacing complete dentures would elicit the same response. Unstimulated and stimulated whole and submandibular/sublingual (SM/SL) saliva were collected before inserting complete replacement dentures and again after 2 days and after 3 weeks of denture wearing. Unstimulated whole salivary flow rate increased significantly 2 days after inserting replacement dentures, decreasing at 3 weeks but remaining significantly above the baseline. Stimulated whole salivary flow rate increased significantly after 2 days but decreased to normal after 3 weeks. Stimulated and unstimulated SM/SL salivary flow rate increased significantly after 2 days, decreasing at 3 weeks while remaining significantly above the values found before denture insertion.

Prevalence of torus palatinus in a population of young and adult Israelis.

The prevalence of torus palatinus, a common exostosis with a debatable pathogenesis, was epidemiologically investigated in a group of 1002 Israeli Jews. This protuberance was observed in 21% of the entire sample, with non-significant differences among different age groups. The smooth type of torus was noted in 72.9%, 68.1% were smaller than 2 cm, and 53.8% were located in the molar area only. The prevalence of torus palatinus in the combined molar-premolar area increased with age, whereas in the molar area it decreased, expressing a significant relation between location and age (p < 0.01). The prevalence of tori larger than 2 cm was much higher in the 21-year and older age groups than in the younger groups. The diverse prevalence of torus palatinus in some of the ethnic groups was also statistically significant (p < 0.05). The aetiology of this common osseous outgrowth is probably multifactorial, including environmental factors acting in a complicated and unclear interplay with genetic factors.

Clinical characteristics and treatment of patients with oral lichen planus in Israel.

Oral lichen planus (OLP) has a multiple-site involvement, with the buccal mucosa being the most prevalent. A possible relationship between OLP and the risk of malignancies in the involved site is documented. The characteristics of OLP have been studied in different populations; however, no similar studies have been conducted in any Jewish population. The purpose of this study was to evaluate the natural history of OLP in Israeli Jewish patients. Data were collected from 157 charts of patients with histologically confirmed OLP. No evidence suggesting a connection between OLP and diabetes, cardiovascular disease, smoking, alcohol use, or positive Candida culture was found. Skin involvement of lichen planus was found in one fifth of the patients. Symptomatic OLP was noted in half of the patients, mainly in those with the erosive form. In 65% of the patients with symptoms, improvement by more than 50% was shown within 2 weeks of steroid use. The transformation rate of 1.3% of OLP into malignancy was observed in the entire group. A constant follow-up for contributing symptoms and for early diagnosis of suspected transformed lesions is of utmost importance.

Tongue, primary amyloidosis, and multiple myeloma.

A case of macroglossia and resulting apertognathia because of primary amyloidosis in a 65-year-old man with multiple myeloma is described. In addition, a retrospective study of the oral manifestations of primary amyloidosis and multiple myeloma in the last 15 years is reported.

Pemphigus vulgaris in adolescence. A case presentation and review of the literature.

Pemphigus vulgaris, a chronic autoimmune vesiculobullous disease, affects people in their fifth or sixth decade of life. Involvement in adolescence is rare with only about 30 cases reported in the last 35 years. This article presents a case of adolescent pemphigus vulgaris in a 15-year-old girl with oral and skin lesions. The patient has been free of lesions with a maintenance dosage of 10 mg of prednisone every other day. The mean age of the 31 patients, including our patient, was 14 years. Skin involvement was reported in 87% of patients, oral involvement in 87%, and skin and oral lesions in 81% with oral lesions preceding skin lesions. Early diagnosis of adolescent pemphigus vulgaris is important to eliminate or reduce the severity of further intraoral or extraoral involvement. The term adolescent pemphigus vulgaris is suggested when the onset is between 12 and 18 years of age.

Efficacy of etretinate (Tigason) in symptomatic oral lichen planus.

This study evaluated the maximum permitted dosage of etretinate (75 mg/day) as the first mode of systemic treatment of patients with symptomatic oral lichen planus. Six patients were treated and examined objectively and subjectively every 2 weeks for 2 months. At the end of the study 50% of patients reached an asymptomatic stage; improvement of clinical signs and symptoms was noted in all patients. However, on reevaluation 2 months after treatment was discontinued some clinical signs of recurrence were seen in all patients. Further follow-up for more than a year showed that the patients had reverted to the same frequency of recurrence as before the systemic use of etretinate.

Periodontal manifestations of adult onset of histiocytosis X.

Histiocytosis X, a reticuloendothelial disorder, includes the three clinical syndromes: Letterer-Siwe and Hand-Schuller-Christian diseases and eosinophilic granuloma of the bone. Twenty-eight cases of adult onset of histiocytosis X involving the head and neck region were found in the literature during the past 20 years; 15 cases were confined to this area, out of which 13 involved jaw bones. Characteristic signs and symptoms of periodontal disease were observed in 22 out of 28 cases (79%) with the lower molar area as a predominant site. A case report of a non-osseous chronic disseminated histiocytosis X with periodontal involvement is presented. The importance of histiocytosis X, as an entity that should be kept in mind as a differential diagnosis of a process where destructive periodontitis is involved, is discussed.

The physiological basis for loss of rootability with age in avocado seedlings.

Rooting ability of cuttings from avocado (Persea americana Mill.) seedlings declined during the first 12 months of development. Cuttings from one-year-old seedlings rooted at a similar rate to cuttings prepared from mature plants. The amount of gibberellin-like substances in the leaves decreased during this time whereas abscisic acid content increased. Application of GA to avocado seedlings induced shoot elongation but failed to maintain the initial, high rooting ability. Treatments with ABA and AM0 1618 had no effect on either rooting or elongation. During the development of roots, the endogenous Avocado Rooting Promoter (ARP) accumulated at a faster rate in juvenile cuttings than in mature cuttings.
