[X-ray computed tomography: examination of choice in cervicofacial pathology? Radio-anatomical correlation. Imagery of the chosen cases].

The authors present their experience of tomodensitometry in cervico-facial pathology. Their work approaches the subject successively from three angles, first comparing 51 cases of tomodensitometric exploration to the classic techniques used in cervico-facial pathology. Next, a correlation is established between the tomodensitometric images and surgical biopsies from 28 pharyngolaryngeal cancers. From the third angle, some selected cases are used to illustrate the contribution by tomodensitometry to the radiology of cervico-facial pathology. From this study, tomodensitometry would appear to be a radiological investigation of choice in cervico-facial oncology for estimating the extent and depth of the tumour, while equalling if not being superior to the traditional radiological techniques for assessing the lesions in this region, whether they be benign or malignant.

Study of lymphoid tissue in paediatric ENT practice.

This study comprises 2 groups of children; allergic (18) and non-allergic (36), having undergone tonsillectomy, adenoïdectomy or both. The humoral immunity profile' bacteriology and neutrophile and eosinophile cell populations of the lymphoid tissues removed were compared. No significant difference was brought into evidence. The bacteriology of the two groups did however show a preponderance of Haemophilus influenzae. This would appear to be in contradiction to the generally accepted theories.

[Personal experience with a myocutaneous flap of the greater pectoralis muscle in reconstructive cervicofacial surgery. Apropos of 27 flaps].

The authors review 27 pectoralis major myocutaneous flaps in head and neck cancer surgery. The indications and applications are discussed. Some technical aspects are pointed out. The results are analyzed and compared with data in the published literature. Because of its numerous advantages, the pectoralis major musculocutaneous flap does establish a real progress in the surgical treatment of head and neck cancer.

Management to prevent high-sited recurrence of pharyngo-laryngeal cancers.

The development of reconstructional techniques in cervico-facial surgery has led to the proposal of surgical solutions, in conjunction with radiotherapy, for the extensive epidermoid carcinomas of the oropharynx and hypopharynx that were formerly rejected by the surgeon. However these stage III or IV epidermoid carcinomas carry a heavy proportion of "high" recurrences, i.e. situated in the parapharyngeal, oropharyngeal and rhinopharyngeal regions. The reasons for this are: the lymphatic continuity between the oro- and hypopharynx and the parapharyngeal regions and the base of the skull; the limitation of neck dissections higher up at the tip of the mastoid and the lower parotid border; the limitation of postoperative irradiation directed upwards. Therefore for these lesions we propose an extended therapeutic programme comprising pre-operative chemotherapy based on cisplatinum, bleomycin and methotrexate, an adequately wide surgical resection followed by reconstruction and an irradiation extending upwards by two lateral fields to the oropharynx, the rhinopharynx and the parapharynx.

[Personal experience in reconstructive subtotal laryngectomy using a sliding epiglottoplasty].

Seven near total reconstructive laryngectomies with epiglottoplasty have been performed by the members of the Saint-Luc ( UCL ) Department of Otolaryngology. The technical aspects are described and the indications and limitations of this procedure are discussed. There is no postoperative complications and the functional results are excellent. The authors think that this procedure must find a place for surgical treatment of laryngeal neoplasms.

Contribution by tomodensitometry to the understanding and diagnosis of benign non-epithelial endolaryngeal tumours. In connection with a case of chondroma, one of lipoma and one of rhabdomyoma.

The authors report on 3 cases of submucosal, non-epithelial tumour of the larynx: one chondroma, one lipoma and rhabdomyoma. A review of the literature is presented. These three cases have benefited by radiological imagery proper to tomodensitometry. This technique is certainly the preferential radiological examination, since it enables an appraisal to be made of the interrelationship of the tumour and its surrounding structures. It gives a probability diagnosis through comparative measurement of the tumorous density.

Surgical management of parapharyngeal cystic hygroma causing sudden airway obstruction.

Parapharyngeal cystic hygroma is a rare tumor of the neck. This report describes two cases in which surgical resection was necessary to overcome sudden airway obstruction and details the surgical technique. These cases were considered "near misses" for sudden infant death syndrome (SIDS) and were revealed by computed tomography (CT) and echography to be parapharyngeal cystic hygroma. The location of this malformation could have produced sudden airway collapse and be erroneously diagnosed as SIDS. The postoperative follow-up was satisfactory and no recurrence was detected. We believe CT and echography should be included in the evaluation of such cases.

Comparison between the GIRBAS Scale and the Acoustic and Aerodynamic Measures Provided by EVA for the Assessment of Dysphonia following Unilateral Vocal Fold Paralysis.

The aim of this study is to establish relevant objective parameters for evaluating dysphonia following unilateral vocal fold paralysis. To do so, the study compares objective and perceptual voice measures. The objective measures were obtained using a voice analysis software (Evaluation Vocale Assistée), whereas the perceptual measures were established with the GIRBAS Scale (grade, instability, roughness, breathiness, asthenia, and strain). All measurements were performed on 40 voice samples: 28 dysphonic subjects with unilateral laryngeal paralysis, and 12 control subjects. The intra- and inter-judge agreements were fairly good, at least for control subjects. The six GIRBAS measures obtained from the pathological voices were higher than those from the control voices (p < 0.001) and the correlation between both groups was good. Grade, breathiness and asthenia correlated with the objective parameters that express the aperiodicity of the phonatory signal (p < 0.01), namely, the coefficient of variability of the fundamental frequency, the coefficient of variability of intensity, and jitter. Our findings suggest that the perceptual reality of laryngeal paralysis-induced dysphonia depends more on grade, breathiness and asthenia than it does on roughness or instability.
