Antiemetic efficacy of escalating doses of alizapride against chemotherapy-induced emesis.

The antiemetic effect of a new benzamide, alizapride, was investigated with escalating doses through four levels starting at 5 mg/kg/cycle up to 20 mg/kg/cycle. 39 patients were accrued who received cancer chemotherapy which included the following drugs in various combinations: cyclophosphamide, adriamycin, fluorouracil, carboplatin and etoposide (VP-16). Complete control of emesis was achieved in a third of the 39 patients. There was no statistically significant difference among the dose levels with regard to the patient's assessment of the incidence and severity of nausea and vomiting. Alizapride was well tolerated at all dose levels tested with minimal toxicity. Mild sedation was reported in 60% of the patients. Neither extrapyramidal reactions nor hypotensive side effects were observed. Thus the therapeutic yield of alizapride could be further studied concerning the optimal dose and schedule as well as its use in combination with other antiemetic drugs.

A randomized study comparing platinum, doxorubicin, and VP-16 with platinum, 4'-epidoxorubicin, and VP-16 in patients with non-small-cell lung cancer.

Forty-four previously untreated patients with advanced non-small-cell lung cancer were treated in a randomized trial comparing platinum (60 mg/m2), doxorubicin (40 mg/m2), and VP-16 (150 mg/m2) (PAV) with platinum (60 mg/m2), 4'-epidoxorubicin (50 mg/m2), and VP-16 (150 mg/m2) (PEV). The overall response rate was 10%. Major response rates were quite similar for the 21 patients treated with PAV (5%) and the 23 patients treated with PEV (18%) (p = 0.2). Of the 23 patients with assigned to PEV, two (9%) achieved complete responses for a median duration of 20 weeks and 44+ weeks. There was no significant difference (p = 0.75) in the median survival among patients treated with PAV (24 weeks) and those treated with PEV (20 weeks). Toxicity was generally mild and tolerable. The lack of response found in both arms of treatment caused the study to be terminated early. Some benefit could be appreciated in patients with limited disease and good Karnofsky performance status.

Evaluation of a protective environment in the management of granulocytopenic patients: a comparative study.

Fever, clinical infection, bacteriologically documented infection, and death from infection were evaluated in 95 consecutive uninfected patients with severe granulocytopenia (less than 0.5 x 10(6)/liter). Patients were randomly allocated to reverse isolation and prophylactic oral nonabsorbable antibiotics or to open ward care. The microbiologic surveillance of air samples and stool cultures showed reduction of pathogenic organisms in patients treated in protective environment units. There was a statistically significant reduction in the incidence of fever (80% vs. 39.6%; P less than 0.001), clinical infections (55.3% vs. 25%; P less than 0.01), bacteriologically documented infections (53.2% vs. 20.8%; P less than 0.01), and deaths from infections (25.5% vs. 8.3%; P = 0.02) in patients treated in a protective environment as compared with patients treated on the open ward.

Oxymetholone effect of acute myeloblastic leukemia cells in vitro.

The possibility that oxymetholone might induce or enhance leukemia after androgen therapy for aplastic anemia prompted us to study the direct action of oxymetholone on the DNA synthesis of AML cells in vitro. The peripheral blasts of 10 patients, 8 with AML and 2 with CML in blast crisis have been studied. The DNA synthesis of the leukemic cells with and without oxymetholone was measured by the 3H-methyl-thymidine incorporation determined by liquid scintillation. The results have been shown a wide variation of DNA synthesis from patient to patient with a range from 2,000 to 40,000 cpm but no significant difference between test and control cultures. We may conclude that oxymetholone does not increase directly the proliferation capacity of the peripheral AML cells cultured in vitro.

Comparative study of cytosine arabinoside therapy alone and combined with thioguanine, mercaptopurine, or daunorubicin in acute myelocytic leukemia.

Three hundred twenty-six patients with acute myelocytic leukemia were randomly and prospectively assigned to four therapeutic regimens: cytosine arabinoside either alone or in combination with daunorubicin, 6-mercaptopurine, or 6-thioguanine. The results in 231 qualified previously untreated patients were analyzed. The combination treatments produced a significantly greater frequency of complete or partial remission than single drug therapy. Treatment with cytosine arabinoside and thioguanine led to 48% age-adjusted complete and partial responses. The median sur survival from diagnosis of all 66 evaluable patients treated with these two drugs was 18 weeks, while the median survival for those who responded to this combination was 15 months.
