[Adjuvant therapy with systemic JM-8 (carboplatin) in superficial bladder tumors. Results after 3 years of use].

We evaluated the 3-year efficacy of adjuvant treatment (after complete TUR) with systemic carboplatin in 34 patients with transitional cell bladder tumors in the superficial stages. Twenty-six patients with a one-year follow-up had a recurrence rate of 23%. Twenty-three patients with a two-year follow-up had a recurrence rate of 65.3%. Six patients with a three-year follow-up had a recurrence rate of 83.3%. Seventeen patients with tumor recurrence within three years had "minimal" recurrence. The course of the disease was very stable and remained unchanged in 4 cases that required no other treatment for one year. This treatment modality indicates a therapeutic effect; however, further studies and changes are warranted in order to obtain better results.

[Use of systemic carboplatin in superficial tumors of the bladder. Preliminary phase II study (toxicity)].

The high recurrence rate of superficial bladder tumors, its frequent association with premalignant tumors, and its evolution into infiltrating tumors in 15% of the cases justify the use of tumoricidal agents as adjuvant therapy. For the past 20 years, several agents have been used intravesically with limited efficacy (+/- 40% recurrence/year), high local morbidity (cystitis, urethral stenosis), and excessively long duration of treatment (+/- 1 year). The efficacy of CDDP in the treatment of bladder tumors has been reported elsewhere. An objective response of +/- 50% (RC + RP) at a dose of 50-70 mg/m2 has been observed. The new generations of platinum, less toxic and easier to use, have raised hopes vis-à-vis its efficacy in the treatment of this tumor type. To date, we have evaluated 26 patients with superficial tumor of the bladder over a period of one year. These patients had been submitted to a treatment protocol with systemic carboplatinum following complete TUR and assessment of bone marrow; renal function, WHO, audiometry, urinary cytology, determination of tumor cell markers (CEA, çGT). Treatment consisted of 3 cycles of i.v. carboplatinum at a dose of 300 mg/m2 (1/30 days). We have corroborated the following features: ease of use, good patient tolerance and scant toxicity. However, its efficacy has as yet to be determined through the disease-free interval.(ABSTRACT TRUNCATED AT 250 WORDS)

[Blind ureters. Proposed classification and study of the national statistics with the contribution of 2 cases].

The authors put forward the bases for a classification of the different kinds of B.U., according to the direction which it takes, the situation of the blind end and the accompanying, renoureteral malformations in the system itself. They review the Spanish statistics in which 38 cases of 6 kinds are compiled, included among the 15 in the classification suggested, as well as presenting two of their own. They refer to the possible ischemic cause of this anomaly.

[Diverticula of the female urethra. Review].

The authors present four cases of diverticuli of the female urethra, and make a review of the subject, basically from the diagnostic and therapeutic points of view. Even though they acknowledge the great usefulness of a clinical exploration which may sometimes provide palpatory pathognomonic signs, they consider essential to perform a urethrography, without overlooking the i.v. urography, due to the image of compression sometimes produced by a subcervical diverticulum on the bladder, which can sometimes be recognized in the cystogram. They also advise a urethroscopy which provides highly valuable additional data. They make a review of recent papers dealing with echographic diagnosis and consider it to be a highly useful method with great prospects. After a short historical examination, they comment the current treatment techniques and indicate the most suitable one in each case. After reviewing the Spanish statistics, they include a total of 26 cases in the present paper.

[Scrotal appendicocele in an infant (clinical note)].

The authors present the case of a 7-month old suckling baby with scrotal appendicocele which simulated a para-testicular tumour. They comment upon the difficulty involved in a preoperative diagnosis and analyse the factors which favour the emigration of the appendix to the scrotum at an early age (persistence of the peritoneo-vaginal duct, non-joining of the peritoneal membrane, the insertion of the appendix into the end of the caecum and increases in the intraabdominal pressure). They point out the need for surgery which will be of a diagnostic-therapeutic nature.

[Partial priapisms. Proximal priapism. Report of a case].

A very rare form of priapism in which the disorder is located in a localized area of the erectile tissue of the penis is presented. We consider this to be the first in Spanish literature and the fifth in the world. According to the review carried out, the most frequent etiology is traumatic, although the history of some cases is vague. Attention is drawn to the diagnosis, and therapy, which was successful in every case.

[Urotelial cystosis. Review of the national statistics with a personal contribution].

After a short explanation of the concept of urotelial cystosis, the authors compile all the cases which have been presented under the name of cystic ureteritis by Spanish urologists up to 1979, to which they add one more, thereby bringing the national statistics to 31. They comment upon the Spanish contribution to the subject in the clinical field as well as the field of etiological research.
