Human leukocyte antigen in hydatidiform mole.

OBJECTIVE: To find any possible association between human leukocyte antigen (HLA) as a genetic marker and the development of hydatidiform mole (HM). METHOD: The lymphocyte microtoxicity technique was used to study the HLA antigens in 3 groups: 18 women with hydatidiform mole and their husbands (study group), 300 normal women (first control group), and 20 normal pregnant women and their husbands (second control group). RESULT: Hydatidiform mole is more frequent in women having HLA A1. The disease is more common in couples with partial compatibility of the B locus. CONCLUSION: HM occurs more frequently in women with certain HLA antigens and when the husband and wife have partial compatibility at the B locus.

Hemoperfusion--state of art.

The authors make a general review of the state of art in HP with particular reference to the history, the main field of application, and the development of specific and selective sorbents. In this text, attention is paid to the application in the immunological field comparing the therapeutical removal of toxins with different sorbents. Next, the authors describe their experimental study with hemoperfusion using coated anion exchange resins for the treatment of acute respiratory insufficiency, the correction of metabolic alkalosis in pyloric stenosis, the removal of a high dosage of antiblastic agent in conjunction with the study of heparin subtraction by coated resins and finally the development of a particular type of resin for phosphates removal in uremia. These different fields of research recognize the necessity to look for a selective exchange by hemoperfusion because the concept of the "magic filter" that removes everything is not scientifically acceptable. Some explanations are given for the behaviour of the strong anion exchange resins regarding simple or complex ions and the influence of the chemical composition of the matrix. In conclusion, the authors underline that selective removal by HP is actually underutilized considering its potency.

[Neonatal listeriosis. A case report].

The author describes a case of early neonatal listeriosis following by a positive outcome. The severity of this infection is underlined as well as the need to examine carefully the role of listeria monocytogenes as a cause of similar influenza-type syndromes in pregnant women.

[Apropos of preventive ophthalmologic interventions in the child population of compulsory school age. Authors' experience].

The results are reported of work carried out during the last five years by the Center of Social Ophthalmology, director Prof. G. Scuderi, which is part of the II Division of the Ophthalmologic Clinic of Rome University "La Sapienza" with the object of trying out together with territorial units a model of preventive intervention acting as a filter for specialized university facilities. In other words, already existing facilities and medical personnel were to be used and trained for specific methods and techniques. From 1985 through 1990, 787 children selected by ophthalmologic screening by school doctors among the compulsory school population of the RM 3 and 5 districts were seen and submitted to periodic checks. This cohort included children aged 5 to 14 without significant differences in sex distribution and with prevalence of the 8 to 10 year olds. Each subject was submitted to complete eye examination including refractometry and orthoptics, color vision, biomicroscopic and ophthalmoscopic examination. The principal findings show that about 36% of subjects examined have a natural vision of 11/10 while the high percentage of ametropia observed was mainly represented by astigmatism and hypermetropy, albeit of minor intensity. Strabismus was found in 2.79% and implied marked visual deficit that was often refractory to any type of treatment. In addition, the prevalence was determined of other abnormalities concerning eye motility, color vision and pathologies involving anterior segment and eyeground. In their conclusion, the authors stress the rarity of organic ocular pathology in children (mostly represented by inflammatory changes of the adnexes and conjunctiva) whereas amblyopia was one of the main causes of visual deficit in children. They stress the importance of early therapeutic intervention in order to allow complete visual rehabilitation. The results of the clinical model for prevention were excellent so that it can even be proposed for large scale prevention of amblyopia.

[Experimental evaluation, in vitro and in vivo, of the risk of infection related to the use of the most common surgical sutures].

Many parameters contribute to surgical infections. Of these, surgical sutures with their chemical and physical characteristics like tissue and cell reaction degree and bacterial adhesion, play an important role. Cell vitality, bacterial adhesion, cell reaction degree were tested "in vitro" and tissue reaction with an "in vivo" model on: plain catgut, silk, polyglycolic acid, polyglactin 910, polyglyconate, polypropylene and nylon. Vitality cell test did not reveal any cytotoxicity; bacterial adhesion showed a lower adhesion degree to synthetic materials; cell reaction showed an increase in soluble receptor (sIL 2R) values in all materials except catgut. Tissue reaction was higher for resorbable sutures, and lower for nylon. These results suggest that special attention should be paid to the choice of suture materials.

[The oxidative burst in the determination of immune response to synthetic materials].

Macrophages play an important role at the implant-tissue interface. Their activity is extremely complex but above all they are phagocytic cells. Phagocytosis is accompanied by a sequence of biochemical reactions known as "the oxidative burst" (BO). The fundamental physiological significance of this phenomenon is to provide the phagocytes with an efficient microbicidal mechanism. Our aim was to prepare an objective method for screening biomaterials that could influence macrophagic defence mechanisms against a possible implant contamination.

[In vitro study of Staphylococcus epidermidis adhesiveness to suture materials].

In surgical divisions, where you can find the highest percentage of hospital infections, the surgical wounds are definitively the most frequent localization. The aim of our study was to evaluate whether bacterial-biomaterial interactions could influence the growth of infections in sutured tissues. We prepared a method for in vitro bacterial adhesiveness testing based on turbidimetric analysis. The results obtained show a different grade of bacterial adhesion in relation to physical and chemical characteristics of the material examined.

[Influence of suture materials on cell viability: a comparative experimental study].

Among the conditions that a material must satisfy to be considered biocompatible, most important is the non negative influence on the cellular viability of the surrounding tissue (cytocompatibility). The aim of our work was to evaluate in vitro, whether different types of materials (usually used in surgery), alter the viability parameters or the morphological characteristics of the cultured cells in contact with them.
