Antigenic and genetic structure of Borrelia burgdorferi.

Lyme borreliosis is a disease caused by the spirochaetes Borrelia burgdorferi, Borrelia afzelii and Borrelia garinii and it is transmitted by ticks. Most of the proteins (outer surface proteins, flagellar proteins and other uncertain location proteins) have a strong antigenic variability. Osp A protein genetic and serological studies facilitated the differentiation of seven serotypes strongly correlated with the known genospecies. The genetic structure of these spirochaetes included a large linear chromosome, several linear microchromosomes as well as a number of circular plasmids.

[Echocardiographic study of hypertrophic myocardiopathy in children. Distribution and changing morphological course].

Thirty-eight children with diagnosis of hypertrophic cardiomyopathy were analyzed by M-Mode, two-dimensional and Doppler. The patients ranged in age from 7 days and 20 years (average 5 years). Three groups appear, based in morphologic myocardial affection. Patients with biventricular involvement ranged in age from 20 days and 6 months (group 1), patients aged until 6 years with left ventricular concentric morphology by echocardiographic examination (group 2), and the older in age group (group 3) with localized mid-ventricular hypertrophy in the same ventricle. A certain degree of obstruction was detected in 15 cases (46.8%). In both this aspect and systolic anterior motion of mitral valve and mitral regurgitation, the highest frequency was obtained in group 3. With respect to others echocardiographic parameters, like isovolumetric relaxation time and left ventricle short fraction, no variations have been observed between the three groups. Very interesting is the fact that one third of the patients of group 1 improved markedly, and another third of cases died. In conclusion, the echocardiography permits identify three groups in the hypertrophic cardiomyopathy in children, according to age and anatomic region affected. The variability of evolution in the youngest group of patients, suggest that certain cases are not true myocardiopathies, but myocardial hypertrophies secondaries to other problems. Only the oldest group supply the classics parameters of the adult hypertrophic cardiomyopathy.

[The presence of anti-Chlamydia pneumoniae antibodies in peripheral vascular and neurological disorders].

OBJECTIVES: To make a retrospective analysis of the synthesis of antibodies to the MOMPS and LPS antigens of Chlamydia pneumoniae in patients with occlusive disease of the peripheral arteries (ODPA) and multiple sclerosis (MS). PATIENTS AND METHODS: We studied 190 samples of plasma from patients included in the following groups: group 1:66 samples from 66 patients with ODPA; group 2:74 samples from 31 patients with MS (20 remittent-relapsing and 11 secondarily progressive), followed over time; and group 3:50 samples from persons acting as controls. In all cases determinations were made using ELISA, of the IgG anti-MOMP and the IgG and IgA anti-LPS. Comparison of the continuous variables was made using the Mann-Whitney U Test. Discrete variables were analysed using the exact bilateral Fisher Test. The Wilcoxon Test over ranges was used to compare the evolution of antibodies in the patients with MS. RESULTS: The percentage of positive results in groups 1 to 3 for anti-LPS IgG were: 24.6%, 18.9% and 20.8%, respectively, with no differences between patients and controls; nor were there any differences with IgA (29%, 29.7% and 25%, respectively). However differences were seen in the anti-MOMP IgG between patients and controls (group 1:80.3%, group 2:37.8% and group 3: 33.3%). In patients with MS the results of the evolution of the antibodies did not reflect a uniform tendency of the levels of the different antibodies. CONCLUSION: A higher level of IgG anti-MOMP was seen in ODPA and MS, although this did not occur with anti-LPS or IgA.

[Mucinous adenocarcinoma of the prostatic gland. Histochemical and immunohistochemical studies].

We report a case of mucinous adenocarcinoma of the prostate gland in an 82-year-old patient who consulted for urinary discomfort. Rectal digital examination revealed a smooth tumor in the left lateral aspect. No bony metastases were observed and the prostate acid phosphatase levels were normal. The histological analysis revealed the typical group of tumor cells in abundant mucin with acid and neutral component disclosed by histochemical methods. The immunohistochemical analysis revealed the prostatic origin of the neoplasm, with tumor cell cytoplasm strongly positive for both prostate specific antigen and prostate acid phosphatase. This histological variant accounts for approximately 0.4% of prostatic adenomas. Only 50 cases have been reported in the literature.
