[Odds ratio or prevalence ratio? Their use in cross-sectional studies].

BACKGROUND: The most commonly used measures of association in cross-sectional studies are the odds ratio (OR) and the prevalence ratio (PR). Some cross-sectional epidemiologic studies describe their results as OR but use the definition of PR. The main aim of this study was to describe and compare different calculation methods for PR described in literature using two situations (prevalence < 20% and prevalence > 20%). MATERIAL AND METHODS: A literature search was carried out to determine the most commonly used techniques for estimating the PR. The four most frequent methods were: 1) obtaining the OR using non-conditional logistic regression but using the correct definition; 2) using Breslow-Cox regression; 3) using a generalized linear model with logarithmic transformation and binomial family, and 4) using the conversion formula from OR into PR. The models found were replicated for both situations (prevalence less than 20% and greater than 20%) using real data from the 1994 Catalan Health Interview Survey. RESULTS: When prevalence was low, no substantial differences were observed in either the estimators or standard errors obtained using the four procedures. When prevalence was high, differences were found between estimators and confidence intervals although all the measures maintained statistical significance. CONCLUSION: All the methods have advantages and disadvantages. Individual researchers should decide which technique is the most appropriate for their data and should be consistent when using an estimator and interpreting it.

Anaphylaxis to Linum.

BACKGROUND: Flax (Linum usitatissimum) seeds are increasingly used in bread and as laxatives. Hypersensitivity to linseeds has been infrequently described, and we report a case of anaphylaxis induced by linseed ingestion in a 39-year-old woman. METHODS AND RESULTS: The clinical course, as well as positive skin prick tests and histamine release tests performed with linseed extracts, suggested a type I hypersensitivity as the underlying cause for the patient's multisystemic involvement. The presence of linum-specific IgE in her serum was confirmed by immunoCAP assay. CONCLUSION: Linum seeds might be a source of allergic sensitization that should be taken into account due to its widespread distribution at health food stores.

Montelukast in early childhood asthma. Predict value of IgG in clinical reply in children 2 to 5 years old?

BACKGROUND: According to current knowledge, asthma is basically an inflammatory process. Its causes and physiopathological mechanisms are various. The final result is a recurrent obstructive bronchial process, with sibilants and/or dypnea, which causes an upset in functional respiratory tests, among which the maximum respiratory peak flow meter diminished for the age, sex, and height of patient. AIMS: Our aim is to evaluate if response to treatment with Montelukast has any link with immnuloglobin values (IgG, IgA, IgM, IgE) at start of treatment. MATERIALS AND METHODS: Included in the study were 32 children, of whom 2 did not begin and 1 who did not provide personal data. There were 29 patients in total, 11 girls and 18 boys. Each made three visits: first where they were instructed, together with their parents, in how to manage the meter and where they received the peak flow meter, Vitalograph, and personal data sheet, where personal and family medical history were noted. The second visit was after 4 weeks, for a clinical assessment and the third visit after 8 weeks. The value register of the PEF would be made morning and night, noting the highest value of three measurements. IgG, IgA, IgM, IgA values were quantified before treatment began. The statistic package STATA 2001 was used in the treatment of data statistics. RESULTS: Our between the value reached by the PEF after treatment and the IgG values at the beginning of treatment (0.712). In lesser measurement for IgA values (0.660). For each 100 mg/ml of increase in the value of IgG, an increase of 10 l/min in the PEF measurement before and following treatment with Montelukast was produced. CONCLUSIONS: IgG values increase with age. Children with a greater IgG value at the beginning of treatment reached higher PEF values after same. It is not known if the results would be similar with another type of treatment and the way in which IgG influences the results. What appears to be confirmed by available studies is that this relation is found in a group of small children, the aim of our study.

Radiochemical analysis of 93Zr.

The zirconium isotope 93Zr is a long-lived pure beta-particle-emitting radionuclide, which is produced by nuclear fission and neutron activation of the stable isotope 92Zr. This element is a constituent of the structural components of nuclear reactor vessels. A selective liquid-liquid extraction method for radiochemical separation of Zr, based on liquid-liquid extraction with 1-(2'-thenoyl)-3,3,3-trifluoroacetone in xylene and a subsequent stripping of 93Zr by an aqueous acid solution, has been developed. The method was utilised to separate Zr from other pure beta-particle and beta-gamma emitters in different kinds of samples. Decontamination factors higher than 99% for the pure beta-particle and beta-gamma emitters and an overall chemical yield of 80% were obtained. The sensitivity of the method allows the determination of the isolated 93Zr by liquid scintillation counting and the minimum detectable activity value obtained was 0.067 Bq over a counting period of 60 min.

Microglandular hyperplasia of the uterine cervix. Cytologic diagnosis in cervical smears.

OBJECTIVE: To identify cytomorphologic features specific to microglandular hyperplasia (MGH) in cervical cytologic smears. STUDY DESIGN: Twenty-four cervical smears from 24 patients obtained before the histologic diagnosis of MGH made on colposcopically directed biopsies during a period of two years (1995-1997) were evaluated. RESULTS: Of cases with MGH, 13/24 (54%) showed the presence of bidimensional or tridimensional cellular clusters made up of cubic or cylindrical glandular cells with vacuolated cytoplasm; cells with dense cytoplasm, basaloid in appearance, corresponding to immature squamous metaplasia; and subcylindrical reserve cells with small, round nuclei and scant cytoplasm. Clusters showed microlumina or fenestrated spaces, preserved polarity and absence of nuclear peripheral dispersion. In the control group these cellular clusters were found in 6/100 (6%). Statistical analysis (chi 2) showed a strong, highly significant association (P < 0.001) of the cytologic parameters selected and the histologic diagnosis of MGH. CONCLUSION: Until now, no specific cytologic parameters were described for MGH. This study underscored the value of cytomorphologic parameters described for typical cellular clusters showing microlumina or fenestrated spaces with shared party walls and an admixture of glandular cells, and immature squamous metaplastic and subcylindrical reserve cells in the cytologic diagnosis of MGH.

Prospective comparative study of 3 treatment protocols using doxycycline or imidocarb dipropionate in dogs with naturally occurring ehrlichiosis.

This paper reports the clinicopathologic responses of 93 dogs with spontaneously occurring ehrlichiosis to 3 different treatment protocols. Thirty-two dogs were treated with doxycycline (10 mg/kg/day for 28 days), 31 were treated with imidocarb dipropionate (5 mg/kg given 15 days apart in 2 separate injections), and 30 were treated with both drugs simultaneously, at the doses as specified. The dogs underwent clinicopathologic evaluation before and after treatment, and were examined periodically during the 24-month period after the treatment. No differences were found in the clinical responses among the dogs in the 3 treatment groups. As for the clinicopathologic response, in spite of the fact that at the end of the study the results obtained with the 3 protocols were similar, the platelet count and serum protein electrophoresis results returned to normal more slowly in dogs that received imidocarb dipropionate as compared to those given the other 2 treatments.

EEG coherences in patients with brain lesions.

EEG coherence between all pairwise combinations of the 10/20 system was studied in two groups: control subjects and patients with space-occupying brain lesions. Coherence was separately computed for the delta, theta, alpha and beta bands. Comparisons between both groups showed highly significant differences in all bands. In the patients with lower coherence values, these differences were mainly in the alpha band. When intra-hemispheric coherences between controls and patients with left hemispheric lesions were compared, significant differences were observed only in the left hemisphere. Comparisons between controls and patients with lesions in the right hemisphere also showed significant differences only in the injured hemisphere. In patients, paired t tests between the values of the intra-hemispheric coherence in the left hemisphere and the corresponding values in the right hemisphere demonstrated that lower coherences were confined to the damaged hemisphere. Our conclusion is that in the presence of lesions disrupting cortex and adjacent white matter the coherence between this area and the remaining cortical areas is lower than normal due to impairment of the fibers that connect the damaged area with the rest of the brain.

["Neurorestorative" cell therapy in Parkinson's disease: an unresolved debate].

At present there is great enthusiasm over the perspectives deriving from so-called cell therapy in Parkinson's disease. This enthusiasm has spread beyond the ambit of the medical community, reaching the general public, and has been fuelled by a considerable ethical and political debate, sidestepping the need for a really scientific analysis of the real qualities and limitations of treatment with stem-cells in neurodegenerative diseases. Parkinson's disease is frequently observed from a simplistic perspective, as a mere neurodegeneration of the nigrostriatal dopaminergic pathway. This viewpoint encompasses different designs that tend to replace the lack of dopamine in the striatum through the use of different types of cell therapy. In this respect, it is important to indicate, on the one hand, the multisystemic and generalised nature of the disease and, on the other, the progressive character of the neurodegenerative process of Parkinson's disease. With this approach, to claim that the mere replacement of striatal dopamine through replacement cell therapy can correct the generalised and progressive character of the disease is a fanciful aspiration, which can only contribute to generating unfounded expectations in the general public. This article attempts to set out from a purely scientific point of view the doubts over the expectations created by these new therapeutic designs.

[Utility of IgG subclasses in the diagnosis and follow up of hydatidosis].

BACKGROUND: The aim of the study is to evaluate the IgG subclasses for the diagnosis and follow-up of hydatidosis disease. MATERIAL AND METHODS: 82 serum of 50 patients previously diagnosed of hydatidosis disease, were studied. This patients were divided into different groups depending on their symptomatology, and 10 serum of healthy individuals were tested for an indirect hemagglutination, and IgG subclasses were studied in all positive cases by an ELISA. RESULTS: IgG1 subclass was positive in 81 out of 82 patients. IgG2 and IgG3 subclasses were positive in 94.4% of symptomatic patients, in 100% of patients with calcified cyst or who had had no radical surgery, and in 0% of patients with calcified cyst or who had had radical surgery. CONCLUSION: IgG1 and IgG4 subclasses can be used together for hydatidosis diagnosis, due to their high specificity and sensibility. IgG4 subclass becomes negative soon if there is a good clinical response by the patient, turns to be positive when there is a disease recrudescence, and keeps positive when residual cyst are left. All this makes IgG4 subclass a good marker in the hydatidosis follow-up.

[Chorea as the presenting form of progressive systemic sclerosis].

We present an exceptional case of a patient with hemichorea of ischemic origin as a form of presentation of progressive systemic sclerosis. The neurologic complications, both central and peripheral, of this condition are considered infrequent and may be due to coexisting or iatrogenic factors, or may be a direct complication of progressive systemic sclerosis. There have been occasional reports of cerebrovascular disease in progressive systemic sclerosis, and this apparent rarity may be related to structural and morphological variations in brain arteries. Recently, there have been reports of primary cerebrovascular changes and vascular calcifications in progressive systemic sclerosis. There are no reported cases associated with movement disorders. The patient is a 66-year-old ex-smoker, and no other vascular risk factors, with chronic Raynaud's phenomenon affecting both hands, and polyarthralgias. Three months previously he began to complain of choreic movements affecting the right-sided limbs without any other neurological manifestations. The abnormal movements responded to haloperidol. There were skin changes suggestive of progressive systemic sclerosis, and capillaroscopy showed abundant avascular zones with dilated capillary loops. Anticentromere antibody titer was elevated to 1/1280. Brain arteriography was normal, and the brain SPECT showed bilateral hypoperfusion bilaterally, predominantly on the left side. Brain MR imaging confirmed an infarct in the left caudate and adjacent internal capsule. The study of this case supports the hypothesis of a direct primary cerebrovascular damage in progressive systemic sclerosis.

[Chlamydia pneumoniae and cerebrovascular disease].

INTRODUCTION: Chlamydia pneumoniae has relationship with artherosclerosis of carotid artery by seroepidemiological studies and by demonstration of the bacteria in atheromata. We made a case-control study to know the seroprevalence of chronic infection of C. pneumoniae and the presence of the bacteria in arterial biopsies. PATIENTS AND METHODS: The cases group was constituted by 26 patients undergoing carotid surgery. In the control group there were 50 patients without atherosclerosis and who underwent removal of their varicose veins. There were matched for sex, age and smoking. We obtained serum samples to determinate IgG antibodies against MOMP by MIF and ELISA. In the cases group, we got the arterial biopsies from carotid artery, and from pudendal arteries in control group. We determined chlamydial DNA on the biopsies by heminested PCR (primers: HL-1, HM-1, HR-1). We measured fibrinogen in both groups. RESULTS: By MIF technique, the seroprevalence was (IgG > 1:32) 69.23% cases versus 24% controls (OR: 7.12, CI: 2.47-20.48). The ELISA showed 76.92% of seropositivity in cases versus 16% in controls (OR: 17.5, CI: 95%: 5.35-57.23). The DNA of C. pneumoniae was found in 18 cases and 6 controls, (p < 0.0001, chi 2). We did not find any relationship between fibrinogen levels and groups. CONCLUSION: We think that there is a relationship between chronic infection with C. pneumoniae and carotid atherosclerosis.

[Effect of folic acid supplementation on total homocysteine levels in hemodialysis patients].

UNLABELLED: Hyperhomocysteinemia is an independent risk factor for cardiovascular mortality in ESRD, but about 80% of total homocysteine (tHcy) is bound to albumin (alb). We have tried, prospectively, to reduce tHcy levels by using folic acid (f.a.) and vitamin B6 (P.P.) supplementation. All patients on HD, not receiving f.a. or P.P. and all new patients, after their third month on HD, were supplemented with f.a. 5 mg/48 hours p.o and P.P. 40 mg/week. We determined folate, P.P. (RIA), vit. B12, KTV, residual renal function (KRU), PCRn, alb and tHcy levels (HPLC). 80 patients, age 62.6 +/- 13.6 years, time on HD 16.2 +/- 25.1 months, all dialysed with AN69 or PPMA, and bicarbonate, were included. The prevalence of hyperhomocysteinemia was 84.4%, and P.P. deficit was present in 32%, with folate in the low normal range. At the beginning of the study, before supplementation, tHcy was negatively correlating only with folate (r = -0.336) (p = 0.01), and not with P.P., vitamin B12, age, albumin, KTV, KRU or PCRn. 58 patients received six months of supplementation, with normalization of P.P. levels, a significant increase of folate (7.25; I.C = 95% confidence intervals: 6.45, 8.05 vs 61.29; I.C.: 44.47, 78.11) (p < 0.001), and decrease of tHcy (24.1; IC: 21.5, 26.3 vs 19.9; I.C: 17.5, 22.4) (p < 0.05). 33 patients have received 12 months of supplementation, but in spite of a continued increase of folate (100.78; I.C: 74.81, 126.74) (p < 0.001), only 3 have normal levels of tHcy; correlating directly tHcy with albumin (r = 0.56) (p = 0.001), that had increased compared to the beginning of the study (3.39; I.C. 3.29, 3.49 vs 3.50; I.C: 3.37, 3.63) (p < 0.05). CONCLUSION: After f.a. and P.P. supplementation, though initially tHcy is reduced, this response is short lived, and tHcy directly correlates with albumin levels. Good nutrition associated with HD adequacy, in absence of B vitamin deficits, seems to be the best determinant of tHcy levels rather than its removal by dialysis tHcy levels should be interpreted taking into account the serum albumin.
