[Treatment of postoperative pain with intravenous PCA system. Comparison with morphine, metamizole, and buprenorphine].

OBJECTIVES: To evaluate the postoperative analgesic efficacy, side effects and acceptance by patients and nurses of intravenous "patient-controlled analgesia" (PCA) with morphine, metamizole and buprenorphine. MATERIAL AND METHODS: In this randomized double blind prospective study of 150 patients in three groups receiving morphine (group A), metamizole (group B) or buprenorphine (group C), the patients had undergone low abdominal surgery with the same anesthetic protocol. Pain was recorded during the first 48 h after surgery on an orally-communicated scale of none or slight = 0, moderate = 1 and severe = 2. Upon the first report of moderate pain, patients were administered an intravenous bolus containing 5 mg morphine, 1 g metamizole or 0.15 mg buprenorphine. A perfusion pump was then connected and set with one bolus of 1.2 mg morphine, one of 333 mg metamizole or one of 0.04 buprenorphine. The maximum dose allowed in 24 h was 40 mg morphine, 8 g metamizole or 1.2 mg buprenorphine. The minimum interval between doses was 30 min for all three groups. Side effects reported were respiratory depression, sedation, nausea, vomiting, pruritus, perspiration and pain upon administration. Patients and nurses were asked to evaluate the system when the pump was disconnected and the results were then analyzed statistically. RESULTS: The analgesic effect was satisfactory in all three groups, with no significant differences among them. The percentages of patients reaching the maximum allowed dose on the first day were 2% with morphine, 18% (p < 0.05) with metamizole and 8% with buprenorphine. No respiratory depression was observed. Sedation was greater with morphine and buprenorphine than with metamizole (p = 0.0001). Pruritus was also greater with morphine and buprenorphine than with metamizole (p = 0.02) and pain upon infusion was greater with metamizole (p = 0.0002). CONCLUSIONS: Intravenous postoperative PCA was effective with all three drugs studied. Patient and nurse acceptance was good and side effects were few in the three groups. The lower rate of side effects for metamizole makes it the drug of choice.

[Comparative study of 3 types of lancets for performing prick tests].

Skin Prick Test is considered the cutaneous test of choice in rutinary clinical practice as well as in research. In the last 15 years a large number of lancets for puncture test have been developed looking for a higher level of standardisation and reproducibility. We have compared three lancets, two of them, NeoAbelló (NA) and Dome/Hollister Stier (DHS), used in puncture test and a third one, Blood Lancet (BL), in the modified prick test. Twenty-two patients sensitised to Phleum pratense were tested with the three lancets at three extract concentrations and histamine 10 mg/mL in duplicate. We compared their wheal sizes by means of parallel line assay, their reproducibility, frequency of bleeding wheals, the disturbance for the patient and the correlation between specific IgE and skin response. Wheal size was similar for puncture tests (DHS and NA lancets) and significatively lower than BL, being necessary to increase 1.9 times the extract concentration when using DHS and NA lancets to achieve the same wheal size than when using BL lancets (p < 0.01). Reproducibility with histamine was similar with the three lancets while with the extract was somehow better with puncture tests but without statistical significance. Bleeding was significantly more frequent with BL although this did not influence the results. Less pain was obtained with BL. Finally, correlation with specific IgE was slightly significant for NA and very similar for the other two. With these results in mind we think that any of the three lancets is valid for the diagnostic of the allergic patient. Nevertheless, puncture tests are probably the most suitable at a research level.

[Metastatic disease of the liver: surgical perspective].

Approximately half of patients with colorectal cancer will develop hepatic metastases and it is estimated that up to 10% of that group will have resectable liver disease. Surgical resection remains the first line treatment option of metastatic liver tumors and has yielded a 20 to 40% five year survival rate. Selection of appropriate patients for resection is critical to a successful outcome. The best results are obtained in patients with isolated metastases. Factors that are associated with a poorer results are the presence of four or more lesions or a surgical margin less than 1 cm. Endocrine metastases can be resected in a palliative fashion but each case has to be individualized. This is also true for non colorectal-nonendocrine metastases. For this tumors the experience is anecdotal and confined to limited reported series. Adjuvant treatment (infusional chemotherapy and chemoembolization) can also have a role in treatment as well as cryotherapy.

[Validation in Spain of the dysfunctional ventilatory response to weaning].

In this article we present the study financed by the Sanitary Investigations Fund (FIS), with the objectives to prove that the Nursing Diagnosis of "Dysfunctional Ventilatory Response at Weaning" (DWRU) incorporated to the taxonomy of the North American Nursing Diagnosis Association (NANDA), is valid in Spain; as well as identify the items which configure its definition. A descriptive observational study was performed in nine Intensive Care Units belonging to six autonomous communities, including as study subjects 89 male and female nurses who are experts in yhe care of critically ill patients with mechanical ventilation. The model of validation proposed by Fehring and the Delphi Technique were used. The data were analysed calculating the average of the weights of each defining feature; percentage and average calculus; opinion percentages about the related factors; variability calculus; and factorial analysis of the result. The results make us conclude that Spanish nurses who have experience in taking care of critically ill patients, identify DWRU as a phenomenon which patients in our environment present and which can be treated by them. Besides, the identification of levels of DWRU into Severe, Moderate and Slight which the NANDA suggests, is better applied using an association of factors than defining higher and lower features.
