[Our experience of parenteral nutrition (PN) in general surgery].

The problems caused by malnutrition in postoperative complications are well know as is the importance of parenteral nutrition (NP). Thanks to the improvement in instruments and methods, NP can be used not only in intensive therapy departments, but also in less well equipped ones. The Authors report their experience of the use of NP in 77 patients from January 1978 to December 1989 treated in the Surgical Department of the Civil Hospital of Susa (TO), USSL 36, Regione Piemonte.

Screening for fetal Down's syndrome with maternal serum markers--an experience in Italy.

The effectiveness of maternal serum alpha-fetoprotein, unconjugated oestriol, and human chorionic gonadotrophin in screening for Down's syndrome (DS) was evaluated on 840 women who underwent amniocentesis for fetal karyotype on account of their age. The risk of a DS pregnancy was established using the method of Wald et al. (1988b), which combines the age-specific risk with that indicated by the levels of the three serum markers. In women over 35, at cut-off risk levels of 1:250 and 1:380, the false-positive rate was 24 and 34 per cent, respectively. In all nine cases of DS, the estimated risk was higher than 1:250. The best screening strategy with the lowest false-positive rate was obtained by combining the three serum markers. The results suggest that this kind of screening can be proposed during genetic counselling for women under 35 and older women wishing to avoid the risk of miscarriage induced by amniocentesis.

Vascular cell adhesion molecule-1 mediates lymphocyte adherence to cytokine-activated cultured human endothelial cells.

The expression and function of a new cytokine-induced endothelial cell adhesion protein, vascular cell adhesion molecule-1 (VCAM-1), was characterized in vitro by using a monoclonal antibody, MoAb 4B9, which recognizes a functional epitope on this protein. As determined by enzyme-linked immunosorbent assay and radioimmunoprecipitation of metabolically labeled cells, VCAM-1 was minimally expressed on unstimulated human umbilical vein endothelium (HUVE), but was rapidly induced by recombinant human tumor necrosis factor-alpha (rhTNF-alpha), rh interleukin-1, and lipopolysaccharide. In contrast to intercellular adhesion molecule-1, VCAM-1 was not induced on dermal fibroblasts or arterial smooth muscle cells after stimulation with rhTNF, or on keratinocytes after stimulation with rh interferon-gamma. MoAb 4B9 significantly inhibited the adherence of peripheral blood lymphocytes (PBL) and lymphocytic cell lines, but not neutrophils, to rhTNF-activated HUVE. The inhibitory effect of MoAb 4B9 on PBL adherence to HUVE was additive to that produced by the CD18 MoAb 60.3. These results show that VCAM-1 mediates a CD18-independent pathway of peripheral blood lymphocyte adherence to cytokine-stimulated HUVE. We propose that lymphocyte binding to VCAM-1, induced on endothelium by cytokines, may be an important component of lymphocyte emigration at sites of inflammation or immune reaction.

Investigations into the relationship of the PPVT-R and the WISC-R with incarcerated delinquents.

Examined the relationship between the Peabody Picture Vocabulary Test-Revised ( PPVT -R), the Peabody Picture Vocabulary Test ( PPVT ), and the Wechsler Intelligence Scale for Children-Revised (WISC-R) with 35 incarcerated delinquents between the ages of 13-10 and 16-10. Mean scaled scores were computed across all measures. A statistically significant difference (p less than .01) between the PPVT -R mean scaled score and all other measures was obtained. The PPVT -R correlated significantly (p less than .0001) with the WISC-R VIQ (r = .87), PIQ (r = .78), FSIQ (r = .86) and the PPVT (r = .80), whereas the original PPVT demonstrated significant yet lower correlations with the WISC-R VIQ (r = .78), PIQ (r = .77) and FSIQ (r = .80). The clinical significance of utilizing the PPVT -R as a measure of receptive vocabulary and its practical relationship to the PPVT and WISC-R with a juvenile delinquent population was discussed.

Implications of regression analysis and correlational data between subtests of the WISC-R and the PPVT-R for a delinquent population.

Computed correlations between the subscales of the Wechsler Intelligence Scales for Children-Revised (WISC-R) and the Peabody Picture Vocabulary Test-Revised (PPVT-R) using as a sample 72 adjudicated male delinquents aged 13-10 to 16-11. Significant relationships at the .0001 level were obtained for 10 subtests with only one, Object Assembly, computed at the .001 level. A forward selection multiple regression analysis resulted in six subtests of the WISC-R correlating to the PPVT-R with a R2 value of .78. The significance and the implications of this relationship for the juvenile delinquent population were discussed.

HLA-ABC and DR antigens in celiac disease. A study in a pediatric Italian population.

HLA phenotypes of 64 Italian pediatric patients with celiac disease (CD) were compared with those of a group of healthy controls. DR3 and DR7 are significantly increased as reported in other populations. In addition an increase of heterozygotes DR5/DR7 was observed in our patients. The Hardy-Weinberg distribution in the patients group shows a disequilibrium due to the genotype DR5/DR7. Our data confirm that more than one HLA gene product is associated with CD: one with DR3 and the other with DR7.
