Cost-effectiveness analysis of lamivudine and adefovir dipivoxil in the treatment of patients with HBeAg-negative chronic hepatitis B.

AIM: To estimate the cost-effectiveness over a 4-year duration of lamivudine and adefovir dipivoxil for patients with hepatitis B 'e' antigen-negative chronic hepatitis B. METHODS: A decision analysis model has been used to perform a cost-effectiveness analysis of lamivudine and adefovir dipivoxil from the perspective of the Spanish Public Health System. Data were obtained from clinical trials. RESULTS: For the base-case, the total estimated cost per patient treated with lamivudine or adefovir dipivoxil for 4 years was 11,457 and 21,939 respectively. Virological response at year 4 for the lamivudine arm was 40.4% and 78.0% for the adefovir dipivoxil arm. The average cost-effectiveness ratio (cost per responding patient at year 4) was 28,375 for the lamivudine arm and 28,132 for the adefovir dipivoxil arm. The incremental cost-effectiveness ratio of adefovir dipivoxil vs. lamivudine (cost per additional responding patient with adefovir dipivoxil) was 27,872, demonstrating that this cost was slightly lower than the average cost-effectiveness ratios of adefovir dipivoxil or lamivudine. The sensitivity analysis demonstrated that the factors that most influence the cost-effectiveness were the response to adefovir dipivoxil and lamivudine at year 4. CONCLUSION: Long-term treatment with adefovir dipivoxil is a cost-effective strategy in patients with chronic hepatitis B 'e' antigen-negative hepatitis.

Study on the efficacy and safety of adefovir dipivoxil treatment in post-liver transplant patients with hepatitis B virus infection and lamivudine-resistant hepatitis B virus.

Hepatitis B virus (HBV) recurrence and de novo HBV infection are frequent events in liver transplantation recipients. Treatment with lamivudine is initially efficient in both infections but the incidence of lamivudine-resistant HBV emergence increases over time. Adefovir appears to be promising in post-liver transplantation patients with recurrent HBV infection and lamivudine-resistant HBV. This study analyzed adefovir treatment in 42 post-liver transplantation patients who developed recurrent HBV or de novo HBV infection with lamivudine-resistant HBV (54.8% HCV-coinfected). Patients received 10 mg of oral adefovir once daily for a mean period of time of 21.5 months (range from 12 to 31 months). In 62.9% of patients, ALT levels decreased significantly. Serum HBV-DNA was undetectable in 64% of the cases. Twenty percent of patients lost HBeAg marker and 13.3% of them developed anti-HBe. In 9.5% of recipients, HBsAg became negative. There was no significant change in serum creatinine levels. In only one patient was worsening of the renal function detected, making dose adjustment necessary. No other side effects were reported. Our results confirm the efficacy and safety of adefovir treatment in post-liver transplantation patients with lamivudine-resistant HBV, neither were adefovir-resistant mutations identified in patients after 21 months of therapy, nor were there adverse events, especially renal toxicity.

Homocysteine, antiphospholipid antibodies and risk of thrombosis in patients with systemic lupus erythematosus.

Cardiovascular disease is a major cause of morbidity and mortality in patients with systemic lupus erythematosus (SLE). Antiphospholipid syndrome (APS) is one of the most important causes of thrombosis in SLE. In addition, an association between hyperhomocysteinemia and increased cardiovascular risk has also been reported. Our aim is to analyse the association of thrombosis with plasma total homocysteine (ptHcy), antiphospholipid antibodies (aPL) and other vascular risk factors in SLE patients. Fasting plasma levels of ptHcy, vitamin B12, folate, total cholesterol and creatinine were measured in 117 SLE patients. Clinical and immunological data were obtained from our prospective computerized database. aPL-positivity was defined according to Sapporo criteria. There was no association between aPL and ptHcy. ptHcy was higher in patients with arterial (median 13.02 versus 10.16 micromol/L, P = 0.010) but not venous thrombosis. In the subgroup analysis, this association was only seen in aPL-negative patients. In logistic regression, aPL (OR 6.60, 95% CI 1.86-23.34) and ptHcy (OR 1.10, 95% CI 1.01-1.19) were independently associated with arterial thrombosis. However, when hypertension, smoking and plasma total cholesterol were added to the model, only aPL (OR 7.38, 95% CI 2.02-26.91) and hypertension (OR 7.70, 95% CI 2.33-25.39), but not ptHcy, remained independently related to arterial events. aPL was the only variable independently related to venous thrombosis (OR 7.68, 95% CI 1.60-36.86). ptHcy concentrations are higher in SLE patients with arterial thrombosis. No interaction between homocysteine and aPL was found. Raised ptHcy may be a marker of increased vascular risk in aPL-negative SLE patients. The role of homocysteine as a marker of vascular risk may depend on the presence of traditional risk factors, although a modest intrinsic effect cannot be entirely excluded.

Basal cell carcinoma on the vermilion border of the lip: a study of six cases.

BACKGROUND: The vermilion border of the lower lip is a frequent location of squamous cell carcinoma (SCC), but it is very rarely mentioned within the published series of basal cell carcinomas (BCCs). OBJECTIVE: We present 6 cases of BCC involving either mainly or exclusively the vermilion border of a lip. METHODS: We reviewed from our files all the cases of BCC diagnosed in a period of 11 years. RESULTS: A number of 3,477 BCCs were histologically diagnosed in that period; 2,872 (82.6%) of them were located on the head, and 66 (2.3%) of the latter (1.9% of all BCCs) were on the lips. In 6 cases, the neoplasm involved either mainly or exclusively the vermilion border of either the lower (5 cases) or the upper (1 case) lip. CONCLUSION: Not every carcinoma of the vermilion border of the lip is a SCC.

Comparison of terbinafine and griseofulvin in the treatment of tinea capitis.

BACKGROUND: Griseofulvin has been used for many years in the treatment of tinea capitis. Increase in resistance to this medication has led to a search for new therapeutic alternatives. OBJECTIVE: Our purpose was to evaluate the therapeutic efficacy of terbinafine in comparison with griseofulvin in the treatment of tinea capitis. METHODS: We performed a double-blind, randomized, prospective evaluation of 50 patients with a clinical and mycologic diagnosis of tinea capitis. One group received 4 weeks of terbinafine followed by 4 weeks of placebo. The other group received 8 weeks of griseofulvin. We evaluated 5 clinical parameters. Mycologic examinations were performed at baseline and at the end of weeks 8 and 12. RESULTS: Patients' ages ranged from 1 to 14 years. Fifty-four percent were girls and 46% were boys. Mycologic examinations disclosed Trichophyton tonsurans in 74% of patients and Microsporum canis in 26%. At week 8, the griseofulvin-treated group showed a cure rate of 76%, and the terbinafine-treated group 72%. At week 12, the efficacy of griseofulvin decreased to 44%, whereas the efficacy of terbinafine was 76%. CONCLUSION: Terbinafine constitutes an alternative for the treatment of tinea capitis. Recurrences were less frequent. No significant side effects were reported.

The Kleinian Psychoanalytic Diagnostic Scale (revised version): presentation and study of reliability.

The Kleinian Psychoanalytic Diagnostic Scale (KPDS) is a psychodiagnostic instrument of assessment based on the psychoanalytical theory of object relations. It was created in order to introduce an intrapsychic and relational dimension into the clinical research on and epidemiology of psychiatric diagnosis. It consists of 15 subscales grouped into the following four dimensions: "Ego Abilities'; "Projective Identification'; "Paranoid-Schizoid'; and "Depressive'. These dimensions constitute different, relatively stable aspects of the mental life of the subject, their demarcation and description permitting the obtention of a chart and profile of the intrapsychic and relational structure. Following several free, unstructured interviews the scale is assessed on the basis of what the subject says and what the rater may detect in the relationship. The inter-rater reliability of the scale, its stability over a time lapse, as well as its internal consistency have all been shown to be high.

Infundibulocystic basaloid neoplasm.

Infundibulocystic basal cell carcinoma was described in 1987 as a new type of basal cell carcinoma with follicular differentiation. A great controversy followed this description, mainly about the malignant or benign character of this neoplasm and the distinction from other tumors with follicular differentiation. We present two new cases of this neoplasm: the first one had the typical features of this entity, except for the absence of stroma; the second one showed in the deep portion basaloid nodules similar to those of a nodular basal cell carcinoma. We review the criteria adduced to establish the malignant character of this neoplasm and conclude that both the nosology and biological behavior of this neoplasm with follicular differentiation remain to be determined.

A case of complete Carney's syndrome.

The triad of gastric leiomyoblastoma, paraganglioma, and pulmonary chondroma was first described by Carney in 1977. The authors report a case of a patient with gastric leiomyosarcoma, multiple pulmonary chondromata, and paraganglioma. This represents a complete form of Carney's triad. The paraganglioma was identified with I-123 MIBG.

A theoretical investigation on the effect of remote amino groups in hydrogen bonding of nucleic acids.

The effect of remote amino groups in the H-bonding of complementary bases of duplex and triplex DNA has been investigated in the gas phase by means of density functional theory. It is found that amino groups incorporated in regions of the purine that are distant from the H-bonding sites might have a notable influence on the stability of H-bonded dimers. The results show that, in addition to primary and secondary interactions, polarization effects can be relevant for the determination of hydrogen bonding in nucleobases.
