Dynamic digital traction for unstable comminuted intra-articular fracture-dislocations of the proximal interphalangeal joint.

Unstable intra-articular fracture-dislocations about the proximal interphalangeal joint present a formidable management dilemma. Fourteen patients with comminuted injuries of the proximal interphalangeal joint were treated with dynamic digital traction and followed for an average of 24 months. Dorsal fracture-dislocation and pilon-type injuries made up the majority of cases. Average proximal interphalangeal joint articular surface involvement was 80%. Immediate motion was instituted after traction application. The device was applied in the office with readily available materials. At final follow-up examination, active arc of motion averaged 89 degrees at the proximal interphalangeal joint and 95 degrees for patients sustaining isolated injuries. X-ray films revealed fracture union, joint remodeling, and preservation of joint space. Patient satisfaction was high, with return to previous levels of activity and excellent functional joint restoration. Dynamic digital traction is recommended for unstable proximal interphalangeal joint fractures with comminution, including those associated with subluxation or dislocation.

Long-term review of the surgical treatment of radial deficiencies.

Sixty-four patients with 101 radial deficiencies form the basis of this review of anatomy, treatment, and long-term follow-up. Average follow-up was 8.6 years, with a range of 1 to 27 years. A radiologic classification of radial deficiencies is presented. The preferred method of treatment is centralization. The goal is to create a centralized, cosmetically and functionally improved hand, yet maintain wrist motion. There were 21 good, 20 satisfactory, and 10 unsatisfactory results. Good and satisfactory results all had adequate preoperative stretching, proper surgical technique, and postoperative brace compliance. Failure to adhere to outlined principles of soft tissue release and adequate centralization resulted in the unsatisfactory results.

Subungual myxoma.

A true myxoma is a rare neoplasm, and there has never been a fully documented case of a subungual myxoma. We present another digital myxoma in a previously unreported location, accompanied by a review of the literature. Simple excision of the neoplasm will be curative in most cases.

Symmetric hyperphalangism of the index finger in the palatodigital syndrome: a case report.

A case of palatodigital syndrome is added to the eight cases in the literature. The accessory phalanx between the index metacarpal and proximal phalanx produces a significant deformity of extreme radial deviation with mild supination. This deformity was treated by rotation and stabilization of the accessory bone. Follow-up at 1 year shows correction of the radial deviation has been maintained and that the finger has grown appropriately.
