[Principles of constructing synbiotic preparations, and evaluation of their clinical effectiveness].

The problem of constructing means of correction of gastrointestinal microflora is utterly important. The most promising direction here seems to be construction of synbiotics containing a multiprobiotic complex of bifidobacteria and lactobacilli, modeling the microbiocenosis of a certain age group, inulin, as a prebiotic factor and a vitamin and mineral complex. The aim of the study was to evaluate the biologically active food supplement "Normospectrum" vs. the commercial preparation "Bifidumbacterin", its ability to correct the intestinal microflora and the functional condition of the gastrointestinal tract in children. "Normospectrum" was well tolerated, favored regression of the main clinical manifestations, and had positive effect on the intestinal microbiocenosis, increasing the proportion of bifidobacteria, lactobacilli and escherichiae with full enzymatic activity, and lowering the proportion of conditionally pathogenic bacteria and fungi in the intestinal tract. The effects of "Normospectrum" on the functional disturbances of the gastrointestinal tract and the intestinal microflora were much more profound than those of "Bifidumbacterin". Good tolerance and positive effects of the new probiotic preparation "Normospectrum" on the intestinal microbiosis allows recommending it for broad clinical application in children when correction of dysbiotic shifts in the intestinal microflora is necessary.

Effect of ascovertin on morphofunctional changes in the ovaries in monopolar coagulation.

A course of ascovertin therapy reduced manifestations of perifocal inflammation and atresia of growing follicles, corrected vascular disorders and changes in the tissue volume ratios outside the necrotic zone after monopolar coagulation of the ovaries in rats. The drug prevented the increase in blood viscosity, erythrocyte aggregation, and LPO activation in the early postoperation period.

[The ultrastructural changes to the ovaries and oviducts in experimental inflammation under the influence of eplir phonophoresis].

Ultrastructural changes of blood vessels of ovaries and oviducts, destruction of the part of follicular epitheliocytes and disruption of other structures of blood-follicular barrier occur at the early period of experimental inflammations of appendage uterus on the background of traditional antibiotic therapy. All changes lead to atresia of growing follicules. Fibrotic-sclerotic changes develop on the 30th day and during later period. Course of eplir phonophoresis decreases ultrastructural disruptions of vessels, follicular apparatus of ovaries and oviducts epithelium. Effect of physiotherapy is manifested though activity of fibroclasts and macrophages, that regulate collagen production/resorption ratio thus preventing the development of fibrotic changes of ovary stroma and oviduct wall.

[The characteristics of the HIV isolated from HIV-infected persons and AIDS patients on the territory of the CIS].

HIV strains were isolated from HIV-infected patients and AIDS patients in CIS. A total of 81 HIV isolates were obtained. The isolates were identified by using immunofluorescence and enzyme immunoassay, by determining the activity of reverse transcriptase, immunoblot, electron microscopy, polymerase chain reaction. Of the 81 isolates 79 were HIV-1 and 2 HIV-2. The strains differed in their infectivity, the kinetics of virus antigen accumulation, and the spectrum of susceptible cell lines. The viruses isolated may be assigned as two groups: high and low infective. The biological properties of the national HIV isolates were shown to be similar to the prototype HIV strains isolated elsewhere.

[Antibiotic therapy in children with pertussis].

The data accumulated within the last years required revision of the indications to the use of antibiotics in treatment of pertussis. One of the aims of antibiotic therapy in pertussis was to prevent colonization of B. pertussis in the respiratory tracts. With that end in view the choice of antibiotics should be limited by those, to which the pathogen is the most sensitive i.e. erythromycin, ampicillin and augmentin. Comparative efficacy of erythromycin and ampicillin during the first 2 weeks of the disease was studied in 79 infants at the age not older than 1 year with pertussis and it was shown that erythromycin was advantageous by its therapeutic activity and less side effects. Expedience of the antibiotic therapy during the spastic period for providing a preventive effect on development of bronchopulmonary complications was studied in 201 patients with pertussis. No preventive effect of the antibiotics on development of the bronchopulmonary complications defined by the secondary bacterial flora was recorded. In the group of the patients treated with the antibiotics prophylactically (group 1) the complications were 2.6 times more frequent than in the patients treated with pathogenetic agents alone (group 2). Intrahospital pneumonia developed in 8.9 per cent of the patients in group 1 and in 1.5 per cent of the patients in group 2. Therefore, antibiotics should not be used at the late periods of pertussis for prophylaxis of secondary bacterial complications.

[Augmentin in the clinical picture of infectious diseases].

Augmentin is a new combination manufactured by Smith Kline Beecham (Great Britain). It is composed of amoxycillin and clavulanic acid and has antibacterial activity. Augmentin was used in clinical trials in the therapy of 50 adult patients with suppurations after surgical operations on the organs of the abdominal cavity, general staphylococcal infections, pneumonia and prophylactically during the preoperative period. It was also used in the treatment of 30 children patients with bronchopulmonary affections and inflammatory otorhinolaryngological diseases. The clinical trials were performed in the Clinic of Infectious Diseases of the N. G. Gabrichevskiĭ Moscow Research Institute of Epidemiology and Microbiology. For comparison ampicillin was used in the trials. Augmentin was shown to be an efficient formulation with antibacterial activity which could be successfully used in the parenteral therapy of severe affections due to organisms sensitive to it. In the treatment of the children patients with pneumonia augmentin by its therapeutic efficacy proved to be superior to ampicillin. The tolerance of augmentin was good.

[Spread of whooping cough among adults today].

Epidemiological surveys with the use of the clinical and bacteriological methods of examination were carried out in 107 foci of pertussis in families (283 adults), 25 foci in children's institutions (103 employees) and 1 focus in a surgical department for infants (29 adults). The infection was shown to be widely spread among adults at present: 23.7% in the surveyed families, 10% in children's institutions and 6.9% in the surgical department for infants. In the familiies, 26.2% of the adults were the first to fall ill with pertussis. no essential differences in the terms of the release of the infective agent, as well as in the course of the disease, in adults and in vaccinated children were found. Antiepidemic measures in respect to adult pertussis patients is proposed.

[Epidemiological evaluation of the effectiveness of the obligatory isolation of whooping cough patients in schools].

Analysis of observations carried out under practical conditions (165 patients with pertussis from 48 foci) and the results of controlled epidemiological trial on the discontinuance of compulsory isolation of pertussis patients at schools (12 classes, 402 children) showed that isolation of the patients failed to stop the spread of infection in the focus. This was attributed to incomplete detection of the patients and their late isolation in conditions of wide incidence of mild forms and formes frustes of pertussis. When patients suffering from pertussis were left at school, there occurred no increase in the spread of the infection in the focus, in duration or severity of the disease and no increase of the period of discharge of the causative agent. However, the number of days of the patient's absence from school was greatly reduced. Taking into consideration the absence of an attack-like cough in patients with formes frustes, short duration and preponderance of the attacks at night in patients with a mild form of pertussis, no disturbances in the general condition of the child, and also no indications to the treatment, schoolchildren should be isolated by clinical indications only (pyrexia, marked catarrhal phenomena, frequent attack-like cough, complications, concomitant diseases); they can be admitted to school as soon as the mentioned signs subsided irrespective of the time lapse from the onset of the disease.
