The "SMAS sling": an additional fixation in face lift surgery.

A pennant-shaped "SMAS sling" is developed from the preauricular SMAS, leaving it attached inferiorly between the level of the earlobe and the angle of the mandible. This is rotated posteriorly over the mastoid process, tension is applied, and it is secured with nonabsorbable sutures to the mastoid fascia, thereby providing a backward and upward lift to a very secure area. Minimal problems and complications occurred in 34 patients over the past four years.

The anesthetics of ear placement: an experimental study.

The angle between the axis of the ear and the bridge of the nose was measured in 46 normal subjects as being 14.9 degrees. This was a statistically significant difference (p less than 0.00001) from being parallel--the orientation recommended in the literature for placement of a reconstructed ear. Observers asked to choose the most aesthetically pleasing ear orientation from a group of four possibilities overwhelmingly chose an orientation with the helix more anterior than the parallel one. Recommendations are given for placement of a reconstructed ear.

Diseases of the breast in young women.

The records of 516 women, less than 30 years of age, requiring biopsies of the breast were reviewed. Four cancers were identified. All 397 biopsies done on patients less than 27 years of age were benign. Young women with masses can be reassured as to the probable benignity of the illness and operative timing made compatible with their other obligations. Greater development of ambulatory care surgical centers is needed to accommodate young women with breast masses to alleviate both anxieties and costs associated with hospitalization.

A simple geometric flap for the closure of skin defects.

The Limberg flap is a useful method of closing defects anywhere on the body which are too large for simple closure but where skin grafting is not desirable. It is an easy flap to perform, and it is reliable and safe to use. It must not, however, be used indiscriminately or by rote. Care must be taken in planning both the excision and the flap so that the points which must move farthest are placed where the skin is maximally extensible. With such planning, accurate and precise designing and careful surgical technique, the Limberg flap can be used successfully to solve a number of otherwise extremely difficult problems.

Gas-producing clostridial and nonclostridial infections.

Clostridial infections have always been treated promptly and aggressively on the basis of history and Gram's strain. We believe that nonclostridial infections with similar fulminating, septic courses also should receive as prompt and radical treatment and perhaps more so, as hyperbaric oxygen therapy is of far less benefit in a nonclostridial infection. It is particularly important to treat the high risk, obese, patient with diabetes and an infection of the perineum or groin promptly and aggressively if the current, high morality is to be decreased.
