[Radiotherapy and combined chemo- and radiotherapy of patients with squamous cell lung cancer (randomized study)].

A randomized study on the results of treatment of 76 cases of squamous cell carcinoma of the lung was conducted at the Center in 1976-1981. Radiation and chemoradiation procedures were studied in two groups of patients. The immediate results of chemoradiation therapy were somewhat better as compared with radiation treatment alone. However, no significant increase in survival rates was recorded. The toxicity of the chemotherapeutic procedures used was shown. Total focal dose was increased to 90-120 Gy and longer survival was registered as a result of consistent application of uneven irradiation and open fields in gamma therapy.

[Radiation treatment results in metastases of small-cell lung cancer to the brain].

Altogether 31 patients aged 36-66 were given gamma-beam therapy for metastases of small cell pulmonary carcinoma to the brain. Before irradiation 12 patients received chemotherapy alone, 17 chemotherapy and irradiation of a primary focus and 2 surgery followed by chemotherapy. Simultaneously with a primary tumor brain metastases were found in 6 out of 7 patients. Brain involvement in 24 patients occurred 2.5-26.4 mos. after initiation of treatment. The presence of brain metastases was confirmed by the clinical symptoms, the electroencephalography and computerized tomography data. Brain irradiation in most of the patients was done in the presence of dehydration therapy. Twenty-four patients received a complete preplanned therapeutic course, in the other 7 patients treatment had to be interrupted because of the aggravation of symptoms. A complete regression of different brain symptoms was observed in 60-100% of the cases, the improvement of the focal symptoms and the state of the higher nervous activity was observed in a somewhat less percentage of the cases. Six out of 24 patients lived for 1 year and longer after irradiation of brain metastases. The results of this study have shown that brain metastases of small cell pulmonary carcinoma respond to radiotherapy. The dose of 30-40 Gy is sufficient to do away with severe neurological symptoms in most of the patients.

[Results of rotational gamma teletherapy of prostatic cancer].

Remote rotational gamma-therapy was carried out in 36 cases of prostatic cancer. Hormonal treatment was given to 27 patients before gamma-therapy. There was a 2-4 week interval in the course of treatment. Single focal dose was 2 Gy and total dose--56-71 Gy. Complete resorption of tumor was in 26 and reduction in tumor size in 9 patients. Recurrences were registered in 9 out of 26 cases within the first two years after exposure termination. Bone metastases were observed in 7 out of 36 cases within 9-48 months after treatment. Hormones were administered to patients with metastases as well as cases of incomplete cure and recurrences. The patients were followed up during periods ranging from 6 months to 7 years. Survival for more than 3 years was recorded in 61% of cases. No radiation injury was observed. Rotational remote gamma-therapy may be used for treatment of local prostatic cancer.

[Combined chemo-radiotherapy of small-cell cancer of the lung].

The authors report the results of the combined treatment of 32 patients with small-cell cancer of the lung by irradiation in a dose of 5000 rad and chemotherapy: cyclophosphan, methotrexate, nitrosoethylurea. An immediate objective improvement was noted in 90 per cent of the patients, an average survival being 10 months, while without the treatment the survival was no longer than 5 months.

[Chemoradiation therapy of inoperable lung cancer].

The results of utilization of different variants of the complex therapy in 416 patients with inoperable lung cancer are reported. 5-fluoruracil, cyclophosphan, methotrexate, chrysomallin were employed as antitumor drugs. Radiotherapy was carried out on a distance gamma machine. The best immediate and early results were gained in patients treated by distance gammatherapy against the background of cyclophosphan injections. The results proved to be most favourable within the terms of 3 years and longer in case of gammatherapy associated with 5-fluoruracil. Application of a fractionated radiation course rendered no positive effect on the results of treatment.
