Adolescent idiopathic scoliosis. Is low melatonin a cause?

BACKGROUND: Although the cause of adolescent idiopathic scoliosis remains unclear, pinealectomy is followed by the development of scoliosis in chickens. Melatonin is the only hormone secreted by pineal gland. OBJECTIVE: To assess serum melatonin levels in adolescents with idiopathic scoliosis. METHODS: Serum melatonin was assayed once, at 12:00 hours, in each of 20 adolescents with idiopathic scoliosis and 10 age-matched siblings. In the patient group, mean age was 14.3 years and the mean Cobb angle was 54.5 degrees (range, 20 degrees-110 degrees). Mean age in the control group was 14 years. RESULTS: Serum melatonin was significantly lower in the patients (mean, 7.7 pg/mL; range, 4 to 13 pg/mL) than in the controls (mean, 29.9 pg/mL; range, 19.3 to 46 pg/mL) (P < 0.00001). The 12 surgically treated patients had a mean melatonin level of 8.1 pg/mL, versus 7.2 pg/mL in the eight other patients (nonsignificant P and regression coefficient values). Serum melatonin was not significantly correlated with the Cobb's angle (regression coefficient, 0.18; P < 0.44). CONCLUSION: Our data suggest that serum melatonin levels may contribute to the pathogenesis of idiopathic scoliosis.

Plantar fasciitis/calcaneal spur among security forces personnel.

A prospective single survey was conducted among Security Forces personnel at the Mobile Hospital, Ministry of Interior, Makkah Al-Mukarramah, Saudi Arabia. One hundred three patients presented to the orthopedic clinic with painful heel. A control group of patients from the other clinics without painful heel was also screened. The average age was 38.81 years and 38.1 years in the patient and the control group, respectively. The mean Quetelet index of body mass in the patient group was 30.36 kg/m2 and in the control group it was 26.71 kg/m2. There was a statistically significant difference of p < 0.05 between the two groups for Quetelet index of body mass. The right side was affected in 37 patients, the left side was involved in 45 patients, and 21 patients had pain bilaterally. Seventy-one patients had spur on the calcaneum. The prevalence of painful heel attributable to plantar fasciitis/calcaneum spur was 1.18%. This study suggests that obesity is a cause and initiator of heel pain and plantar fasciitis/calcaneal spur and that improper footwear aggravates the condition.

Osteoarthritis of the knee among Saudi Arabian security forces personnel.

The prevalence and pattern of osteoarthritis of the knee and its association with obesity among security forces personnel was investigated. A single survey with a control group was done at the Mobile Hospital, Ministry of Interior, Makkah Al Mukarramah. One hundred twenty-five patients presenting to the orthopedic clinic with painful knees of at least 12 months duration gave detailed histories and were treated with bi-planar conventional radiography; they were matched with a similar number from the clinics of internal medicine without painful knees. One hundred three patients (82.4%) with 126 painful knees had various degrees of osteoarthritis. The mean age was 41 and 41.76 years for the patient and control group, respectively. The medial tibio-femoral and patello-femoral compartment were involved in 116 cases (92%). Fifty-nine knees (46.8%) had mild, 46 (36.5%) had moderate, and 21 (16.6%) had severe osteoarthritis changes. There were statistically significant differences between the two groups for weight and Quetelet index of body mass (p < 0.047 and < 0.0001). In the study group, the mean Quetelet body index was 31.6638 kg/m2, and in the control group it was 28.5633 kg/m2. The prevalence of osteoarthritis among the security forces personnel was 1.19%. The medial and patello-femoral compartment was affected in the majority of cases, and obesity was confirmed as one of the important causes of osteoarthritis in the Saudi Arabian population.

Malignancy in chronic osteomyelitis sinus. Report of three cases.

Three cases of squamous cell carcinoma in a chronic osteomyelitis sinus is reported. These tumours are usually managed by amputation alone. One of our patient had a histologically invasive and keratinising tumour who died after amputation. The other two cases are still surviving, one even without any treatment. We believe that in patients with histologically undifferentiated and invasive tumours aggressive mode of treatment should be carried out by way of amputation, regional lymph node clearance and a short course of chemotherapy/radiotherapy.

Retrofascial nontuberculous psoas abscess.

Psoas abscess is usually associated with tuberculous spondylitis, but also occurs in relation to inflammatory bowel disease. We present 17 cases of primary pyogenic psoas abscess seen during a 10 year period at the King Fahd Hospital, Al-Khobar. There were 13 males and 4 females with a mean age of 24.9 years (range one to 55 years). Five patients had sickle cell disease. The average delay in presentation was 4.5 weeks (range 2 to 9 weeks). Pyrexia and a painful hip with a flexion deformity were the most obvious signs. The sedimentation rate and white cell count were markedly raised in every case. Staphylococci were cultured in 10 and anaerobic streptococci, salmonella and E coli in 2 each. Ultrasonography, CT and MR imaging were used to confirm the diagnosis. Early recognition and drainage are important to ensure a rapid recovery.

The spine in sickle cell disease.

Bone changes in sickle cell disease occur due to marrow hyperplasia, tissue ischaemia and infarction due to vaso-occlusion. Between 1982 and 1991 thirty four patients were treated in the Orthopaedic and Neurosurgery Departments of the Kind Fahd University Hospital, Al-Khobar, with spinal complications due to sickle cell disease. There were 21 males and 13 females aged between 4 and 28 years (mean 17.4 years). Structural changes in the vertebral bodies due to marrow hyperplasia occurred in 44% of the patients. Avascular necrosis leading to collapse of the vertebral bodies was seen in 9 (27%) patients. Infective spondylitis was the most serious complication seen in 8 (24%) patients; the majority needed anterolateral decompression and bone grafting. The spine is often affected in sickle cell disease and aggressive treatment with close follow-up is required to avoid disabling complications.

The scud missile disaster in Al-Khobar, Saudi Arabia, 1991: the orthopaedic experience.

The orthopaedic experience of the scud missile disaster in Al-Khobar, Saudi Arabia during the Gulf war is reviewed. This was by far the most severe of 70 scud attacks. The majority of casualties had 'orthopaedic' injuries, fractures and soft tissue lacerations of the extremities and the back. Our experience confirmed the importance of the principles of planning triage beforehand and open wound treatment with secondary suture. Repeated ward rounds with reassessment of the patients proved beneficial. The ready availability of hospital beds in high-risk situations of regional war highly facilitated the management of mass casualties.

Avascular necrosis of the femoral head in sickle cell disease. An integrated classification.

Avascular necrosis of the femoral head (ANFH) is one of the crippling complications of sickle cell disease (SCD), and to date there is no classification that could identify patients in the early phase of the necrosis. Based on the experience of 66 ANFH in 43 patients with SC hemoglobinopathy, the necrosed hips were classified into four grades, Grade I to Grave IV, based on patient history, clinical evaluation, and radiologic examination. In Grade I, the patient complains of the mildest symptoms with minimal clinical signs and radiologic changes. The most severe form, Grade IV, is where the findings are unbearable pain with disability and marked roentgenographic evidence of osteonecrosis. This classification identifies patients in Grade I and II, which, with early diagnosis and proper management, could delay further deterioration of the necrosed femoral head.

Orthopaedic complications in sickle cell disease. A comparative study from two regions in Saudi Arabia.

The haematological and orthopaedic complications of patients with sickle cell disease from two different regions of Arabia are presented. Nineteen patients from the South-western region were matched for age and sex with an equal number from the Eastern region. The mean sickle cell haemoglobin was 75.95% in those from the South-west and 77.5% in those from the East. The haemoglobin concentration was marginally lower in the Eastern region patients at 9.19 g/% compared with 9.51 g/%. Bone and joint infections occurred in 17% of Eastern region patients and in 15% in those from the South-western region. There was no significant difference between the haematological parameters and the orthopaedic complications in the two regions. We conclude that these complications are severe in the Eastern region and the disease is not benign as previously thought. Aggressive treatment of orthopaedic complications is indicated in sickle cell disease in the Eastern region.

Fractured neck of the femur in young adults.

This retrospective study at King Fahd University Hospital was undertaken to assess the hospital-based incidence and the complication rate among young Saudi nationals with a fracture of the femur. Hospital records of 17 patients between 17 and 45 years of age (mean 27.4 years) with a fracture of the neck of the femur were studied for the period 1982 to 1988. Factors such as type and severity of injury, associated injuries, delay in internal fixation and the type of implant used, were analysed. Fracture of the neck of the femur in young adults was 28 per cent (22/79) of these fractures in all age groups during the study period. There were no complications encountered, such as avascular necrosis or late segmental collapse.

Manage of acute osteomyelitis in children--should it be conservative?

73 children with acute osteomyelitis seen at King Fahd University Hospital, Al-Khobar, Saudi Arabia were reviewed. Majority of patients were between the 6-10 year age group and male children predominated. All patients had incision and drainage and antibiotic therapy for a minimum of 6 weeks. Metaphysis of the tibia was the commonest site of infection and the staphylococcal species was cultured in 77% of children. Ampiclox was the most sensitive antibiotic. The average follow up was 4.9 years and the incidence of chronic osteomyelitis was 1.75%. In conclusion this study confirms that clinically it is not possible to detect a subperiosteal abscess within the first 48 hours. As blood culture is not 100% reliable in isolating the ineffective organism and incision and drainage plays a double role, firstly in the identification of the offending organism and secondly in the evacuation of pus. A six-week antibiotic therapy is essential to prevent any relapses. The correct management of acute osteomyelitis in children is surgical drainage of the affected site combined with antibiotic therapy.

Arteriovenous fistula for hemodialysis. A report of 112 consecutive cases.

Arterio-venous fistula of Brescia was created in 112 consecutive patients during a five year period from May 1983 at King Fahd University Hospital of King Faisal University, Al-Khobar, Saudi Arabia. These fistulae were created for hemodialysis in patients with chronic renal failure. There were 67 males and 45 females. The youngest patient was 13 years and the oldest 75 years. Complications were negligible (0.85%), even-though 31% were diabetics and 24% had sickle cell hemoglobinopathy. The arteriovenous fistula failed in 6 patients and the patency rate was 94.6%. The minimum follow up was three months and the maximum fifty-two months. The end-to-side anastomosis gave better results. For long term dialysis arterio-venous fistula of Brescia should remain as the first choice.
