Small bowel perforation caused by metastasis from an extra-abdominal malignancy: report of three cases.

Small bowel perforation is rarely caused by metastasis from an extra-abdominal malignancy. This report describes three cases of small bowel perforation that occurred secondary to a metastatic tumor. The first case involved a 72-year-old man with malignant lymphoma of the larynx that had been treated with chemo- and radiation therapy; the second involved a 70-year-old man with rhabdomyosarcoma of the mediastinum that had been treated with radiation therapy; and the third involved a 41-year-old man with lung carcinoma that had been treated with surgery 10 months prior to perforation. Each patient presented with acute abdominal pain, had X-ray findings of free air in the abdomen, and underwent limited emergency surgery. Wedge resection and closure of the ileum was performed for the first patient and partial bowel resection with the creation of an intestinal stoma was performed for the second and third patients. In each case, the histologic findings of the resected specimens were consistent with the extra-abdominal primary tumors. Although the patients recovered sufficiently to begin eating and moving about, all three died of cancer or cancer-related complications within 45 days of surgery. We conclude that surgeons should be aware of the poor prognosis of such patients and perform only the minimal surgery required.

Chondroblastoma of a metacarpal bone mimicking an aneurysmal bone cyst: a case report and a review of the literature.

Chondroblastoma of the metacarpal bone has been extremely rare and only seven cases have been reported in the English literature. Here we reported the eighth case of a chondroblastoma that developed on the first metacarpal bone of the right hand of a 21-year-old man. Radiographs showed an expansile osteolytic lesion with a multilocular appearance. In MR images, the lesion showed low intensity in T1 and high intensity in T2-weighted images with multiple fluid-fluid levels, which are findings resembling those of an aneurysmal bone cyst. From the pathological findings, however, it was recognized as a chondroblastoma with aneurysmal bone cyst-like change. Good clinical results was obtained by the subtotal resection of the metacarpal bone with a columnar-shaped iliac bone graft.

Osteoblastoma-like osteosarcoma.

We report an osteoblastoma-like osteosarcoma in the right proximal fibula in a 22-year-old woman. Radiographs showed an irregular osteolytic lesion from the metaphysis to the epiphysis in the proximal fibula with partial destruction of cortical bone. Tissue from a biopsy indicated a typical osteoblastoma. Curettage and bone graft was performed. One year after the surgery, local recurrence occurred, and a wide excision was performed. Histological examination of the en-bloc surgical specimen revealed the tumor had permeated through the host bony trabeculae, although the nuclear atypia was not marked. Immunohistochemical expression of MIB-1 was detected in 9.0% of cells.

Cystic fibrous dysplasia mimicking giant cell tumor: MRI appearance.

We report the case of a 43-year-old man who presented with an osteolytic and expansive lesion in the left distal femur mimicking a giant cell tumor. Magnetic resonance imaging (MRI) showed that most of the lesion was cystic, and histological examination revealed fibrous dysplasia with marked cystic degeneration. Radiographic findings of cystic fibrous dysplasia in the end of a long bone may be similar to those of a giant cell tumor, and a biopsy is essential for the final diagnosis.

Esophageal schwannoma.

A rare case of esophageal schwannoma is presented. A 63-year-old woman was admitted to our hospital with a 5-year history of dysphagia. The barium esophagogram showed a protrusive smooth tumor in the upper thoracic esophagus. The tumor was removed through right thoracotomy. There was no anatomical relationship between the tumor and vagal nerve trunk. From pathologic findings and positive immunohistochemical staining for S-100 protein, the diagnosis of esophageal schwannoma was made.

HLA-DR expression of synovium and correlation with clinical features of patients with rheumatoid arthritis.

To investigate whether the expression of human leukocyte antigen-DR (HLA-DR) in synovial tissues obtained at synovectomy contributes to the clinical features of patients with rheumatoid arthritis (RA), immunohistochemical analysis was performed to determine the intensity and pattern of HLA-DR expression in synovial tissue from 96 patients with RA. The clinical features before and 1 year after the synovectomy were investigated. At the time of the surgery, duration of morning stiffness was associated with the degree of HLA-DR expression in synovial lining layer, and this synovial lining expression of HLA-DR was more frequently observed in elbow and wrist joints than in knee joint. In patients who underwent knee synovectomy, erythrocyte sedimentation rate and C-reactive protein level one year after the surgery were significantly higher in the patients with intense expression of HLA-DR in the synovial lining. Our findings showed that the expression of HLA-DR in the synovial lining contributes to several clinical features, and the expression in large joint such as knee may related with disease course of patients with RA.

Fibroma of tendon sheath arising from the radio-ulnar joint.

A report of a 65-year-old male with a tumor arising from synovial tissue of the radio-ulnar joint. On magnetic resonance imaging, the tumor was demonstrated as a heterogeneous and lobulated mass with a low signal intensity both in T1- and T2-weighted images. Histological findings of the tumor were identical to those of fibroma of the tendon sheath. In the peripheral villous synovial tissue, several small and fibrous nodules were observed, and their histological features were identical to those of the main tumor. Immunohistochemically, the tumor cells showed diffuse and intense reactivity to vimentin, muscle actin and S-100. These results indicated that the tumor might be a fibromatous analog of synovial chondromatosis.

Extra-abdominal desmoid tumor of the hand: a case report and review of the literature.

Extra-abdominal desmoid tumor of the hand is rare and only 10 cases have been described in the literature. We present a 14-year-old boy with a recurrent extra-abdominal desmoid tumor in the dorsal site of the right hand. MR image demonstrated the tumor in the third dorsal interosseous muscle, and adhered to the radial side of the forth metacarpal bone. The lesion revealed iso-signal intensity on T1-weighted images and high intensity on T2. We performed a marginal excision. Histological examination of the tumor showed proliferation of the fibroblastic cells with abundant collagen bundles. He developed local recurrence for the third time. The size of the third recurrent tumor has not been changed for 2 years and 3 months. Therefore, we have not performed any additional surgery. Since extensive resection markedly diminishes the function of the hand, we consider that a marginal surgical margin is acceptable for the quality of daily life of patients with a desmoid tumor of the hand.

Sarcoma botryoides of the uterine cervix: a case report.

Rhabdomyosarcoma of the uterine cervix is a very rare disease. We present a case of rhabdomyosarcoma (botryoide type) arising from the introitus of the uterine cervix, and make a discussion on the diagnosis, treatment and involving factors of prognosis.

Loss of arterial dilation in the reendothelialized area of the flow-loaded rat common carotid artery.

We have investigated regenerated endothelial cells and their possible contribution to arterial dilation in response to increased blood flow in rat common carotid artery (CCA). After endothelial denudation using a balloon catheter in the left CCA, an arteriovenous shunt was constructed between the left CCA and the left external jugular vein at 20 mm distal from the orifice in the denuded group. Animals that were given the arteriovenous shunt without denudation were used to form the nondenuded group. The blood flow rate in the left CCA was increased by sixfold after operation in the denuded group. We observed that endothelial cells were gradually regenerated from the orifice to the distal area and that the reendothelialized area after 4 to 8 weeks was approximately one third of the left CCA (5.31 +/- 1.49 mm at 4 weeks, 5.47 +/- 1.56 mm at 8 weeks). In the reendothelialized area of the left CCA after 4 to 8 weeks, the lumen diameter was significantly smaller than that of the nondenuded group and showed no significant difference from age-matched nonsurgical animals. The intimal and medial thickening, which would result in arterial stenosis in the reendothelialized area, was not observed in the denuded group, although the denuded control showed significant intimal thickening. From these results, we conclude that regenerated endothelial cells reduce intimal thickening but do not respond to increased blood flow to dilate the artery.

Case report 858: Postradiation osteosarcoma of the patella.

A case of postradiation osteosarcoma of the patella in a 54-year-old man was presented. The lesion in the patella was diffuse and highly sclerotic, and was partially radiolucent in radiographs. The whole patella was removed by Miyakawa's method. Diagnostic criteria, clinical features, and radiographic findings of postradiation sarcoma were described. The rarity of osteosarcoma of the patella was emphasized.

Resection of metastatic thyroid carcinomas to the liver and the kidney: report of a case.

Differentiated thyroid cancer is considered to have a quite favorable prognosis. However, some patients die as a result of distant metastasis, which mainly consists of pulmonary, mediastinal, or osteal metastases. The biological features of the tumor, such as a slow growth gradient, indicate the possibility of achieving comparatively satisfactory results in the treatment of such distant metastases. A complete surgical excision of the distant metastases in differentiated thyroid carcinoma has been reported to offer the best chance for prolonged survival. A case of unusual metastasis sites on the caudate lobe of the liver and right kidney occurring in a 72-year-old woman is herein presented. The location of the surgically treated distant metastases of the papillary thyroid carcinoma make this case unique.

Large pituitary adenoma of the sphenoid sinus and the nasopharynx: report of a case with ultrastructural evaluations.

A unique case of nonfunctioning pituitary adenoma exclusively involving the sphenoid sinus and the nasopharynx is reported. The computed tomography and magnetic resonance imaging (MRI) clearly depicted the presence of a large, soft mass in the sphenoid sinus and its extensive invasion to the sphenoid wing and the clivus. In particular MRI was found useful in delineating precise anatomic relationships between this sphenoid sinus tumor and the pituitary fossa. The sphenoid sinus tumor was partially resected by sublabial transnasal approach, and the intact dura mater of the base of the pituitary fossa was confirmed. Pathologic examinations including immunocytochemical and ultrastructural studies showed that the tumor was classified as a nonfunctioning acidophilic pituitary adenoma. Despite the endocrine-inactive tumor, the presence of small secretory granules in the cytoplasm demonstrated by electron microscopic studies was of significant importance in establishing the diagnosis. This rare tumor is reviewed in the literature in the context of nasopharyngeal extension of pituitary adenomas, and a possibility of ectopic occurrence and growth is also discussed in the presented case.

[Efficacy of preoperative irradiation in midline oligodendrogliomas. Report of two cases].

Two midline oligodendrogliomas in young males were successfully totally removed after preoperative irradiation. A 33-year-old male with right lower extremity weakness had a large hypervascular mass occupying the left lateral ventricle. Even after 31 Gy whole-brain irradiation, massive bleeding occurred at surgery and resulted in only partial removal. The residual tumor markedly regressed with disappearance of abnormal vascularity after subsequent local boost irradiation. At the second operation, the tumor was totally removed. A 32-year-old male with progressive headache had a hypervascular mass with gross calcification in the right lateral ventricle. The tumor was partially resected due to its abundant vascularity and blood loss. After 60 Gy local irradiation, the tumor was moderately shrunk with a significant reduction in vascularity. At the second operation, the tumor was totally removed. Preoperative irradiation as an adjunct to surgery may increase the resectability of highly vascular tumors such as midline oligodendrogliomas.

An ultrastructural study of pulmonary capillary vessels in blood volume-overloaded rat.

Blood volume overload to pulmonary capillaries was experimentally induced by arterio-venous fistula between the left common carotid artery and the left external jugular vein in 42 S-D rats. The animals were sacrificed 1,2,4,8 and 24 weeks after the operation. Just after anastomosis, blood flow index of the shunted rats was about 4 times larger than in the controls and the high value was kept throughout the experiment (p less than 0.05). The relative right ventricular weight was about 1.5 times larger than in the controls (p less than 0.05). The capillary endothelium of the alveolar septum showed nuclear enlargement, swelling of the cytoplasm with microvillous projection, and increases of pinocytotic vesicles and microfilaments. In some cases, focal edema was seen accompanied with an increase of immature stromal cells. The basement membrane showed vague in contour in edematous area, especially in the epithelial side of the alveolar septum. These changes appeared most predominantly in 2 weeks and subsided almost completely in 24 weeks. Number of endothelial nucleus of the shunted rats increased significantly in 2 and 4 weeks and was normalized in 24 weeks. The surface area and volume of the capillaries also increased significantly in 2 and 4 weeks of the shunted animals. These findings showed that the capillary endothelium reacted and then was adapted to the blood volume overload.

Massive ovarian edema associated with polycystic ovary.

A 14-year-old girl with massive ovarian edema (MOE) of the right ovary and a polycystic ovary is presented. The right ovary was moderately enlarged and incompletely twisted. Histologically, it showed diffuse interstitial edema and multiple cysts, frequently surrounded by a line of luteinized cells. Such cysts are suspected to consist of highly degenerative ovarian follicles, in addition to an edematous corpus albicans and cystic change in the edematous stroma. Focal edematous change and occasional indefinite cystic structures, as seen in the left polycystic ovary of our patient, indicate an early stage of MOE. The pathogenesis of MOE in the present case was therefore suspected to be based on the underlying state of bilateral polycystic ovary. The luteinized cells would have been derived from granulosa cells and from the internal theca cells of the follicles in addition to stromal cells.

Structure and pulmonary toxicity relationship on O,O-dimethyl S-alkyl phosphorothioate esters.

The structure-pulmonary toxicity relationship was investigated using three different O,O,S-trimethylphosphorothioate (OOS-TMP) analogues, which is known to induce a typical mortality pattern (delayed death) and pulmonary injury; O,O-dimethyl S-ethyl (OOS-DMEP), O,O-dimethyl S-propyl (OOS-DMPP), and O,O-dimethyl S-butyl (OOS-DMBP) phosphorothioates. The mortality pattern in rats dosed with OOS-DMEP was similar to the "delayed death" pattern: the LD50s in rats for OOS-DMEP decreased dramatically from more than 200 mg/kg within 24 hr to 41.1 (males) or 13.8 (females) mg/kg 7 days after treatment while the LD50s in rats for OOS-DMPP and OOS-DMBP did not. Histopathological examinations revealed that OOS-DMEP induced pulmonary oedema and bleeding at a dose of 1/2 LD50 by 72 hr after dosing while the other two compounds did not. Thus, it was concluded that OOS-DMEP induces pulmonary injury, thereby eliciting the "delayed death" mortality pattern.
