Surgical treatment and radiological consideration of supergiant cerebral aneurysms.

We have collected four cases of supergiant cerebral aneurysm, each greater than 6.0 cm in diameter. Two of these cases were saccular aneurysms of the anterior communicating artery and middle cerebral artery, of which the necks were clipped with resection of the aneurysms. Another patient had a fusiform aneurysm of the middle cerebral artery, which was resected accompanied by a reconstructive procedure of cerebral blood flow. The last patient had a fusiform aneurysm of the internal carotid artery. Following internal carotid artery occlusion surgery with superficial temporal artery-middle cerebral artery anastomosis, the aneurysm completely disappeared radiologically within several months.

Lateral situated sinus pericranii. Case report.

We hereby report a case of a laterally situated sinus pericranii, which is rare. According to the articles regarding this kind of lesion, carotid angiograms revealed no pathological findings, and venograms taken with direct injection of contrast medium into the mass were useful. It should be stressed that the lesion could be demonstrated in the late phase of carotid angiograms, and digital subtraction angiography was especially useful for diagnosis. The literature is also reviewed.

Radiosensitization by a new nucleoside analogue: 1-[2-hydroxy-1-(hydroxymethyl)ethoxy]methyl-2-nitroimidazole (RP-170).

A new potent hypoxic cell sensitizer, a 2-nitroimidazole nucleoside analogue having methoxyglycerol as a sugar moiety at the N-1 position of the imidazole ring (RP-170), has been synthesized. Its radiosensitizing activities in vitro and in vivo were investigated and compared with those of misonidazole (MISO) and etanidazole (SR-2508). As might be expected from the almost identical electron affinities of the three compounds, they were equally effective against hypoxic EMT6 cells in vitro. The in vivo-in vitro excision analysis showed that RP-170 was also as effective as MISO and etanidazole to radiosensitize solid tumor cells in vivo. An intraperitoneal administration of 200 mg/kg of RP-170 and an intravenous administration of the same dose of etanidazole showed an equal sensitizer-enhancement ratio of 1.51 to solid EMT6/KU tumors. Its effectiveness was also demonstrated by growth delay assay using solid SCCVII tumors. As predicted from the low partition coefficient, RP-170 and etanidazole showed apparently lower toxicity in vivo than MISO; their LD50/14 were 4.3, 4.8, and 1.8 g/kg in our experiment, respectively. Moreover, RP-170 showed fast clearance from serum in mice (t1/2 = 10.24 min) and poor penetration into neural tissues. Although RP-170 does not show any advantages over etanidazole in terms of sensitization or toxicity, RP-170 might be preferable under certain circumstances because it can be given orally.

[The clinical course of chronic methamphetamine psychoses].

3 cases with chronic methamphetamine psychoses (MAP-P) were studied from the clinical psychiatric point of view and discussed: 1.) The average time of occurrence of psychotic symptoms in our 3 cases after beginning of MAP abuse was two or three years. 2.) Although the psychotic states (often schizophrenia-like) were stable for a short time, the symptoms recurred promptly after reinjection of a small amount of MAP, unspecific stress or drinking sake (flashback phenomenon). The recurrence may happen even after several years. 3.) In cases of chronic MAP-P disturbances of consciousness are rare. Chronic process of MAP-P cases may show acute transient disturbances of consciousness (flashback phenomenon). 4.) The change of affects and behaviour ranges between irritable and indifferent attitudes. While there are close connections between the disturbance of feeling and change of personality (Wesensänderung), it is very difficult to decide whether premorbid character traits are dominant or there is a change of personality or emotional disturbance. 5.) About acute psychic symptoms which are responsible for recurrence of MAP-P, MAP-abusers make the experience that their intimate personal affairs are overlooked and become public (uncovered defensive experiences). In chronic cases of MAP-P we often meet patients with hypertrophic self-feeling or delusion of grandeur predominant in the symptomatology. 6.) In our cases sensations were amalgamated into each other and there were hallucinatory experiences without clear division between them. 7.) In chronic cases the auditory hallucinations criticized or censured the patients for their behaviour. Thought echoing (Gedankenlautwerden) was found. We found signs of mind reading (Gedankenausbreitung).

Biological and immunological properties of Sugi basic protein-pullulan conjugate. I. Suppressive effect on IgE antibody production and on IgE-mediated reactions.

Sugi basic protein (SBP), a major allergen of Japanese cedar pollen, conjugated to pullulan acquires suppressive activities for IgE, but not for IgG antibody production, and for IgE-mediated PK reactions in a SBP-specific manner. When either normal or IgE anti-SBP antibody-producing mice were treated with the conjugate, IgE anti-SBP antibody levels scarcely increased even after immunization with SBP + alum. This suppression was shown to be SBP specific and IgE selective. The ability of SBP-pullulan conjugate to elicit PK reactions on passively sensitized rat skin was markedly reduced. Furthermore, the conjugate inhibited the reactions mediated by the IgE antibodies and native SBP when it was injected into the IgE antibody-sensitized sites 2 h prior to SBP challenge. The reduced ability of the conjugate to elicit PK reactions was not due to loss of antigenic determinants during the conjugation procedures because binding activity of the conjugate to anti-SBP antibodies was as strong as that of native SBP. These results indicate that SBP-pullulan conjugate may be useful for 'desensitization therapy' against the pollinosis.

Measurement of allergens associated with dust mite allergy. II. Concentrations of airborne mite allergens (Der I and Der II) in the house.

Assays of mite allergens (Der p I, Der f I and Der II) in the air of houses became feasible with the use of a low-noise air sampler and a sensitive radioimmunoassay described previously. The levels of the airborne allergens Der I (Der p I + Der f I) and Der II in the living room of 10 houses during usual domestic life were very low, 29.5 and 6.3 pg/m3, respectively, with a Der I: Der II ratio of 4.7:1. At the time of bedmaking, they greatly increased, about 1,000-fold, to 30,900 and 12,600 pg/m3, respectively, with a Der I: Der II ratio of 2.5:1. The amounts of Der I and Der II in the floor dust of the living room were 2,040 and 2,690 ng/g of fine dust, respectively, with a Der I: Der II ratio of 0.8:1. Der I seemed more prone to become airborne than Der II.

Atlantoaxial laxity in rheumatoid arthritis.

We found that 14 of 162 rheumatoid arthritis patients with chronic occipitocervical pain had anterior atlantoaxial instability in the absence of any corresponding radiographic changes in the joint cartilage or subchondral bone. Our findings suggest that ligamentous instability is a prerequisite for this type of change. At the time of the detection of the instability, the median duration of disease was 12 (6-28) years. Rheumatoid occipitocervical pain may be initially caused by facet-joint arthritis or inflammation in the ligaments, and at a later stage also by irritation of the C2 nerve roots.

Tissue conditioners containing poly (butyl methacrylate) powder. I. Viscoelastic properties of homopolymer/plasticizer mixture.

The viscoelastic properties of new experimental tissue conditioners which consist of synthesized poly (butyl methacrylate) PBMA and a liquid of aromatic-ester plasticizer, without alcohol, were investigated to evaluate the effects of the molecular weight of polymer. The master curves of storage modulus G', dynamic viscosity eta' and loss tangent tan delta were constructed from the viscoelastic data at different temperatures. The effects of the molecular weight of polymer on the viscoelastic properties G' and tan delta were observed particularly, in a low frequency range. However, the dynamic viscosity eta' was not very sensitive to the molecular weight. These results suggest that PBMA produces more desirable properties for tissue conditioners, and that it is possible to manufacture new materials without alcohol.

Mode of action of erythropoietin (Epo) in an Epo-dependent murine cell line. I. Involvement of adenosine 3',5'-cyclic monophosphate not as a second messenger but as a regulator of cell growth.

A subclone of the interleukin 3 (IL-3)-dependent murine cell line DA-1 was recently established, and its growth found to be dependent on erythropoietin (Epo) as well as on IL-3. This subclone, named DA-1ER, has been used to study the mechanisms of action of these two growth factors, though most especially of Epo. In the present study, the involvement of adenosine 3',5'-cyclic monophosphate (cAMP) in the growth of DA-1ER cells was analyzed. In initial experiments, cAMP levels in cells stimulated with either recombinant Epo or recombinant IL-3 were monitored. The results showed that no significant changes in cellular cAMP levels had occurred. The cAMP-enhancing agents, N6-2'-o-dibutyryl-adenosine 3',5'-cyclic monophosphate (dbcAMP), 3-isobutyl-1-methyl-xanthine (IBMX), and cholera toxin (CT) were then applied to DA-1ER cells to see whether they could induce cell growth, but none of them was effective. The data indicated that cAMP was not the so-called second messenger for Epo or IL-3. The regulatory effects of cAMP on Epo- and IL-3-stimulated cell growth were next examined. It was found that whereas Epo-stimulated growth was markedly inhibited by cAMP-enhancing agents, IL-3-stimulated growth was relatively resistant and inhibited only by high doses of these agents. These data suggested that cAMP could play different roles in the same cells, depending on which growth factors were applied, and that it finely regulated proliferation and differentiation of multireceptor-bearing hematopoietic precursor cells.

Axonal transport and distribution of endogenous calcitonin gene-related peptide in rat peripheral nerve.

Calcitonin gene-related peptide (CGRP) has been found in both sensory and motor neurons. It has been suggested that CGRP is transported from neuron cell bodies to their terminals, where it may act as an anterograde trophic factor. However, it is not known how fast CGRP is transported or whether CGRP found in the innervated target organ indeed originated in neural tissues. We have quantified endogenous CGRP in the rat peripheral nerve by a newly developed enzyme immunoassay. The CGRP immunoreactive material obtained from neural tissues coincided with synthetic rat CGRP in fractional distributions separated by gel filtration. After ligation of the sciatic nerve, tissue CGRP accumulated in the segment central to the ligature. The rate of anterograde transport of CGRP was about 1 mm/hr in both sensory and motor fibers. In the sciatic nerve, only a small fraction of CGRP measured was found to originate from the motor nerve fibers. This may be due in part to the disproportionately large number of sensory fibers in the sciatic nerve and in part to the possible presence of CGRP in sympathetic nerve fibers. The CGRP content in the dorsal root fibers was significantly lower than that in the peripheral processes of the sensory neurons. The CGRP content in the hind leg muscle was much higher than that expected from the amount of CGRP per nerve fiber in the sciatic nerve. Most CGRP in muscle disappeared following denervation. It is concluded that CGRP highly concentrated in nerve terminals is supplied by axonal transport from the neuron cell bodies.

Development and distribution of the major pollen allergen (Cry j I) in male flower buds of Japanese cedar (Cryptomeria japonica).

We investigated the production of the major pollen allergen [Cry j I] of Japanese cedar (Cj) in the course of male flower bud development. We found that most of the pollen was at the tetrad stage in early September, and they developed to the mature stage in mid-October (1987) or early November (1988). Large amounts of Cry j I seemed to be extractable to ABS solution from the mature stage pollen but not from the pollen at other stages, that is, the tetrad and immature stages. Mature pollen could be ruptured with ammonium bicarbonate buffer, but tetrad and immature pollen could not. By immunofluorescent technique, antigen Cry j I was detected in the pollen only at the mature stage of Cj pollen development. Therefore, we think that Cry j I is produced at the time of pollen maturation.

Particle size of airborne mouse crude and defined allergens.

Laboratory animal allergy is a serious occupational diseases of many workers and scientists engaged in animal experimentation. Control measures depend upon characterization of allergens including airborne particles. This study measured the particle size of crude mouse urine and pelt aeroallergens generated in mouse housing rooms and compared them with mouse serum albumin, a defined major allergen. Allergens were detected by specific immunological methods. Most crude and defined allergens (74.5-86.4%) concentrated on a filter with a retention size greater than 7 microns. In distrubed air, allergen concentration increased 1.4 (albumin) to 5 (crude) fold and the proportion of small particles increased from 1.4% in calm air to 4.5% in distrubed air. This information on the generation and size distribution of aeroallergens will be important in the development of effective counter measures.

Evaluation of dust respirators for elimination of mouse aeroallergens.

The efficiency of various dust respirators for eliminating mouse allergens [mouse urine proteins (MUP), pelts proteins (MPP) and serum albumin (MSA)] were evaluated with use of low-volume air samplers and immunochemical methods. Three kinds of dust respirators from one manufacturer which have different efficacy in the exclusion of dust particles were put on the fiber glass filter in each air sampler. Then the air in a mouse housing room was sampled. The allergens passed through the respirators, were trapped in the fiber glass filters, and then extracted from the filters. The allergens of MUP and MPP in the extract were measured by an inhibition method of fluorometric enzyme-linked immunosorbent assay (ELISA) for IgE antibody and those of MSA measured by a fluorometric sandwich ELISA. The respirator with the lowest capability of exclusion was found to eliminate 65-86% of respective allergens. The other two respirators with higher powers eliminated 98% of MUP. MPP and MSA were eliminated to undetectable levels through these respirators. This study provided a means for the evaluation of dust respirators for animal aeroallergens.

N-terminal amino acid sequence of a major allergen of Japanese cedar pollen (Cry j I).

A purified preparation of a major allergen of Japanese cedar pollen, sugi basic protein (SBP, Cry j I), was separated into 5 subfractions of 50-45 kDa. All of the SBP subfractions were confirmed to be reactive to IgE antibodies from patients with Japanese cedar pollinosis, and also to mouse anti-SBP monoclonal antibodies. The sequences of 20 N-terminal amino acids of these 5 subfractions were found to be identical. Peptide mapping analyses of the SBP subfractions showed similar patterns, with some differences which might in part be due to the existence of an N-linked carbohydrate chain. The N-terminal amino acid sequence of SBP was identical to the reported sequence of an allergen of mountain cedar which vegetated in North America.

Effect of dietary eicosapentaenoic acid on azoxymethane-induced colon carcinogenesis in rats.

The effects of eicosapentaenoic acid (EPA, n-3 polyunsaturated fatty acid) and linoleic acid (n-6 polyunsaturated fatty acid) on azoxymethane-induced colon carcinogenesis in rats were studied. Male Donryu rats were given two types of semipurified diet containing 4.7% EPA plus 0.3% linoleic acid and 5% linoleic acid. The rats were given s.c. injection of azoxymethane (7.4 mg/kg body weight once a week for 11 weeks) and sacrificed 15 weeks after the last injection of azoxymethane. The tumor incidence and tumor yields (tumors per rat) of the colon were significantly lower in rats on the EPA diet compared to those on the linoleic acid diet; i.e., 33%, 0.41 +/- 0.61 and 69%, 1.66 +/- 1.69, respectively. In the analysis of phospholipid fatty acid composition, the colon tumor showed higher levels of arachidonic acid and lower levels of linoleic acid than those in the normal colon mucosa in both diet groups. Despite the increase of arachidonic acid in colon tumor, the EPA diet suppressed the excessive production of prostaglandin E2, which may be accompanied with neoplastic formation, whereas linoleic acid diet caused a marked increase in the tumor content of prostaglandin E2 compared to normal colon mucosa. These results suggest that EPA exerts its inhibitory effect on colon carcinogenesis by modulating lipid metabolism and inhibiting prostaglandin E2 synthesis in tumor cells.

Antibodies against basic fibroblast growth factor inhibit the autocrine growth of pulmonary artery endothelial cells.

Anti-recombinant human basic fibroblast growth factor (rbFGF) antibodies efficiently inhibited the basal proliferation of bovine pulmonary artery endothelial (BAE) cells. The cell-free extract of BAE cells stimulated the proliferation of bovine capillary endothelial cells and this activity was completely abolished by the antibodies. Furthermore, on heparin HPLC the activity was eluted at exactly the same retention time as that for authentic pituitary bFGF. These observations directly indicate that the BAE cells produce bFGF that stimulates their own basal growth by binding to specific receptors expressed on the cell surface.

Human interferon-gamma lacking 23 COOH-terminal amino acids is biologically active.

We constructed five mutated cDNAs encoding human interferon-gamma (IFN-gamma) derivatives lacking 19-23 COOH-terminal residues and expressed them in Escherichia coli. All the derivatives were purified to homogeneity. They showed substantially the same order of antiviral activity in vitro as the intact molecule, and behaved as a dimer. The far- and near-UV circular dichroism spectra of the derivatives were quite similar to those of the intact one. These results indicate that the 23 COOH-terminal amino acids at least are not essential for achieving the full antiviral activity and constructing the higher structure of human IFN-gamma.
