Neurogenic hypernatraemia: case report.

A 49-year-old man was submitted to neurosurgery for a cranio-pharyngioma. The lesion, which appeared to involve the antero-inferior wall of the third ventricle, caused lack of appropriate antidiuretic hormone (ADH) release in response to hypernatraemia and plasma hyperosmolality. The probable mechanism of this hypothalamic syndrome is suggested.

Intrinsic haematopoietic activity of cerebellar haemangioblastomas. Ultrastructural study of three cases.

Three cerebellar haemangioblastomas were studied by electron microscopy. One of these shows electron-dense granules within the tumour cells, indicative of erythropoietic activity, as previously described in a haemangioblastoma. The biological significance of these granules is discussed. Biological assay of serum erythropoietic activity was performed in one case.

Benign intracranial hypertension without papilledema: case report.

An obese young man presented the typical features of benign intracranial hypertension (BIH), including raised intracranial pressure. Despite elevated intracranial pressure, papilledema did not develop. The syndrome resolved after medical treatment and, on examination 6 months later, the patient was normal. The absence of papilledema in this case is discussed together with data available from the literature.

Scanning electron microscopy (s.e.m.) of biopsy specimens of ruptured intracranial saccular aneurysms.

Surgical specimens of 4 intracranial saccular aneurysms were studied by scanning electron microscopy. The internal surface of the aneurysms showed crater-like defects and cytoplasmic bridges. In some areas the endothelium was preserved, but its longitudinal convolutions were higher and thicker than those found in unchanged areas. On the damaged endothelial surface there was an increased number of blood cells. The adventitia resembled that of a normal cerebral artery. In conclusion the alterations observed are similar to those found in atherosclerosis and are most likely due to the high wear and tear provoked by the blood streaming into the aneurysm. The results of this study are in agreement with the findings of a companion transmission electron microscopy study and emphasize the importance of degenerative changes on the development and rupture of intracranial saccular aneurysms.

Histological and ultrastructural study of intracranial saccular aneurysmal wall.

The material studied consists of 10 cases of intracranial saccular aneurysms. Four came from autopsies, and in each of the other six aneurysmal wall was obtained at surgery after clipping of the aneurysm. The most significant findings from this pathological study are the almost complete disappearance of the internal elastic lamina at the level of the aneurysmal neck, sclerosis of the muscle coat, and in satellite vessels and vasa vasorum disruption of the internal elastic lamina and partial luminal occlusion. The importance of ischaemic changes in the aneurysmal wall is discussed. Rupture of the aneurysm at the distal extremity of the sac depends probably on the progressive brittleness of its wall which becomes sclerotic and less resistant to the blood pressure within. Splitting or rupture of the aneurysm appears to be dependent on degenerative ischaemic alterations in its wall.

CT and ACTH treatment in infantile spasms.

Computed tomography of 8 cases with West's syndrome before, during and after ACTH treatment are reported. The scans, performed at the third week of therapy, showed consistent widening of the sulci, cisterns and ventricles in all the patients. Of these, 2 patients underwent ICP monitoring which showed higher than normal values. A return to the normal ICP values in association with the disappearance of the CT findings was observed in both cases. It is concluded that widening of the sulci, cisterns and ventricles are not findings of atrophy, but a condition of initial communicating hydrocephalus, which is in accordance with the hypotheses of Riikonen and Lyen.

[Temporal post-traumatic epilepsy: corticographic and ultrastructural study of a case].

A case of post-traumatic temporal epilepsy, submitted to a temporal lobectomy, is reported. During surgery, electrocorticography was carried out in order to differentiate epileptic areas from normal tissue. The ultrastructural study of nervous tissue removed from both normal and epileptogenic areas showed diffuse and aspecific process of neuronal alterations without a correlation to the electrographic epileptic areas. The importance of trauma in the evolution of these alterations is discussed.

[Morphological changes in prolactinoma induced by bromocriptine treatment].

The structural modifications to PRL-secreting pituitary adenomas induced by medical treatment with bromocryptine are described. Whereas the consistency of the tumour diminishes after a shgort period of treatment, making surgical removal easier, it increases after a long period (years) owing to interstitial fibrosis phenomena that impede surgical removal.
