Physician-patient electronic communications.

Patients who do not have adequate access to their physicians may feel isolated from decisions concerning their medical treatment and care and believe they have been abandoned to an impersonal health care system. Some physicians are addressing these concerns by adopting new technologies such as e-mail communication. The authors discuss some of the promises and challenges unique to physician-patient electronic communication.

Morphological correlates of urinary enzyme loss after extracorporeal lithotripsy.

Urinary loss of the tubular marker enzyme N-acetyl-beta-D-glucosaminidase (NAG) immediately following extracorporeal lithotripsy suggests corresponding morphological changes in the kidney. To date, the morphological correlate of the enzymuria remains unclear. In this animal study with Wistar rats acute morphological changes in the tubulus cells beneath isolated tubulus necrosis were demonstrated. The mechanically induced lesions of the cell organelles included fragmentation of the lysosomes and severe alterations of the cell membrane. The tubulus damage was quantified. With the help of histochemical NAG staining and electron microscopic observations, a significant correlation between number and intensity of shock waves and tubular damage was found. The intracellular lesions described here are at least part of the morphological basis of shock-wave-induced enzymuria. The results show that enzymatic changes in urine reflect visible renal damage.

Effects of rib cage or abdominal restriction on lung mechanics.

The effects on lung mechanics of equal (37%) reduction in total lung capacity (TLC) by rib cage or abdominal restriction were studied in 10 healthy males. Lung recoil pressure (Pst) was simultaneously measured from three sites in the esophagus. This also provided an estimate of the vertical pleural pressure gradient (PPG). Deformation of the right hemithorax was quantified by roentgenograms in three subjects. At the same lung volume, abdominal restriction decreased lung height and increased anteroposterior diameter compared with the control case, whereas rib cage restriction had opposite effects. Maximum expiratory flow increased equally with both types of restriction, and average Pst increased equally with both types of restriction. There was a significant correlation between degree of TLC reduction and increase in Pst that was similar for both types of restriction. This study indicates that changes in lung mechanics depend primarily on the amount of volume reduction and not on the type of deformation producing the volume decrease.

Costs under capitation.

Physician practice costs can be categorized as either fixed (salaries, rent) or variable (office supplies, medical supplies, some labor costs). An optimal mix of these fixed ratio will change with fluctuations in patient volume and can be dramatically different in a capitated vs. a fee-for-service environment. The authors discuss techniques for calculating a practice's fixed and variable costs, the effect of volume changes on cost structures and ways to minimize negative financial consequences of declining patient loads.
