Quincke's edema in a dialysis patient after administration of acrylic bone cement: possible role of ethylene oxide allergy.

A 33-year-old female dialysis patient suffered from osteomyelitis and luxation of the dens axis with cervical myelopathy. In the past she had had three attacks of anaphylaxis after treatment with dialyzers that had been sterilized with ethylene oxide. IgE-type antibodies directed against human serum albumin-ethylene oxide complexes could be demonstrated in the patient's serum by radioallergosorbent techniques. Immediately after an operation in which acrylic bone cement (Palacos-R) sterilized with ethylene oxide was implanted for stabilization of the cervical spine, the patient developed massive edema of the larynx, pharynx, and tongue, suggesting Quincke's edema. It is concluded that ethylene oxide present in acrylic bone cement may induce acute allergic reactions in sensitized patients. Dialysis patients may be at special risk, since the incidence of ethylene oxide allergy in this patient population is about 10%.

Large-scale reductions in speeding and accidents in Canada and Israel: a behavioral ecological perspective.

We assessed the effects of posted feedback and warning ticket programs on speeding and accidents in two cities. In Experiment 1, speeding feedback signs were effective even when 10 were used in Dartmouth, Nova Scotia, and reductions in speeding were associated with reductions in accidents. The effectiveness of the signs was correlated with the number of intersections and residences within 0.5 km beyond them, and the signs had no effect on untreated streets. In Experiment 2, posted feedback and a warning program reduced speeding and accidents on 14 streets in Haifa, Israel.

[X-ray diagnosis of ovarian carcinoma].

Methods of radiology, lymphography, and computer tomography included, are not used in all patients with ovarian cancer, neither in the preoperative phase nor in the search for the recurrence. Therefore these improved X-ray diagnostic possibilities are discussed. We describe the role of lymphography, sonography, and computer tomography in the diagnosis of ovarian cancer.

Attention and visual reaction time in schizophrenia.

Schizophrenic behavior is sometimes characterized by inefficiency and symptoms possibly attributable to difficulties in coordinating attention. In fact, breakdown in cognitive and perceptual processes may be secondary to an initial disorder in mechanisms which coordinate focused attention. These processes were therefore tested in four subgroups of 10 schizophrenics each and in a normal comparison group (N = 10). A visual reaction time task was designed to operationalize definitions of width of cue utilization (responses to number of cues and position of cues), selective attention (color of cues), and ability to develop a set (probability of cues). Perceptual deficit was most prominent among long term nonparanoid schizophrenics who reduced their observation to central position cues when faced with a large number of potentially distracting stimuli. These patients, as well as short term nonparanoid schizophrenics, often failed to differentiate color even though this stimulus information provided an alternative method for reducing the number of stimuli requiring attention. Short term nonparanoid schizophrenics showed broad but disorganized attention to visual cues and inability to develop a probability set. Paranoid schizophrenics were not deviant in coordinated attention.

Atherosclerosis and mortality statistics.

WHO mortality statistics are subject to severe criticism all over the world due to the ICD code, the monocausal evaluation and th inhomogenity of data which are only partially verified by post mortem findings. Despite this fact WHO statistics are the only possibility to compare morality in different areas. In order to prove their reliability we have collected data of patients who have died in 9 Vienna hospitals. Our material consists of more than 15000 out of 25000 death cases in the year 1976. The autopsy rate was about 70%. To handle this amount of datas a computer system was used. In comparison with the official Austrian statistics our total atherosclerosis rate was higher due to the multicausal evaluation and the much higher percentage of CHD. Atherosclerosis of the brain was found in a lower percentage than officially mentioned when autopsy was performed. An example for single case studies is the myocardial infarction in a defined age group to support the importance of statistics based on a high rate of autopsies and the use of electronic data processing.

Low-molecular- weight Rauscher leukemia virus protein with preferential binding for single-stranded RNA and DNA.

A sensitive nitrocellulose filter assay that measures the retention of 125I single-stranded calf thymus DNA has been used to detect and purify DNA-binding proteins that retain a biological function from Rauscher murine leukemia virus. By consecutive purification on oligo (dT)- cellulose and DEAE-Bio-Gel columns and centrifugation in 10 to 30% glycerol gradients, RNA-dependent DNA polymerase has been separated from a second virion DNA-binding protein. The binding of this protein to DNA was strongly affected by NaCl concentration but showed little change in activity over a wide range of temperature or pH. After glycerol gradient purification, polyacrylamide gel electrophoresis of this protein showed one major band with a molecular weight of approximately 9,800. This protein binds about as well as to single-stranded Escherichia coli or calf thymus DNA or 70S type C viral RNA. The binding to 125I single-stranded calf thymus DNA is very efficiently inhibited by unlabeled single-stranded DNA from either E. coli or calf thymus and by 70S murine or feline viral RNA. Much larger amounts of double-stranded DNA are required to produce an equivalent percentage of inhibition. This protein, therefore, shows preferential binding to single-stranded DNA or viral RNA.

[Experimental and clinical studies on the relevance of proton spin tomography in patients with coxarthrosis].

We examined 2 in formalin fixed hip specimens from a corpse, 27 patients with coxarthrosis and 3 healthy young volunteers with MRT concerning the illustration of the hip joint and pathological changes by coxarthrosis. Specimen of the femoral head after endoprosthetic operation could be achieved in 18 cases for comparison with MRT. All pathological alterations as cartilage alterations, sclerosis, cysts, effusion, edema, fibrosis were figured with high sensitivity. Cartilage was figured much better by gradient echo sequence with short echo time than by spin echo sequence. Gradient echo sequence made it possible to figure and quantify thinning of cartilage with high sensitivity. Both sequences showed artefacts because of the chemical shift in the area of the cartilage-bone boundary, which impaired especially the judgement of cartilage by spin echo sequence. Gradient echo sequence allowed because of the phase-contrast effect the distinction of hematopoietic and fatty bone marrow and possibly the delineation of bone marrow edema.
