Erysipelas. Changing faces.

A retrospective study of 526 adult patients admitted to the Sheba Medical Center for treatment of erysipelas during the period 1972-1982 showed a steady increase in the number of cases admitted as well as a striking change in its classical pattern of distribution, with 86% of the lesions occurring in the lower extremities opposed to 5.9% in the face. Local trauma and superficial abrasions were identified as the major predisposing factors, whereas upper respiratory tract infection was almost negligible among the patients. Peripheral vascular disease, diabetes, and prolonged treatment with corticoids, immunosuppressants, or radiation were less frequently associated with the development of the disease. New trends were also observed for the age groups and seasonal distribution of the population studied.

Reactive perforating collagenosis.

Seven patients with reacting perforating collagenosis (RPC) were diagnosed from 1974 to 1982. Experimental production of lesions in two patients confirmed the contributory role of superficial trauma. The pathogenesis of RPC involves transepithelial elimination of altered collagenous bundles. This process may be observed in a wide variety of disparate skin diseases.

Painful piezogenic pedal papules on a child with Ehlers-Danlos syndrome.

A 5-year-old girl with Ehlers-Danlos syndrome developed painful piezogenic pedal papules. She had suffered from pain in her heels for the previous two years and had undergone extensive orthopedic examinations that revealed no abnormalities. On admission, she had typical signs associated with piezogenic pedal papules (PPP), with pain induced by standing and disappearance of the lesions and the pain on relief of pressure. These papules were due to herniation of subcutaneous fat into the dermis, possibly because of structural defects of the connective tissue. To the best of our knowledge, this is the first reported case of painful PPP occurring in childhood. We suggest that the herniation of subcutaneous fat in our patient was most probably due to the connective tissue defect that occurs in the Ehlers-Danlos syndrome.

Transient acantholytic dermatosis occurring during pregnancy.

A case of transient acantholytic dermatosis (TAD) in pregnancy is reported. The eruption appeared in the third month of the pregnancy and completely resolved two months later. Reports concerning the occurrence of this disorder in young people are rare, and, to the best of our knowledge, this is the first patient in whom onset of the disease has been detected during pregnancy.

Urticaria and fixed drug eruption in a patient treated with griseofulvin.

We report a case of urticaria and fixed drug eruption ( FDE ) caused by griseofulvin. Although griseofulvin-induced urticaria is well documented, FDE secondary to griseofulvin is distinctly uncommon. Moreover, the appearance of urticaria and FDE in the same patient following treatment with griseofulvin is, we believe, the first to be described in the literature. The concept of FDE is briefly reviewed.
