[Satisfaction following automated percutaneous lumbar discectomy as a function of clinical parameters].

182 patients assessed their condition after automated percutaneous lumbar discectomy (APLD). The analysis of descriptive data parameters++ provides new guidelines for the indication of this minimally invasive procedure: persistent leg pain despite conservative treatment, lumbar disc prolapse in one segment, minimal monological pathology, This ideal constellation implies a restriction of the common guidelines. Under these premises 50% of the patients will be satisfied. So the indication for APLD seems to be justified under more unfavorable patient profiles if the natural course of sciatica was poor.

[A special case: fracture of the spine with spondylodesis].

Fracture of spondylodesis is rare. The case of a 31-year old woman, who incurs a fracture of the fused spine 18 years after Risser's spondylodesis, is reported. This case is contrasted with two other cases exclusively known by literature.

[Sonographic diagnosis in injuries of the shoulder joint].

The value of sonography for diagnosis of injuries of the acromioclavicular joint was examined in a prospective investigation. In 22 successive patients sonography was used in two defined positions, the results were compared with standard X-rays under weight loading. Only in 9 cases the findings could be controlled during operative exploration. In 16 cases, sonographic and radiologic findings were corresponding, in 6 cases both methods lead to different results. In all corresponding cases sonography was able to demonstrate direct or indirect signs of the damage of acromioclavicular and coracoclavicular ligaments. In the operatively controlled group in all cases the sonographic diagnosis was confirmed.

[Results of resection of the head of the ulna].

A follow up of 57 wrists with resection of caput ulnae shows different value to different groups of patients. The painful arthrosis of distal radioulnar joint gives indication in posttraumatic cases. Release of pain and improvement of range of motion will be achieved; loss of strength is the most impairing problem for younger patients. 13 patients out of 21 with posttraumatic deformities were satisfied with the post-operative outcome; 13 patients showed significant loss of strength, too. In case of rheumatoid arthritis with caput-ulnae-syndrome the resection is without alternative, needs to be combined with the arthrodesis of radiolunar-joint, if necessary. 20 of 34 rheumatics were satisfied with the outcome after resection of caput ulnae; in 6 cases an ulnar drift was evident, which implied simultaneous or later partial arthrodesis of the wrist. For all patients release of pain decided on their satisfaction with surgery.

[Results of surgical therapy of knee flexion contractures in patients with myelomeningocele].

This study reports the results of 98 operations for correcting knee-flexion-contractures, which were performed between 1972 and 1989 in 60 patients with myelomeningocele. 13 knees had hamstring lengthening and 85 knees had radical flexor release. In 4 patients, who had flexion-contractures of more than 50 degrees, the soft tissue release was combined with supracondylar extension osteotomy. 58 patients had additional hip-flexion-contractures and 39 patients had feet deformities. The average age at time of surgery was 8 years, 3 months. The average follow-up-period was 65 months with a minimum of 1 year and a maximum of 13 years. In 92 knees a permanent extension ability could be achieved. In 6 knees a recurrence of flexion-contracture occurred making a second surgery necessary. The main problem in the postoperative period were 13 skin necroses, which were seen mainly after an s-shaped incision. As a conclusion a perpendicular midline incision is recommended. After surgery 11 patients could be provided for the first time with an orthoses. In the remaining the upright body position or the erection of the orthoses could be improved.

[Thiemann disease in monozygotic female twins].

Thiemann's disease is a hereditary necrosis of the epiphysis of the basal limb of the big toe. This paper describes its synchrone occurrence in monozygotic twins. The course is seen as being concurrent with the findings in literature. The etiopathology of Thiemann's disease is discussed showing the dominant heritage by this examination of twins.

[Hereditary progressive arthro-ophthalmopathy (Stickler syndrome)].

The hereditary arthro-ophthalmopathy (Stickler-Syndrome) is a relatively frequent dominantly inherited disorder of connective tissue. The main symptom is strong congenital myopia, which develops often to blindness. On the other hand there is a dysplasia of the meta- and diaphysis of the tubular bones, which varies in form and expression. Within difficulties with differential diagnosis to near syndromes it is important to get a clear diagnosis by ophthalmologic, radiologic and family investigations. Within three members of a family the orthopaedic symptomatology is shown. In case of Stickler-Syndrome surgery in the skeleton system should be prevented.

Therapy outcome after multidisciplinary treatment for chronic neck and chronic low back pain: a prospective clinical study in 365 patients.

OBJECTIVES: This prospective longitudinal clinical study analyses the therapy outcome of 365 patients with either chronic neck (n = 134) or low back (n = 231) pain treated with a multidisciplinary biopsychosocial therapy approach. METHODS: Patients with chronic neck pain (NP) or low back pain (LBP) for 3 months or longer, corresponding sick leave for longer than 6 weeks, and clearly defined inclusion and exclusion criteria underwent a 3-week standardized inpatient multidisciplinary biopsychosocial therapy. Baseline sociodemographic, occupational, functional, and psychological data at entry into the study (T0) were comparable in both groups. At the 6-month follow-up (T1), five different therapy outcomes were analysed in both groups: back-to-work status, generic health status (the 36-item Short Form Health Survey, SF-36), pain intensity (visual analogue scale), functional capacity (Hannover back capacity score), and satisfaction with the therapy. RESULTS: Both treatment groups improved significantly in all outcome criteria between T0 and T1. In the total group, the back-to-work rate was 67.4%. At the final follow-up there were no significant differences between the group with chronic NP and the group with chronic LBP in the outcome criteria back-to-work status, improvement of health status and functional capacity, satisfaction with therapy, and reduction of pain. CONCLUSION: Evaluation of the main results of this study suggests that patients with chronic NP also derive significant benefit from a multidisciplinary treatment strategy, demonstrated in the literature so far mainly for patients with chronic LBP.
