[Inguinal hernia surgery in 1993 in Slovakia].

The authors present an account of a survey (by questionnaires) focused on surgery of inguinal hernias. The recovery rate of questionnaires was 84%. The standard techniques in surgery of child hernias are Gross (29%) and Girard (29%). In primary hernias more than 50% are made by Bassini's and Girard's plastic operation. So far the worldwide accepted plastic operation of Shouldice is rarely used 14%. In the techniques of relapsing hernias also inadequate plastic operations are used; the ratio of Lichtenstein's plastic operation which has the lowest relapse rate is only 4.5%.

[The human spleen in idiopathic thrombocytopenic purpura].

Idiopathic (autoimmune) thrombocytopenic purpura (ITP, AITP) represents a relatively frequent impairment. It involves a syndrome of various diseases with a shortened thrombocytes survival caused by anti-platelet antibodies. The majority of cases are of secondary character. Spleenectomy often evokes a complete remission of thrombocytopenia. The study describes morphologic findings in spleens of 30 patients with the clinical diagnosis of ITP/AITP. The findings were gained by light microscopy from formol-paraffin blocks and histochemical findings from cryostat sections of non-fixed tissue. The alcaline and acidic phosphatases, nonspecific esterase, chloracetate esterase, and dipeptydilpeptidase IV were investigated enzymohistochemically. Immunoglobulins were examined immunohistochemically and T lymphocytes by means of monoclonal antibodies. The affinity HPA--Helix pomatia agglutinin, PHA--phytohemagglutinin from Phaseolus vulgaris, SBA--soy-bean agglutinin from Glycine max. and PSA--peas bean agglutinin from Pisum sativum were investigated by means of specific antilectin antibodies. The human spleen during idiopathic thrombocytopenic purpura accumulates neutrophilic polymorphonuclear granulocytes; platelets-stagnate and are destroyed. These processes can be identified in histologic sections e.g. also by means of anti-fibrinogen antibodies. The red pulp contains foam cells to various extent. Besides generally known processes, the white pulp also displays alterations in composition of cellular compartment of the periarterial lymphatic sheaths. Human spleen distinguishes modified blood platelets as alien corpuscles, and thus eliminates them from the blood circulation system by its immunologic and other mechanisms, the details of which still remain to be clarified. Germ cell testicular tumours with lung metastases: chemotherapy and surgical treatment.

Eighty patients with stage IV testicular germ cell tumours with lung metastases were treated with PVB chemotherapy and subsequent surgery in cases of residual disease. Out of 80 patients 28 (35%) achieved complete response following chemotherapy alone. Thirty-six patients (45%) with partial response underwent surgery: 17 had lymphadenectomy because of residual mass in the retroperitoneum, 15 had pulmonary surgery alone and 4 had both operations. Of these 36 patients 27 achieved complete response following cytostatic and surgical treatment. Sixteen patients died following PVB chemotherapy, 10 of them due to progression of disease, and there were six (7.5%) drug-related deaths. The authors refer to the importance of surgical treatment of residual metastatic mass in the lungs following PVB chemotherapy. Germ cell tumours of the testis are the most curable solid neoplasms treated by the oncologist. Advances in their management are due to the introduction of cisplatin-based combination chemotherapy and surgical removal of the residual mass [8]. Progress in chemotherapy of testicular tumours has changed the attitude towards thoracotomy and surgical removal of lung metastases. The aim of this study is to evaluate combined cytostatic and surgical treatment of disseminated testicular tumours with emphasis on surgical removal of residual lung metastases following chemotherapy.

[Combined therapy in patients with testicular germ cell tumors in the dissemination stage (pulmonary metastasis)].

The efficacy of combined cytostatic-surgical treatment was evaluated in a prospective study involving 70 patients with germinal tumors of the testis metastasizing into the lungs. Complete remission was achieved in 50 (71.4%) patients, 30 of these received only chemotherapy. Thoracotomy was performed in 14 patients with residual pulmonary CT finding, 6 of them underwent bilateral operations. Of the total 21 operations of the lungs residual malignant tumor was found 4 times. After a mean follow up period of 29.8 months since onset of treatment 48 patients (68.6%) survived. Within a mean of 11.4 months after onset of treatment 22 patients (31.4%) died. The authors consider thoracotomy with resection of the metastatic focus to be the only method for determining reliably the biological nature of the residual pulmonary lesion after chemotherapy. The result of the procedure determines the further measures, i.e. sequential chemotherapy or regular check ups. (Tab. 1, Fig. 6, Ref. 15.).

[Surgical treatment of residual pulmonary metastasis of testicular germ cell tumors after chemotherapy].

Advances in chemotherapy achieved a high percentage of complete remission after primary cytostatic treatment in germinal testicular tumours. In 30-40% of patients in the IVth stage of the disease supplementary surgery is, however, necessary--retroperitoneal lymphadenectomy or metastasectomy of the lungs. The authors give an account of initial results of surgical removal of metastases in the lungs in seven patients of 48 treated with cytostatics on account of a germinal testicular tumour. The low risk of thoracotomy, if correctly indicated, justifies us to describe this operation not only as auxiliary but therapeutic.

[Splenectomy in hairy cell leukemia].

Splenectomy was performed in five patients with hairy cell leukemia at the Surgical Department of the Hospital with Policlinic, Bezrucova, Bratislava, over the years 1982-1987. One of the patients died 3 years after operation due to progression of the disease, the other four patients have been surviving for 2-7 years following surgery without relapse of the disease. The presented results, compared with the world literature, justify splenectomy to be advocated as the primary therapeutic procedure in hairy cell leukemia.

Preservation surgery of the spleen.

Authors point out possibilities of preservation of the spleen or its parts in injuries and some diseases, especially non-parasitic cysts. Review is completed by a case report of successful management of a splenic pseudocyst by partial resection.

[Long-term results of surgical treatment of portal hypertension].

Large number of contradictory operative surgeries refer best to embarrasments, existed on this section of surgery. Today, from the amount of operative surgeries prevail the endoscopic sclerotization of the gastroesophageal varices. On basis of retrospective study of 48 portosystemic shunts was showed, that 28 operated patients survive average 8.1 year, till now 4 patients of them are alive, within 8-14 years. In 7 cases of azygoportal deconexion by Torres-Dengi, average time of survive was 2.5 year. There experiences led the authors to elaborate an algorithm of treatment of portal hypertension, where the postosystemic shunts are the portosystemic shunts are the effective palliative operative surgery in indicated cases.
