Production of substrate for galactose oxidase by depolymerization of an arabinogalactan-peptide from wheat flour.

Water extractable arabinogalactan-peptide (WE-AGP) isolated from white wheat flour was depolymerized enzymatically to liberate substrate for a galactose oxidase from Dactylium dendroides. A crude liquid pectolytic preparation from Aspergillus niger (p70) displayed activities capable of converting WE-AGP into a substrate for galactose oxidase. The most favorable substrate was observed when WE-AGP was not fully depolymerized into galactose and arabinose. alpha-L-Arabinofuranosidase B from A. niger was also able to produce substrate from WE-AGP; arabinofuranosidase-treated WE-AGP was a better substrate for galactose oxidase than galactose. Treatment by the crude p70 and purified enzymes showed that alpha-L-arabinofuranosidase was partly responsible for the production of substrate, whereas beta-galactosidase did not result in any substrate production or improve the effect of alpha-L-arabinofuranosidase. However, the positive effect of alpha-L-arabinofuranosidase was increased when p70 was added at the same level of arabinofuranosidase activity, suggesting that additional enzyme activities present in p70 were responsible for production of substrate for galactose oxidase.

Capsular contracture after breast reconstruction with the tissue expansion technique. A comparison of smooth and textured silicone breast prostheses.

Twenty consecutive breast reconstructions were done by the expansion technique. Either textured or smooth expander prostheses were used followed by silicone implants. Histological samples were taken from the capsule when the expander was removed and the implant inserted. The results show no difference in capsular formation between the groups, although the microscopic evaluations from the group with textured prostheses showed considerable numbers of silicone droplets. The patients were generally pleased with the results even though some reconstructed breasts were slightly firm when evaluated postoperatively.

[Permanent central venous catheters in oncologic patients].

Permanent central venous catheters play an important role in the treatment of patients with malignancies. Two systems are available, Hickman-Broviac Catheters (HB) and Port-a-Cath (PAC), the latter is fully implantable. Since 1982, the systems have been used in the Finsen Institute. In the period 01.01.1984 to 31.03.1988 a total of 232 HB and 52 PAC were inserted in 245 patients with solid malignant tumours with one exception. All the catheters were inserted with the venous cut-down technique and at the end of the period mainly in local analgesia. The total indwelling time was 36,859 days, mean 99 days (1-616 days) for HB and 92 days (8-519 days) for PAC. Seventeen catheters remained in place for more than 365 days. A total of 114 complications concerning 88 catheters were registered. The most serious complications were sepsis and thrombosis. The frequency of sepsis was low, 0.06 (HB) and 0.01 (PAC) per 100 catheter-days. The corresponding frequency of thrombosis was 0.03/100 catheter-days for both systems. Sixty-one HB and five PAC had to be removed because of complications. In general, the complication rate was lower for the PAC method. In addition, the nursing care was reduced to a minimum. The surgical cut-down method is preferable, as catheter implantation is possible even in patients with disorders of the coagulation system.

[Carcinoma of the urachus].

The urachus is an embryological structure originating from the allantois and situated in Retzius' space. Neoplasms originating from the urachus are very rare. They become clinically manifest late in the course of the disease. Deformation of the apex of the bladder and supravesical calcifications are described as radiological findings but these are of limited diagnostic value and should be supplemented by ultrasonic scanning, computed tomography and possibly by magnetic resonance scanning which will provide information about the extent of the disease. The final diagnosis is based on the macroscopic findings and the macroscopic picture. Three cases representing various aspects of the disease are presented. Treatment consists of en bloc resection in the form of partial cystectomy and excision of the tissue in Retzius' space and the umbilicus. The results are, however, disappointing as the five-year survival rates in the majority of materials do not exceed 10%. Irradiation is of doubtful value and chemotherapy is scarcely of value in the treatment.

Partial obstruction of the sigmoid colon by surgical implantation.

Invasive transitional cell carcinoma of the bladder is known to extend directly to other organs in the pelvis. When metastases occur they almost always adhere to a rather consistent pattern of dissemination. We present an unusual case of partial obstruction of the sigmoid colon due to a metastatic transitiocellular bladder tumour four months after transurethral resection of the tumour complicated with a perforation into the peritoneal cavity. Direct implantation of the tumour with the resectoscope is the most possible explanation, but seeding on an intact serosa cannot be excluded either.

Intraoperative cryolysis of intercostal nerves in thoracic surgery.

Pain is a major problem associated with thoracotomy. Intraoperative cryolysis of the intercostal nerves around the incision can essentially reduce the postoperative analgesic requirement and diminish the impairment of ventilatory capacity. To obtain maximum effect, it is recommended that the sites of drainage should also be treated with cryolysis.

Peritoneal adhesion formation after the use of oxidized cellulose (Surgicel) and gelatin sponge (Spongostan) in rats.

Experimental studies have suggested that oxidized cellulose and the combined use of gelatin sponge and dextran 70 may prevent intraperitoneal adhesion formation. Peritoneal adhesions were produced in 50 rats by a standardized caecal trauma. In 10 rats the injured caecum was covered by oxidized cellulose, in 10 by gelatin sponge, and in 10 by a mixture of gelatin sponge and dextran 70. Twenty rats served as controls. Neither oxidized cellulose, gelatin sponge, nor the combination of gelatin sponge and dextran 70 was found to be effective in preventing peritoneal adhesions. However, neither oxidized cellulose nor gelatin sponge resulted in an increased adhesion formation.
