[Induction of optokinetic nystagmus in infants and young children by a horizontally, diagonally or vertically moving striped pattern].

Horizontal optokinetic nystagmus (H-OKN) in infants is often described in contrast to diagonal (D-OKN) and vertical (V-OKN) nystagmus. No systematic investigation has been carried out to test the elicitability of D- and V-OKN in relation to the direction of the moving stimulus or the effect of stimulating the two eyes simultaneously or separately. We examinated the elicitability of H-, D- and V-OKN in more than 400 infants between birth and 5 years of age in two test situations: with both eyes open and with one eye covered. The stimulation device was a (hand-driven) drum of 12.2 cm diameter, on which a paper sheet with a pattern of black and white stripes (20 mm) kept in a horizontal, diagonal and vertical position, respectively. The distance to the infant's face was kept at ca. 25 cm. The occurrence of OKN was studied in all the six stimulation situations. Results demonstrated a clear developmental course in the monocular tests situation of the occurrence of OKN: at first only horizontal OKNs can be elicited, thereafter diagonal and finally vertical OKNs appeared. In general OKNs in all 6 testsituations are evident from the age of 3-24 months. However, in risico infants, deficits in at least one of the test situations were observed until the age of 5 years. Our results point towards a role of testing OKN as a screening method for detecting visual or brain dysfunction.

[Fresh extensor tendon injuries of the forearm and hand (management, results and problems)].

1944 patients with 2418 lacerations of extensor tendons in the hand were treated since 1966. Follow up study was possible in 1031 (53%) patients. The discussion about the best method to evaluate the results of treatment lead to a better scheme for critical examination. The preoperative status was taken into consideration as well as the subjective meaning and brought into harmony with the objective results.

[Measurement of left ventricle asynergy by various measuring procedures. II. Findings in patients with post-infarction states].

In 30 patients with postinfarction condition (11 infarctions of the posterior wall, 11 infarction of the anterior wall, 8 aneurysms of the anterior wall) the regional contraction of the left ventricle was analysed by 3 different metrical methods (semiaxis, surface and angular method) on the basis of the kineventriculogramme in the 1st oblique diameter. With all 3 methods a significant reduction of the mean contraction amplitude could be proved in the affected ventricular region. Using the semiaxis method, in all 30 patients an asynergy could be identified. By the surface method 2 patients with infarction of the posterior wall and 1 patient with infarction of the anterior wall, by the angular method 3 patients with infarction of the posterior wall and 1 patient with infarction of the anterior wall were not established. According to the disturbance of the entire function of the ventricle the number of the segment with disturbed contraction was largest in the group with aneurysm of the anterior wall and smallest in the group with infarction of the posterior wall. Even here the semiaxis method showed the greatest and the angular method the smallest sensitivity.

Functional imaging and detection of local recurrence in soft tissue sarcomas by positron emission tomography.

BACKGROUND: Besides anatomically-based radiodiagnostics, functional imaging may be used for characterizing soft tissue sarcomas (STS). This study evaluates positron emission tomography (PET) with three different radiotracers (fluorine-18 deoxyglucose [FDG], carbon-11 aminoisobutyric acid [AIB] and oxygen-15-labeled water [O15-water]) for imaging STS and the detection of local recurrence. PATIENTS AND METHODS: 21 patients presented with 9 primary STS, 5 recurrent STS and 10 lesions suspicious for local recurrence (radiological tomography). Standardized uptake values (SUV) were calculated in tumor and normal tissues (muscle/blood vessels). Surgery with pathological examination or follow-up data were referred to as control criteria. RESULTS: All tracers accumulated in STS with no difference between primary and locally recurrent tumors. The uptake in STS was increased compared to muscle (FDG, P < 0.0001; AIB, P = 0.0039; O15-water, P = 0.002) and to blood vessels for FDG (P < 0.0001) as well as AIB (P = 0.0038). Of 10 patients with suspected recurrence, 6 presented neither PET criteria for recurrence nor local failure in the specimens or during follow-up, while 4 cases with positive PET scans were ultimately diagnosed with local failure. CONCLUSION: Besides FDG, AIB and O15-water were shown to accumulate in primary and recurrent STS of different histologic type. A precise differentiation from normal muscle was found for all tracers. FDG and AIB differentiated viable tumor from blood vessels. PET using FDG, AIB and O15-water is suitable for functional imaging of STS and the detection of sarcoma recurrence.
