[Refraction and visual acuity after laser coagulation treatment for retinopathy of prematurity in stage 3+].

PURPOSE: Correlations between myopia < or =-4.0 D, birth weight, gestation age at birth, extension of laser photocoagulation and mild posterior pole changes were assessed in eyes of 1-year-old children who underwent laser treatment for stage 3+ retinopathy of prematurity (ROP). In addition the relationship between best-corrected visual acuity (VA) and mild posterior pole alterations was evaluated at 3 years of age. PATIENTS AND METHODS: Patients who underwent argon or diode laser photocoagulation between 1996 and 2000 for ROP 3+ disease were analyzed in this retrospective study. Cycloplegic refraction was determined in 72 eyes of 1-year old patients ( n=39). At 3 years of age cycloplegic refraction was measured in 36 eyes of 19 patients, and visual acuity was determined in 25 eyes of 13 children. RESULTS: Myopia of < or =-4.0 D significantly correlated only with mild posterior pole changes (i.e. clinically significant dragging of the temporal vessels of the retina or macular heterotopia, p=0.001). The correlation was significant between a VA reduction to <0.8 and mild posterior pole alterations ( p=0.014). CONCLUSIONS: The prevalence of myopia < or =-4,0 D and visual acuity <0.8 appears to be increased even with mild posterior pole changes. Birth weight does not seem to be significant factor in the prevalence of myopia < or =-4.0 D following laser coagulation of stage 3 threshold ROP.

[Adult Hirschsprung's disease with mental retardation and microcephaly].

Hirschsprung's disease occurs rarely and sporadically in adult, involving males. In cases, which are manifested perinatally, the so called Hirschsprung-associated congenital anomalies (mainly central nervous system, urogenital and cardiovascular) may present (2-21%), which have not observed in adult. Mental retardation and Hirschsprung's disease more frequently are associated with Down syndrome (5-10%). The discoveries of molecular genetics in the last 4-5-years through the examination of transgenic ("knockout") mice, proved the basic role the mutation of 4 genes: the RET (receptor tyrosin kinase), a proto-oncogene, coding its ligand, the glial cell-line derived neutrophic factor (GDNF), the gene of the endothelin-B receptor (ENDRB) and the gene one of its ligand, the endothelin-3 (EDN3), in the pathogenesis of Hirschsprung's disease. In our case, the short segment Hirschsprung's disease caused respiratory and cardiac failure, which was recognized by autopsy. Besides, the severe mental retardation, the role of the long term use of antipsychotic medicines comes up in the prolongation and masking of the symptoms. The accompanied mental retardation and microcephalia in early childhood are known, which are associated anomalies with Hirschsprung's disease. In cases of Hirschsprung diseases at adults, no other associated congenital anomalies has been published. The mental retardation in Down-syndrome, in association with Hirschsprung's disease (and presumable in our case, too) is supposed to be the consequence of the mutation in the gene of GDNF. In this case, we observed, that the so called short segment H-d was accompanied at a 33 years old men patient with mental retardation (who was originated from a gypsy ethnic minority), because of it the connection of the nurses and the patient was disturbed and the main symptom of the H-d (chronic obstipation) remained hidden. The mechanic ileus was going on behind the scenes, and in addition to the cardiac failure caused the death of the patient. Practical conclusion of the case is that, Hirschsprung's disease should be suspected in all adult patients, who had severe obstipation persisting since childhood, especially in males.

[Endoscopic thoracic sympathectomy].

Technique and advantages of thoracal sympathectomy, using the mediastinoscope, are described in this paper. Clear-cut evidence was produced to lasting suspension of sympathetic action during the follow-up period.

The possibilities of real-time sonography in the diagnostics of peripheral pulsating resistances. Our experiences of some cases.

Real-time sonography was performed in 5 patients in 7 cases for peripheral pulsating resistances. The change developed in the inguinal fold in one case after arteriography and in 3 patients following a vascular operation. In an additional case, a pulsating mass located in the neck proved to be the kinking of the subclavian artery. After a brief literary review of the ultrasound-diagnostics of the vessels, the authors have found sonography a suitable means for judging the relationship between the vessel or graft and the pseudoaneurysm, the size of the communication and eventual thrombus formation.

Data on the clinical aspects and therapy of primary gastrointestinal lymphomas.

Fifteen cases of primary gastrointestinal lymphoma diagnosed over 8 years are reviewed. In the period 1980 to 1982 there was a cumulative appearance of GI lymphomas, nine out of 15 cases were diagnosed in that period. According to its localization, lymphoma occurred in 12 cases in the stomach, and in 3 in the small intestines and the colon. One case of gastric lymphoma was Hodgkin type, the others were non-Hodgkin types. The clinical symptoms were not characteristic of lymphoma. The age of the patients was, on the average, ten years lower than the mean age of carcinoma patients. Preoperative diagnosis by gastric biopsy was successful in four cases. In patients with lymphoma of the colon not subjects to surgery, colonoscopy verified the origin of lymphoma. Exact clinical classification in the majority of cases was made intraoperatively. In the non-operated cases, sonography and lymphography were performed. In general, operation was attempted, but patients in stage II, in very poor condition, were possibly not operated. The possibility and indications of the "second look" operation are discussed. Histological typing was made according to the Kiel classification. In the literature, in the most controversial question of therapy, individual consideration of the cases is recommended. Based on our experience, in devising therapy or therapeutic strategy as well as concerning prognosis, the degree of malignancy according to histological type, clinical stage and anatomical localization seem to be the most decisive factors. In exceptionally malignant cases a protocol with doxorubicine + bleomycin + teniposide and prednisolone was applied.

[Michael Servetus' physiology in the light of his anthropology].

This essay starts with a short description of the life and works of Michael Servetus. Then the part of the innovator the Spanish physician played in describing the minor circulation is being examined alongside the contributions made by other contemporary physicians who tried to explain this physiological process. The ideas of the philosophical anthropology of Servetus are represented with special stress of the fact that beyond the theological frame, in his main opus Christianismi restitutio there are typical characteristics from the Renaissance (i.e. ethic relativism etc.). At the end it becomes clear that the description of the minor circulation is only to serve the purpose of explaining Servetus's theological thesis according to which the seat of the soul is in the blood. He stipulates that the "sensible soul" is to be found in the choroidal plexus of the ventricles of the brain or else in the pineal gland. It is one of the merits of Servetus, too, that he had foreseen the capillaries (capillares) which he describes as well in the case of the lung vessels as in the case of the choroidal plexus in the brain. The last part deals with the history and background of the new edition of Christianismi restitutio, published 1790 in Nuremberg, which again is based on some recent archival research.

Lymphatic cardiomyopathy: cardiac lymphostasis in the pathogenesis of arrhythmias.

Artificial lymphatic cardiomyopathy was produced in 30 dogs by ligating the regional lymph nodes and the efferent collecting lymph trunks of the heart. In the development of cardiac lymphostasis the appearance of arrhythmias and conduction disturbances was revealed by repeated ECG recordings. The arrhythmic condition of the heart increased and pacemaker function of the sinus node deteriorated in lymphatic cardiomyopathy. Cardiac lymphostasis could be detected during open heart surgical procedure and in long lasting tachycardiac attacks. It is probable that the cardiac lymphostasis has a role in the pathogenesis of some arrhythmias.

The effect of cardiac lymphostasis on the microcirculation of the heart: effect of cardiac lymphoedema on the development of an arteriovenous shunt circulation.

The effect of cardiac lymphostasis on the microcirculation of the heart was studied in 18 dogs. By ligation of the main lymph trunks and regional lymph nodes of the heart, cardiac lymphoedema--lymphogenic cardiomyopathy--was induced in 9 dogs, while 9 served as control animals. For microcirculatory investigations: 1) gelatin Indian ink injection, 2 benzidine reaction, 3) PVC injection corrosion preparation was applied. Characteristic changes were demonstrated in the microcirculation and capillary circulation of the heart in cardiac lymphostasis. In some capillaries-mainly where the interstitial oedema exists--the capillary circulation decreased: inhomogeneously vessel-free spots were formed in the heart. Around the vessel-free capillaries, elongated capillaries were found including very dilated pre- and post-capillary vessel sections. Arteriovenous shunts can be revealed in the heart in consequence of lymphoedema. The pathogenesis of the microcirculatory circulation changes caused by cardiac lymphostasis was discussed.

Hypertension secondary to bradycardia: blood pressure regulation under the effect of impaired cerebral blood flow and bradycardia.

Ninety-two patients of advanced age were studied in whom arterial hypertension was associated with chronic bradycardia, the cessation of bradycardia after pacemaker implantation was followed by a significant fall or permanent normalization of the high blood pressure. In three cases cerebral blood flow was found low, but it increased when the heart rate was normalized on pacing. In order to check the results obtained in humans, cerebral blood supply was restricted by carotids ligation, in some animals the vertebral artery of one side was also ligated. During obstruction of cerebral blood flow, bradycardia induced by av block or vagal stimulation was followed by an increase in arterial blood pressure. It is suggested that disturbances of cerebral blood flow associated with bradycardia have a decisive part in the pathogenesis of hypertension in old age.

Effects of amantadine on the estrous cycle in rats.

Amantadine, administered in single doses of 5-25 mg/kg s.c. to normal cycling rats, induced prolonged estrus, i.e. blocking of ovulation. This result supports the inhibitory role of the dopaminergic mechanisms in the LRF secretion. In larger doses, the drug led to persistent diestrus, too, probably by blocking the dopaminergic mechanisms involved in the PIF secretion. The observed effects agree with the partial dopaminergic agonist character of amantadine.
