Medullary carcinoma of the thyroid gland.

Nine cases of medullary carcinoma of the thyroid gland (MTC) are reported. Four of the carcinomas were of the familial type. Five of the patients were men and four were women. Patient age ranged from 23 to 66 years, with a mean age of 40 years. The median age of the four patients with the familial MTC was 32 years. A total or a subtotal thyroidectomy was performed in four and five patients, respectively, associated with a modified neck dissection in six patients with involved cervical lymph nodules. An underlying pheochromocytoma of the left adrenal was excised in one patient prior to thyroidectomy. In all cases the parathyroid glands were identified, and in two cases of familial MTC, in which they were grossly enlarged, the parathyroid glands were removed. Four patients died as a result of their disease within 3 years, whereas patients are well 4 to 12 years after surgery. The best chance of cure lies in early diagnosis and an aggressive surgical removal of the primary tumor and any cervical metastases.

Reconstruction of full-thickness defects of the cheek in cancer surgery.

Two techniques are presented that can be used for reconstruction of full-thickness defects of the cheek. Defects involving a small area of mucosa can be reconstructed with an anteriorly based neck-pectoral skin flap in combination with primary closure of the mucosa. Defects involving a large part of the mucosa can be managed with a deltopectoral flap in combination with a pectoralis major myocutaneous flap. The two techniques have been applied with satisfactory results in a group of patients with neoplasms of the cheek.

Effect of aprotinin on fibrinopurulent peritonitis in rats.

The effect of aprotinin on the clinical and pathologic course of experimentally induced peritonitis in the rat was studied. Peritonitis was induced in 40 rats by creating a closed ileal loop 4 cm long 5 cm from the ileocecal valve. The rats were divided into two groups of 20 rats each. Group 1 served as a control group, whereas each animal in Group 2 received a bolus dose of aprotinin (10 ml) intraperitoneally immediately after closing the laparotomy. In the aprotinin-treated group, survival was drastically increased (p less than 0.01) and formation of adhesions and abscesses was considerably reduced. The results of peritoneal cultures showed a decreased incidence of Escherichia coli and Clostridia in the aprotinin-treated group. We conclude that the administration of aprotinin significantly prolongs the survival time of animals with peritonitis and reduces the development of adhesions and abscesses in the peritoneal cavity. This beneficial effect can be attributed to decreased fibrinogen deposits within the peritoneal cavity and the stabilization of the organism after bacterial shock. Thus, bacteria were more susceptible to cellular and noncellular clearing mechanisms.

Effect of beta-adrenergic blockade on physiologic growth in the Wistar rat.

The effect of propranolol (Pr) on body weight was studied in 144 Wistar rats of both sexes from weaning to 17 weeks of age. Pr was administered to 77 of these rats (chosen at random), in a dose of 100 mg/kg body weight daily. Body weights were monitored every third day throughout the study. During their 17th week of age, all the rats were weighed for the last time and then sacrificed. The following organs were carefully dissected and weighed: brain, thymus, lungs, heart, liver, kidneys, adrenals, testes and ovaries. Gain in body weight was significantly reduced in the propranolol treated animals compared to control in both sexes. Organ weights were also less in propranolol-treated rats except for the kidneys of both sexes and the heart and testes of the males. However, the ratio of most organ weights per body weight was greater, indicating that the inhibition of weight gain was less severe in those organs compared to that of total body weight.

Surgical management of diseases of the adolescent female breast: a clinicopathologic study.

Sixty-three patients, ages 10 to 20 years with diseases of the breast were operated on during a 10 year period. A variety of pathologic entities were found. The most common were diagnoses fibroadenoma, cystic disease, and cellulitis with abscess formation. Malignancy was found in three patients: lobular carcinoma in situ in one, angiosarcoma in the second, and lymphosarcoma in the third.

Beneficial effect of omental wrapping of unsafe intestinal anastomoses. An experimental study in dogs.

The effect of omental wrapping on the safety of intestinal anastomosis was studied experimentally in 12 dogs. In six animals serving as controls, complete division of the small intestine and immediate restoration of continuity was carried out, after rendering the intestinal segment ischemic by ligating the mesenteric vessels for a distance of 10 cm. In a second group of animals, the ischemic anastomosis was wrapped with a pedicled strip of omentum. Gangrene and peritonitis developed in all animals of the control group, whereas the omental graft afforded complete protection against leakage from the devascularized anastomosis and peritonitis in the test animals.

Prediction of malignancy in solitary thyroid nodules in a country with endemic goiter.

From January 1962 through October 1975, 455 patients with single thyroid nodules were operated on at King Paul Hospital. Malignancy was proved in forty-three patients. The overall incidence of carcinoma was 9.5 per cent. A higher incidence of cancer was found in patients less than ten years of age (40 per cent), between eleven and twenty years of age (20 per cent), and more than sixty-one years of age (17.4 per cent). Malignant nodules were more frequent in males (17.5 per cent) than in females (8.3 per cent). Radioactive iodine scanning does not distinguish benign from malignant nodule. Solitary thyroid nodules require operative excision supplemented with replacement therapy.

Preliminary evaluation of the potential prognostic value of serum levels of immunoglobulins (IgA, IgM, IgG, IgE) in patients with gastric cancer.

Sixty patients with advanced gastric carcinoma who refused to receive cytotoxic chemotherapy were examined for serum immunoglobulin levels (IgG, IgM, IgA, IgE). Three samples were obtained every two months thereafter. The group of patients who had above-normal values of one or more of the examined immunoglobulins had a longer survival than the other (p < 0.024). Immunoglobulin values were independent of the Helicobacter pylori antibody titer and of acute phase reactants. It is concluded that survival potentially correlates with serum immunoglobulin levels. Further studies including larger numbers of patients and correlating serum immunoglobulin levels with specific clinical parameters are needed to establish the prognostic role of serum immunoglobulins in patients with gastric carcinoma.

The influence of hemodilution on left ventricular function.

In nine anesthetized mongrel dogs anemia was produced by exchanging blood with plasma substitute thus reducing hemoglobin gradually in three steps. Aortic, atrial and ventricular blood pressures, cardiac output, electrocardiogram, phonocardiogram and the first derivative of the left ventricular pressure were continuously monitored. Blood samples were taken to determine hemoglobin, blood gases and whole blood viscosity. Progressive hemodilution resulted in a significant increase in cardiac output and left ventricular stroke work, while total peripheral resistance, oxygen content and whole blood viscosity decreased significantly. There were no significant changes in cardiac pressures, myocardial contractility, diastolic pressure time index and blood gases. The oxygen supply/demand ratio had gradually declined, while electrocardiogram showed no significant changes. These results suggest that moderate isovolemic hemodilution in animals with normal coronary vessels does not impair left ventricular function as this was manifested by the unchanged hemodynamic and electrocardiographic findings.
