[Endobronchial neurogenic tumors].

Endobronchial neurogenic tumours are exceptional and little known. The authors report two cases of such tumours: a neurofibroma of the right main bronchus in a 32 year old man and a schwannoma of the left main bronchus in a 10 year old child. Pneumonectomy was necessary in both cases due to destruction of the pulmonary parenchyma distal to the obstruction. The incidence of this type of tumour is estimated to be between 0.2 and 4%. They may be either schwannomas or neurofibromas, the discovery of which always raises the problem of the possible association of von Recklinghausen's disease malignant forms, neurogenic sarcomas or malignant schwannomas may also be encountered. The diagnosis is based on endoscopy which reveals the tumour and allows biopsy. Treatment of these essentially benign tumours should be conservative, provided the diagnosis is made early, prior to parenchymal destruction. The prognosis is poor in the malignant forms and chemotherapy may be useful in malignant schwannomas.

[Place of thoracotomy in thoracic complications of hydatid cysts of the liver].

The authors present 16 cases of hydatid cyst of the liver with rupture into the thorax, operated by thoracotomy. In 15 cases, the cyst ruptured into the right hemithorax and in one case, it ruptured into the left hemithorax. We adopted the classification proposed by our master S. Mestiri, which completes that proposed by Dévé. The diagnosis is essentially based on thoraco-abdominal ultrasonography, which visualises the hydatid cyst, reveals intrathoracic collections, demonstrates the diaphragmatic tear and studies the biliary tract looking for obstruction or dilatation. The diagnosis was established in 12 out of 16 cases. There were 3 postoperative deaths (18.7%): two from septic shock and one from cerebral vein thrombosis. Only one patient had to be operated via an abdominal incision one month after thoracotomy for cholangitis secondary to stones in the common bile duct which were not seen on the preoperative ultrasonography. Laparotomy is required whenever there is a biliary problem and may be sufficient in type 1 lesions. Thoracotomy is necessary whenever there is an intrathoracic collection (types II, III, IV) and is sufficient in the absence of any biliary tract lesions.

[Extralobar pulmonary sequestration. A diagnostic pitfall].

The authors report a case of dual malformation: pulmonary (sequestration) and mediastinal (pericardial). This case is original in that clinically and at standard radiography sequestration was mistaken for hydatid cyst. The diagnosis was corrected after anatomico-pathologicale examination of the operative specimen.

[Small intestine occlusion caused by the silent opening of liver hydatid cysts into the peritoneum. Apropos of an uncommon case].

The authors report one exceptional case of organic small bowel occlusion. The etiological diagnosis was performed by ultrasonographic examination and showed two hydatid cysts of the liver opened into the peritoneum and a peritoneal fluid collection corresponding to the hydatid fluid. The authors emphasize the absence of clinical symptoms during the opening of the hydatid cysts of the liver into the peritoneum and the usefulness of emergency ultrasonographic examination of the entire abdomen in cases of small bowel obstruction without any evident etiology.

[Malignant para-testicular tumors. Apropos of 3 unusual cases].

Three uncommon cases of malignant para-testicular tumors are described. The first one was a malignant mesothelioma discovered after cervical node biopsy, the second, a high grade lipoblastic liposarcoma and the third a malignant mesenchymoma with 4 components. The literature is reviewed with special regard to these 3 types.

Results of a conservative treatment combining induction (neoadjuvant) and consolidation chemotherapy, hormonotherapy, and external and interstitial irradiation in 98 patients with locally advanced breast cancer (IIIA-IIIB).

Ninety-eight patients with locally advanced breast cancer (Stage IIIA-IIIB) were entered into a pilot study combining intensive induction (neoadjuvant) chemotherapy (VTMFAP) with or without hormonochemotherapy, external and interstitial radiotherapy, and consolidation chemotherapy with or without hormonochemotherapy. Tumor regression over 50% was observed in 91% patients after chemotherapy, and complete clinical remission occurred in 100% patients after irradiation. The rate of local relapse is 13%. The 3-year disease-free survival is 62% and 3-year global survival is 77%. Initial chemotherapeutic tumor regression greater than 75% is the main predictive factor for disease-free survival.

Antineoplastic Combined Chemotherapy Protocols

Phase I clinical study of the new amino acid-linked nitrosourea, S 10036, administered on a weekly schedule.

Diethyl-1-[3-(2-chloroethyl)-3-nitrosoureido]ethylphosphonate (S 10036) is a new nitrosourea that has been evaluated in a clinical trial because of its activity in the National Cancer Institute panel screen and its rational chemical approach. A Phase I study was conducted in 22 evaluable patients with advanced cancers. The drug was given as a slow i.v. infusion over a period of 60 min on days 1, 8, 15, and 22 followed by a 4-week rest period. The dose levels ranged from 25 to 200 mg/m2/week for 4 consecutive weeks using a modified Fibonacci scheme. Thrombocytopenia was the only acute dose-limiting toxicity and started at a dose of 100 mg/m2/week and above. Hematological toxicity was delayed, cumulative, and dose related. Nausea and vomiting were moderate to severe and dose related. Three responses (one complete and two partials) have been noted. Phase II studies of S 10036 are planned at a dose of 100 mg/m2/week for 4 consecutive weeks ("induction therapy") for patients without prior therapy and 100 mg/m2/week for 3 consecutive weeks for those with prior chemotherapy or radiotherapy. Because of the cumulative toxicity, the recommended dose for the second cycle of S 10036 chemotherapy ("maintenance therapy") is 100 mg/m2/week every 3 weeks.

Nucleotide sequence of the gene coding for yeast cytoplasmic aspartyl-tRNA synthetase (APS); mapping of the 5' and 3' termini of AspRS mRNA.

A 3.8 Kb DNA fragment, which contains the structural gene of aspartyl-tRNA synthetase (AspRS) and its flanking regions, has been fully sequenced by the combined M13/dideoxy chain terminator method. From the single open reading frame of correct length (1671 bp) we deduced an amino acid sequence consistent with that of several peptides of AspRS. No significant internal sequence repeats were observed in the primary structure of the protein. The AspRS gene (APS) has a codon usage pattern typical of non abundant proteins. S1 nuclease analysis of APS mRNA showed a major start 17 bases downstream from a "TATA box" and stops near an RNA polymerase terminator sequence.

Amino Acid Sequence

Adjuvant chemotherapy in ocular malignant melanoma. Study of 20 cases.

The frequency of hematogenously propagated hepatic metastases occurring from ocular melanoma led us to treat 20 patients with adjuvant chemotherapy, 19 patients starting chemotherapy during the month following enucleation and 1 patient, a year after enucleation. With a median follow-up of 6 years, 17 patients (80%) are disease-free. Three patients developed hepatic metastases at 24, 24 and 30 months, respectively. The results suggest that adjuvant chemotherapy is effective in preventing metastases from ocular melanoma.
