[Cancer chemotherapy for lymph nodal metastases using activated carbon particles adsorbing methotrexate].

A new dosage format (MTX-CH) of methotrexate (MTX) comprises MTX adsorbed onto fine activated carbon particles which has strong affinity to lymphatics. MTX-CH was tested for its therapeutic effects on lymph node metastases in mice experimental model. It is known that metastases have been established in the regional lymph nodes of all mice 7 days after subcutaneous inoculation with P388 leukemia cells at 5 x 10(5) cells/mouse in left hind foot pad, and that assay mice survival time becomes short in proportion to log [P388 cell number inoculated intraperitoneally]. Therefore, drug treatment was carried out on day 7, and the regional nodes were extirpated and transferred intraperitoneally to assay mice on day 10. The survival time of mice to whom transferred were the nodes of mice treated with MTX-CH was significantly longer compared with those treated with MTX aqueous solution.

Cellular subsets of the milky spots in the human greater omentum.

The cellular composition of the human milky spots was investigated on surgically removed specimens of the greater omentum of three 8-month-old infants operated on for neuroblastoma. Monoclonal antibodies and immunohistochemical methods for recognition of macrophages. B-lymphocytes and T-lymphocytes and toluidine-blue staining for mast cells were used. The mean number of cells in one milky spot amounted to 570 +/- 33. This cell population was composed of 47.5% macrophages, 29.1% B-lymphocytes, 11.7% T-lymphocytes and 6.1% mast cells. Since inflammation was absent in the material investigated, the numerical data found in the present paper could be regarded as representative cell levels of normal milky spots.

Enhanced anti-cancer efficacy on lymph node metastasis using peplomycin adsorbed on small activated carbon particles.

A new dosage form (PEP-CH) of peplomycin was tested for therapeutic efficacy against lymph node metastasis in mice. PEP-CH is a suspension comprising 4 mg/ml of activated carbon, 2 mg/ml of peplomycin and 1.6 mg/ml of polyvinylpyrrolidone in saline. Mice were subcutaneously inoculated with 3 x 10(5) MH134 tumor cells into the left hind paw. Drugs were given on day 10 when cancer had been metastasized in the left popliteal lymph node. Mice were killed on day 17 and the left popliteal lymph node and the left deep inguinal lymph node were extirpated. Since the degree of the metastatic lesion and the lymph node weight correlated with a statistically high probability with each other, the degree of metastatic lesion was evaluated through comparison of lymph node weight. The left popliteal lymph node and the deep inguinal lymph node were 10.5 mg and 4.5 mg in average weight, respectively, in the mice given PEP-CH containing 0.1 mg of peplomycin subcutaneously into the left hind foot-pad. The weights were significantly smaller than those in the mice given an identical dose of peplomycin aqueous solution subcutaneously into the left hind foot-pad or intraperitoneally.

Intraoperative cleansing of the impacted colon using an endotracheal tube.

A simple technique for intraoperative cleansing of the impacted colon is described. Saline-irrigation through the torn balloon of the endotracheal tube (38 French) and massaging the impacted feces facilitated drainage of the obstructed colon. This technique was done in two emergent colon operations and immediate anastomosis could be performed safely. An endotracheal tube is always available in an operating room, so this technique is useful and effective in an emergent colon operation without special preparation.

[Toxicity of cis-platinum loaded with lactic acid oligomer microspheres in mice].

We have evaluated the acute toxicity of an experimental new dosage form administered intraperitoneally in mice, consisting of cis-platinum loaded with lactic acid oligomer microspheres (CDDP-ms). The LD50 value of the CDDP-ms was 23.8 mg/kg, which amounted to 176% of the LD50 of the cis-platinum solution (13.5 mg/kg). Autopsy findings revealed no additional toxicity due to this dosage form.

Milky spots in the human greater omentum. Macroscopic and histological identification.

Macroscopic and histological investigations were made from surgical specimens demonstrating milky spots in the human greater omentum from subjects of various ages. The milky spots in the human greater omentum appear as tiny, cotton-wool-like masses to the naked eye. Histologically, the milky spots consisted mainly of many macrophages with diffuse cytoplasmic esterase reaction products and esterase-negative B lymphocytes surrounding the vascular networks. Macrophages phagocytosed many carbon particles which were introduced as a carbon suspension during the operation. The vascular networks were blood capillary convolutions with a glomerular shape. Silver impregnation showed the delicate networks of reticular fibers which constitute the framework of the organ. The number of milky spots was highest in infancy and gradually decreased with age.

Scirrhous carcinoma cell invasion into the stomach wall detected by monoclonal antibody S202: a comparison between immunoperoxidase and hematoxylin-eosin stain.

We used anti-scirrhous carcinoma monoclonal antibody (MAb) S202, which reacted strongly with scirrhous gastric cancer, to evaluate the depth of cancer cell invasion into the stomach wall. Sixty-eight scirrhous gastric cancer specimens were stained with the avidin-biotin peroxidase complex method using MAb S202. There were some discrepancies between the histological evaluation of the immunoperoxidase (IP) and hematoxylin-eosin (HE) staining regarding the depth of tumor invasion. In two of the 14 HE staining cases in which carcinoma had extended as far as the proper muscular layer, IP staining revealed that the lesions had actually penetrated this layer. As well, in two of the 18 HE staining cases in which carcinoma cells had not penetrated as far as the serosa, IP staining revealed that they had, in fact, reached the serosal surface. The invasive carcinoma cells were more easily discerned using IP staining with MAb S202 rather than using histological staining with HE. Thus, MAb S202 may be useful for precise identification of carcinoma cells.

[Intraoperative chemotherapy with intraperitoneal activated carbon particles adsorbing mitomycin C against peritoneal dissemination of gastric cancer].

A new form of dosage (MMC-CH) was composed of activated carbon particles adsorbing mitomycin C. Intraperitoneal administration of MMC-CH was tested clinically for prophylactic and therapeutic effects on peritoneal carcinomatosis of gastric cancer. The criteria of MMC-CH's administration were equal or less than 70 years old, more than 40 kg in body weight, no disfunction of liver and kidney, no particular findings in electrocardiography, S2 or S3 in the grade of serosal invasion, P0, P1, P2 or P3 in the grade of peritoneal dissemination, according to the General Rules for the Gastric Cancer Study in Surgery and Pathology by the Japanese Research Society for Gastric Cancer. MMC-CH was given to 44 patients undergoing gastrectomy for gastric cancer in our department from 1985 to 1988. The 44 patients were composed of 12 patients with P0 findings (P0 patients), 8 patients with P1 findings (P1 patients), 12 patients with P2 findings (P2 patients), and 12 patients with P3 findings (P3 patients). MMC-CH at 50 mg/person in terms of mitomycin C was administered intraperitoneally before the operation wound was closed. Fifty-seven patients in our department from 1983 to 1987 for whom the same criteria were applicable and did not receive MMC-CH therapy, served as the control group. The 57 patients were composed of 23 P0 patients, 21 P1 patients, 10 P2 patients, and 3 P3 patients. There was statistically with chi 2 test no significant difference of age, sex, depth of infiltration macroscopically and microscopically defined progression of lymph-nodal metastases between the MMC-CH group and the control group. Survival rate was calculated with Kaplan-Meier's method in the overall patients in each of the MMC-CH group or the control group. The overall survival rate in the MMC-CH group was statistically significantly (p less than 0.01-0.05) higher from day 460 to day 552 and from day 736 to day 800 than that in the control group. Next, the patients were classified into two subgroups, namely the subgroup composed of P0 patients and the subgroup composed of P1, P2, and P3 patients, in order to examine each of the MMC-CH's prophylactic effects on subsequent dissemination or its therapeutic effects on established dissemination. Survival rate was calculated with Kaplan-Meier's method in the two subgroups through the same procedures used for the overall survival rate.(ABSTRACT TRUNCATED AT 400 WORDS)

Localization of antigen defined by anti-scirrhous gastric carcinoma monoclonal antibody S202 in fixed human cancer tissues.

Monoclonal antibody S202 reactive with scirrhous gastric carcinoma cell line MK-01 was used in an immunoperoxidase assay to characterize the antigenic profile of a variety of fixed samples. Representative sections of malignant tissue from the stomach, colon, esophagus, pancreas, bile duct, gallbladder and breast were examined by the avidin-biotin complex immunoperoxidase technique and compared. The antibody reacted with 90 out of 98 stomach adenocarcinomas, and with 30 out of 32 colon adenocarcinomas with diffuse cytoplasmic staining, whereas it reacted with only 19 out of 37 breast carcinomas, and 5 out of 10 esophageal squamous cell carcinomas. The antigenic determinants appeared to be a carbohydrate, since it was highly sensitive to sodium periodate.

[Microscopic changes in the human liver after intra-arterial infusion of anticancer agents suspended in lipiodol].

Microscopic injury to the human liver after an intraarterial infusion of anti-cancer agents suspended in lipiodol has been evidenced by Hematoxylin-Eosin staining and by Oil Red O staining. In cases involving primary hepatic tumors, the liver tumor tissue has been injured severely while normal tissue showed no injury. In cases involving metastatic liver tumors, the tumor tissue, and also the normal liver tissue was injured though the latter injury was only mild. However, when multi-anticancer agents that were suspended in lipiodol were infused intra-arterially, the normal liver tissue was injured, even from a single drug suspension.

[Cytofluorometric and histopathological studies on non-cancerous liver lesions after lipiodol-transcatheter arterial embolization].

We have investigated the effects of preoperative Lipiodol-Transcatheter arterial embolization (Lp-TAE) on both the nuclear DNA content of noncancerous liver cells by DNA-cytofluorometry (using NIKON SPM-RF1-D), and the histopathological findings of liver lesions. Lp-TAE through the hepatic artery was performed on ten primary hepatocellular carcinoma patients using Gelfoam in combination with adriamycin (40 mg) or mitomycin C (20 mg), emulsified in Lipiodol (10 ml) and 60% Urografin (2 ml). And hepatic resection was carried out after one or two months later in each case. Histologically, primary hepatocellular carcinoma lesions showed various patterns of necrosis, while the non-cancerous liver cells did not show hepatocellular infarction in the liver cirrhosis. The results of the nuclear DNA content showed an increase in the fractions of poliploid cells in the non-cancerous lesions treated with Lp-TAE compared with that in the normal age-matched controls. But we found no remarkable differences in the ploidy patterns between Lp-TAE treated patients and untreated patients with liver cirrhosis. In conclusion, preoperative Lp-TAE does not induce any remarkable histological hepatocellular damage of non-cancerous lesion and does not affect nuclear DNA content of non-cancerous liver cells.

[Trans-arterial embolization or transarterial infusion of anti-cancer drugs suspended in a lipid contrast medium against unresectable metastatic liver cancer].

We had 84 patients with unresectable metastatic liver cancer from May, 1980 to December 1987 in the National Matsudo Hospital. Eighteen of them had no treatment; 26 of them had serial transarterial embolization (TAE) with or without infusion of anti-cancer drug dispersed in lipiodol; 7 of them had only serial trans-arterial infusion (L-TAI) of an anticancer drug suspended in lipiodol. A significant survival advantage of patients with TAE or L-TAI was noted when compared with no treatment, but there was no difference between the survival rate of patients with TAE and L-TAI. There were some 82 patients with unresectable metastatic liver cancer from October 1984 to March 1988 in Kyto Prefectural University of Medicine Hospital. Thirteen of them had L-TAI using one anti-cancer drug (one-drug group); 8 of them had L-TAI using two drugs (two-drug group); 22 of them had L-TAI with three drugs (three-drug group). The survival rate of the three-drug group was superior to the one- and two-drug groups. It was concluded that TAE was not necessary for metastatic liver cancer, but L-TAI must be undertaken, because combined cancer chemotherapy enhanced the effect of L-TAI.
