Endothelium-dependent regulation of coronary tone in the neonatal pig.

We examined the effects of endothelium-dependent responses on coronary perfusion pressure (CPP) in isolated, blood-perfused neonatal pig hearts under conditions of controlled coronary flow. Baseline CPP was increased 8%-21% by the cyclooxygenase inhibitor indomethacin (10-100 microM), and 30%-92% by NG-monomethyl-L-arginine (L-NMMA, 10-100 microM), an inhibitor of nitric oxide (NO) synthase, suggesting that both prostaglandin and nitric oxide synthesis contribute to basal coronary tone. Both acetylcholine (ACh) and bradykinin (BK) decreased CPP. These effects were enhanced by preconstriction with endothelin-1. L-NMMA markedly attenuated BK-induced coronary vasodilation and converted the ACh response to constriction, indicating a significant role for NO release in these responses. After 1 h of total, global normothermic ischemia and 45 min of reperfusion, vasoconstrictor responses to endothelin-1 and ACh were enhanced, while BK-induced dilation was significantly reduced. L-Arginine supplementation during reperfusion did not restore vasodilatory responses to ACh or BK. The magnitude of L-NMMA-induced coronary vasoconstriction during reperfusion was similar to that observed without ischemia-reperfusion. Coronary vasodilation in response to sodium nitroprusside, a NO precursor that causes endothelium-independent vasodilation by directly activating smooth muscle guanylate cyclase, was unaffected by ischemia-reperfusion. We conclude that NO production in the neonatal coronary circulation contributes to both basal tone and the response to ACh and BK. After ischemia-reperfusion, basal NO production and smooth muscle relaxation mediated by guanylate cyclase are intact, whereas agonist-stimulated dilation is significantly impaired.

[Prophylaxis with ciprofloxacin in transurethral surgery].

The purpose of the study was to compare in a prospective randomized study the effect of three doses of prophylactic administration of ciprofloxacin in 25 patients and a control group of 26 patients without prophylactic antibiotic treatment in conjunction with transurethral surgery. In the group of patients with prophylaxis there is a significantly lower incidence of clinical manifestations of infection, as compared with the group of patients without prophylaxis which reduces the necessity to administer antibacterial drugs during the postoperative period.

[Functional evaluation of the penile venous system using dynamic pharmaco-cavernosometry and cavernosography].

The penile venous system plays an important role in the haemodynamics of erection. Failure of the venous occlusive mechanism leads to venous incompetence and impotence. The authors have examined the functional capacity of the penile venous system by dynamic pharmacocavernosometry in 31 patients with erectile dysfunction. This diagnostic tool enables to evaluate quantitatively the extent of the venous incompetence. In 26 patients it was possible to localize cavernosographically the site of venous leakage through one of the three venous systems draining the penis. Dynamic cavernosometry and cavernosography together with systematic knowledge of the anatomy of the penile venous system are of primary importance for correct diagnosis and treatment of the erectile dysfunction caused by venous incompetence. (Tab. 2, Fig. 7, Ref. 27.)

[Revascularization of the penis].

Revascularization of the penis is indicated in those patients in whom peripheral arterial occlusion of the internal pudendal artery alone or in combination with the venous incompetence of the penis, was detected to be the cause of erectile dysfunction. The authors present the results of penile revascularization in 12 patients impotent after crushing pelvic fracture associated with urethral rupture. Revascularization of the dorsal artery of the penis by the inferior epigastric artery was used as the method of surgical treatment. Satisfactory functional results were achieved in 58% of patients. In other 8 patients with combined arteriogenic and venogenic impotence, arterialization of the venous system of the penis was utilized. In 6 patients the method of "triple" anastomosis of the inferior epigastric artery, dorsal artery of the penis, and penile deep dorsal vein was used. In 2 cases the deep dorsal vein of the penis was arterialized by the inferior epigastric artery. The cumulative success rate of venous arterialization of the penis was 62.5%. (Fig. 5, Ref. 15.)

[Prevention of hematoma and edema after scrotal surgery].

Postoperative hematoma and edema are the most frequent complications of scrotal surgery. The authors refer their own experience with a simple, reliable method for prevention of postoperative hematoma and edema described by Oesterling.
