Obstructive uropathy. An unusual presentation of primary linitis plastica of the colon.

A patient with primary linitis plastica of the colon has been presented. The unusually aggressive nature of this tumor is demonstrated in a young man who presented with obstructive uropathy. Extensive renal involvement is a rare complication of this disease, and obstructive uropathy is a previously undescribed presentation. If early recognition is to favorably alter prognosis, then awareness of this form of colonic malignancy is essential.

Paramagnetic metalloporphyrins as potential contrast agents in NMR imaging.

Several water-soluble paramagnetic metalloporphyrins were found to increase significantly the relaxation rate (1/T1) of water. Such compounds are known to be selectively retained by tumors in animals, and consequently we identify them as potential contrast agents for tumors in NMR imaging.

Chemical and in vivo studies of the anion oxo[N,N'-ethylenebis(2-mercaptoacetimido)]Technetate(V).

Studies of the anionic coordination complex 99Tc-oxo[N,N'-ethylene-bis(2-mercaptoacetimido)]technetate(V) ([TcO(ema)]-) are described. Syntheses performed both at carrier levels (10(-5)M) and with no carrier added (less than 10(-8)M) indicate that the complex is formed virtually quantitatively from pertechnetate ion over this range. Tissue distributions in normal rats are similar at both concentrations up to one hour after administration. It has been shown--using a combination of high-pressure liquid chromatography and field-desorption mass spectrometry--that the anion is excreted unchanged into both urine and bile. The effectiveness of this N2S2 donor set in sequestering Tc-99m, and the in vivo stability of the resulting complex, suggest that modified chelates of this structural class could provide a series of useful diagnostic agents.

Gadolinium-enhanced T1-weighted versus T2-weighted imaging of scrotal disorders: is there an indication for MR imaging?

To evaluate the use of gadopentetate dimeglumine in magnetic resonance (MR) imaging of scrotal disorders, the clinical, ultrasound, and MR imaging data of 29 patients (age range, 19-75 years) with various intra- and extratesticular disorders were retrospectively analyzed. T1- and T2-weighted spin-echo images (T1-T2 group) were compared with T1-weighted spin-echo images before and after intravenous administration of gadopentetate dimeglumine (T1-Gd group). A receiver operating characteristic (ROC) analysis of the findings was undertaken. Better contrast between tumor and parenchyma and a clearer demonstration of the tunica albuginea were noted in the T1-T2 group (although not of diagnostic relevance). ROC analysis revealed no differences between the two imaging groups in the diagnosis of tumor, trauma, hydrocele, or hemorrhage; however, epididymitis was diagnosed more easily with contrast enhancement (0.8834 vs 0.7759, P = .04) and the diagnosis of orchitis was expressed more strongly (0.8221 vs 0.7184, P = .17). Four of the five observers were more confident in making the diagnosis with contrast enhancement. With MR imaging, the diagnosis was correctly suggested in three patients in whom clinical and ultrasound data were inconclusive. Gadolinium-enhanced MR imaging gives additional information in scrotal disorders and facilitates diagnosis. It may be helpful when findings at physical examination and ultrasound differ and when plain T1- and T2-weighted images are equivocal.

Hydrodynamic thrombectomy of hemodialysis fistulas: first clinical results.

PURPOSE: A hydrodynamic thrombectomy system was used for the treatment of recent dialysis shunt thrombosis. PATIENTS AND METHODS: Sixteen shunt thromboses in 14 patients were included in the study. There were seven polytetrafluoroethylene grafts and nine native arteriovenous fistulas. Occlusion time ranged from 6 to 48 hours, and thrombus length ranged from 4 to 40 cm. RESULTS: Thrombectomy was technically successful in 15 of 16 instances. No significant residual thrombus was found in 15 cases. In one case, half of the thrombus remained in the vessel and the procedure failed technically. One embolus to the radial artery occurred after balloon dilation following hydrodynamic thrombectomy and was removed percutaneously. Early rethrombosis within 24 hours occurred in five shunts; four more rethrombosed within 2 weeks to 3 months. Eleven shunts were available for follow-up. Cumulative patency was 41% after 6 months. CONCLUSION: Hydrodynamic thrombectomy is a promising concept for declotting of both hemodialysis grafts and native shunts and may offer an alternative to thrombolysis and surgical thrombectomy.

Staging of urinary bladder neoplasms with MR imaging: is Gd-DTPA helpful?

This study was performed to investigate whether intravenous administration of Gd-DTPA can improve the accuracy of MR imaging in the detection and staging of bladder neoplasms. In 68 patients with suspected urinary bladder neoplasms, MR examinations were performed with T1-weighted SE sequences before and after intravenous administration of Gd-DTPA. The findings were compared with surgical staging using the TNM classification. Overall staging accuracy of contrast enhanced MR was 46%; if stages Ta-T3a were combined into one group, the accuracy was 69%. Accuracy was low (19%) in tumors without muscular bladder wall invasion (Ta). In cases with extravesical spread (greater than or equal to T3b), the accuracy of staging was 87%. Contrast enhanced MR detected extravesical extension of tumor with a sensitivity of 93% and a specificity of 95%. Contrast enhancement increased the sensitivity for detection of urinary bladder neoplasms from 70% on precontrast T1-weighted scans to 79% on postcontrast scans. In comparison with T2-weighted scans, the Gd-DTPA enhanced T1-weighted scans had better image quality and lower acquisition times.
