Recurrent intracaval renal cell carcinoma: the role of intravascular ultrasonography.

The presence of extension into the vena cava does not preclude curative resection for extensive renal cell carcinomas. However, preoperative assessment of (1) the proximal extent of the tumor and (2) the degree of adherence within the vena cava is necessary to plan operative strategies. The following report describes the successful use of intravascular ultrasonography in the preoperative evaluation of a patient with recurrent renal cell carcinoma with vena caval extension.

Radiation therapy of ureteral metastases from breast cancer.

A series of 6 patients with ureteral metastases from breast carcinoma treated by radiation therapy has been evaluated. Most patients received a tumor dose of 3,600 rads in 3 to 4 weeks, and showed remarkable clinical improvement, with relief of the ureteral obstruction and reversal of symptoms and signs of uremia after radiation therapy. One patient died before an adequate dose of radiation therapy could be given. Early recognition and early treatment of ureteral metastases have been stressed. Radiation therapy techniques have been described.

Combined radioimmunoassay of four steroids in one ml of plasma: I. Progestins.

Using partially specific antisera combined with a 1 step celite microcolumn chromatography, progesterone (P), 20alpha-hydroxypregn-4-ene-3-one (20alpha-P), 17-hydroxyprogesterone (17-P), and 16alpha-hydroxyprogesterone (16alpha-P) could be measured in the same 1 ml aliquot of plasma. The chromatographic step removed known interfering steroids and conferred specificity to the assay. After correction for recovery the sensitivities, expressed as ng/ml of plasma, were respectively: 0.04 for P, 0.03 for 20alpha-P, 0.02 for 17P, and 0.01 for 16alpha-P. Recovery experiments, using steroid-free plasma to which various amounts of each steroid were added and then measured in the assay in 12 replicates, confirmed adequate accuracy and precision. The ability to measure multiple progestogens in small volumes of plasma should permit comprehensive evaluation of the role of these steroids in health and disease.

20-alpha-Dihydroprogesterone

Using partially specific antisera combined with a 1 step celite microcolumn chromatography, androstenedione (A), 5alpha-dihydrotestosterone (DHT), testosterone (T), and androst-5-ene-3beta, 17beta-diol (delta 5-diol) could be measured in the same 1 ml aliquot of plasma. After correction for recovery the sensitivities, expressed as ng/ml of plasma, were respectively: 0.025 for A, 0.05 for DHT, 0.025 for T, and 0.1 for delta5-diol. Recovery experiments, using steroid-free plasma to which various amounts of each steroid were added and then measured in the assay in 12 replicates, confirmed adequate accuracy and precision. The ability to measure multiple androgens in small volumes of plasma should permit comprehensive evaluation of the role of these steroids in health and disease.

A prospective study of the effects of ultralow volume (ULV) aerial application of malathion on epidemic Plasmodium falciparum malaria. II. Entomologic and operational aspects.

In a large-scale study in the Miragoane Valley of Haiti, designed to test the effects of aerial ultralow volume (ULV) malathion on epidemic Plasmodium falciparum malaria, spray operations resulted in an immediate and sharp decline in numbers of the vector, Anopheles albimanus. The adult population of this mosquito remained at less than 1% of previous levels until several weeks after a 50-day spray period (27 October-16 December 1972) during which six cycles were completed. The study area offered ideal conditions of wind, temperature, humidity, and mountain barriers. Mosquitoes in the area were highly susceptible to malathion. Results indicated that aerial ULV treatment with malathion can reduce A. albimanus populations rapidly and effectively when applications are made over an area as large as 20,000 acres. Preliminary results showed that effective control was not achieved in areas one-quarter that size; these areas were not sufficiently large, and infiltration of mosquitoes from adjacent untreated areas was possible.

Consumers of oral contraceptives in a social marketing program in Honduras.

This paper presents data on the characteristics of oral contraceptive users in Honduras, obtained from a point-of-purchase survey conducted in pharmacies in 1986. The oral contraceptive Perla, provided by the Social Marketing Program, accounted for 42 percent of sales. Forty-five percent of those using Perla were considered "new" users; either they were purchasing contraceptives for the first time or had used only Perla. Almost half of those who switched to Perla previously had obtained their orals in the noncommercial sector. Brand substitution occurred among users of commercial brands at about the same rate as among Perla users (51 percent versus 55 percent). Most women switching contraceptives did so within two months of their last purchase.
