Oligonucleotide typing reveals association of type I psoriasis with the HLA-DRB1*0701/2, -DQA1*0201, -DQB1*0303 extended haplotype.

Although the pathogenesis of psoriasis is still a matter of debate, there are several lines of evidence supporting the concept of this disease being immunologically mediated with T cells playing a crucial role. Because a considerable portion of the cellular infiltrate in psoriasis consists of activated T-helper cells, expression of HLA class II antigens might be of particular importance for the understanding of its pathogenesis. Therefore, we investigated the HLA type of patients with type I (early onset, positive family history) and type II (late onset, no family history) psoriasis by means of serology (n = 89) and genotyping using sequence-specific oligonucleotide probes (n = 64). Serologic analysis of class I documented the association of type I psoriasis with HLA-Cw6, -B13, and -B57, whereas type II psoriasis showed a weaker correlation with HLA-Cw2 and -B27. Genotyping using SSO for class II detected the elevation of the HLA-DRB1*0701/2 allele frequency from 13% in normal population to 36% in type I, but only to 15% in type II psoriatics. Moreover, positive correlations with type I psoriasis were detected for HLA-DQA1*0201 and HLA-DQB1*0303. The HLA-DRB1*0701/2, -DQA1*0201, -DQB1*0303 extended haplotype was found exclusively in type I psoriasis. This is the first report documenting the association of distinct HLA class II alleles with type I psoriasis as detected on the DNA level, an approach both more specific and more sensitive when compared to serology.

Type I and type II psoriasis show a similar usage of T-cell receptor variable regions.

In nonpustular psoriasis, principally two forms can be distinguished [Christophers E. Henseler T: Patient subgroups and the inflammatory pattern in psoriasis. Acta Dermatol Venereol 69(suppl 151):88-92, 1989): Type I frequently shows positive family history, linkage disequilibrium for human leucocyte antigens (HLAs) Cw6, B13 and Bw57 as well as an early onset. Type II manifests itself around the 5th decade, it is more frequently than normal associated with Cw2 and B27. In the light of this association with HLAs an autoimmune pathogenesis has been discussed. In order to investigate the pathogenetic function of T cells we obtained biopsies from patients with type I (n = 10) and type II (n = 10) psoriasis. Three-step peroxidase staining was performed using a panel of monoclonal antibodies directed against five variable (V) regions of the beta chain (V beta 5a, V beta 5b, V beta 6, V beta 8, V beta 12) and one of the alpha chain (V alpha 2) of the T cell receptor (TCR). Positive or negative selection of a particular TCR V region could not be detected in the demonstrable repertoire. Furthermore, the usage of the V regions under investigation revealed a similar pattern in the two forms of psoriasis.

[Concomitant psoriasis vulgaris and atopic dermatitis. A study of 1,065 patients with psoriasis].

The simultaneous presence of psoriasis and atopic dermatitis was checked for in 1065 patients with psoriasis and was found in 18 of them (1.7%), which is in keeping with the statistical probability. In these 18 the efflorescence was also examined histologically. All 18 (100%) reported marked pruritus; white dermatographism was observed in 16 patients (89%); in 15 patients (83%) psoriatic phenomenas could be produced, and 11 patients (61%) also had allergic rhinitis or bronchial asthma. Immunologically, 16 of the 18 patients (89%) showed elevated IgE levels, the geometric mean level being 888 IU/ml. The HLA-Cw6 was positive in 9 patients (50%).

[Liberation of human leukocyte elastase by hypertonic saline baths in psoriasis].

Human leukocyte elastase, a proteolytic enzyme of neutrophils, can be determined by a highly sensitive enzymatic assay. Bathing in hypertonic salt solutions allowed considerable amounts of human leukocyte elastase to be eluted from psoriatic lesions. Optimal elution was achieved with sodium chloride concentrations of 1 M and higher. Thirty patients with psoriasis of varying degrees of severity released significantly increased amounts of human leukocyte elastase following a 10-min bath in salt water. During daily treatment with salt water baths and UV-B radiation the elutable amounts of elastase decreased dramatically within a few days, reaching normal levels when the skin had cleared. Determination of human leukocyte elastase in salt water eluates of psoriatic skin seems to be useful for quantification of therapeutic effects. It appears conceivable that human leukocyte elastase plays a role in the psoriatic tissue reaction.

[Thermo-sodium phototherapy of psoriasis vulgaris].

Report on the treatment of psoriasis in saturated sodium chloride water from a natural source followed by irradiation with ultraviolet UV-A- and UV-B-lights, similar to the treatment at the Dead Sea. Approximately 5335 patients were treated in this manner since 1977. Over 80% of all cases were cleared within 4-6 weeks, whereas 72% of cases with psoriasis-arthritis were improved. This treatment does not influence the rate of relapses. The possible effects on the skin of 27% saturated sodium chloride water is described in detail.

[Psoriasis patients in the sauna].

An examination of 213 male psoriasis patients showed neither a positive nor a negative influence on the skin lesions for 87,9% of the patients, an improvement for 10,7% and a deterioration for 1,4% in a sauna bath. Evaporation measurements of untreated psoriasis in a sauna bath showed a 50% decrease in perspiration. This was proved also by the silicone print method. These values increased after removal of the psoriasis-scales. Skin temperature measured at the same time did not show any difference.

[Experience with the Thermalsole-phototherapy in psoriasis vulgaris].

Report on 250 patients suffering from psoriasis, who were treated by a new method. This consists of a bath in saturated sodium chloride water from a natural source followed by irradiation with ultraviolet light (maximal output at 350 to 400 nm and peak at 323 nm respectively). 84 percent of the patients showed clearing after 24+/-10 days of treatment.
