[Pruritus--a little known iron-deficiency symptom].

Five episodes of iron deficiency anemia associated with pruritus were observed in 4 women. In contrast to the majority of cases in the literature the pruritus in our patients was localized and not generalized. It disappeared 1--14 days after the beginning of iron replacement. --Iron deficiency should be considered in the differential diagnosis of both generalized and localized pruritus. The literature suggests that pruritus associated with low serum iron levels in malignancy may respond to iron therapy

[Pruritus in Hodgkin's disease responding to cimetidine].

A 34-year-old woman who had had intractable pruritus for 2 years experienced dramatic relief from the symptom under treatment with cimetidine. On investigation, stage IIIb Hodgkin's disease was diagnosed and responded well to chemotherapy with a remission lasting now for 2 years. After the beginning of chemotherapy cimetidine was discontinued and the pruritus did not recur.

[The treatment of arrhythmias with amiodarone].

The efficacy of amiodarone in the treatment of cardiac arrhythmias is discussed in the light of published results and our own experience. Oral and intravenous administration are compared with respect to electrophysiologic changes in the heart. The possibility that these changes are mediated by amiodarone-induced inhibition of metabolic pathways in cardiac tissue which depends on thyroid hormone is discussed. Among the undesirable effects, chief consideration is given to thyroid function disturbances.

The effect of a single dose of propranolol on human erythrocytes in vivo.

In vitro propranolol has a profound effect on erythrocytes as indicated by a decrease of the packed cell volume and a shift to the right of the oxygen dissociation curve. Because of the lack of satisfactory in vivo studies, the effect of 80 mg propranolol and placebo was compared in twelve volunteers in a double-blind study. Although plasma propranolol concentrations varied greatly from individual to individual, all propranolol subjects demonstrated effective beta-blockade. Packed cell volume, mean cellular volume, plasma trapping after haematocrit centrifugation, P 50, 2,3-DPG, ATP, erythrocyte filterability and blood viscosity at different packed cell volumes (50% and 80%) and shear rates (11.5, 23, 46, 115 and 230 s-1) did not change significantly following administration of either propranolol or placebo. These findings are of interest in view of the use of beta-blocking agents in critically ill patients.

Red cell filtration by paper filters.

A method for the assessment of red cell filterability with paper filters is described. Two milliliters of a washed red cell suspension with a hematocrit of 50% was filtered through a filter paper cone in a glass funnel and the filtration half-time (FT1/2) was measured. The filter papers were calibrated by previous measurement of the filtration time for the suspending solution, the calibration time (CT). A linear correlation between CT and FT1/2 was found for individual red cell suspensions (p less than 0.001) as well as for a normal population (p less than 0.001). Consequently, the index FT1/2/CT should represent a more reproducible parameter than FT1/2 and experimentally this appears to be the case. In fact, elimination of erratic values in a normal population was realized. Furthermore, discrimination between normal and abnormal values was improved and the coefficient of variation of single measurements passed from 28.4 to 9.4%. The reproducibility and reliability of this extremely simple method are therefore decisively enhanced without complication of the procedure.

[Erythrocyte migration on filterpaper--a simple micromethod for the measurement of erythrocyte filtrability (proceedings)].

A micromethod for measurement of erythrocyte filtrability is described. The time is recorded for the spreading of drops (25 microliter) of an erythrocyte suspension in filter papers which are calibrated with the suspension solution. The spreading time for the erythrocytes in relation to a reference calibration time correlates well with the filtration time of a modified filtration technique according to TEITEL.

Anemia, Hemolytic↗

[Congenital cytomegalovirus infection after maternal renal transplantation (author's transl)].

A 34-year-old female gave birth to a 32 to 35 week-old girl one year after she had undergone renal transplantation. The child was dwarfed and microcephalic and also had congenital platelet deficiency and purpura. In addition it was anaemic and suffered from severe neonatal icterus. An initial hypogammaglobulinaemia disappeared except for low IgA levels. The immunoglobulin deficiency is probably correlated with the virologically and serologically proven cytomegalovirus infection. The incidence of this disease seems to be high in children of mothers with renal transplantations.
