Influence of ribavirin on the micronucleus formation and in vitro proliferation of human lymphocytes.

Cytotoxic effects of the antiviremic drug ribavirin (1-beta-D-ribofuranosyl-1,2,4-triazole-3-carboxamide) were evaluated in vitro measuring micronucleus formation and cell proliferation kinetics in whole blood cultures employing cytokinesis block (CB) micronucleus test. The cells were exposed to ribavirin doses ranging from 0.05 0.65 micromol/ml at three different incubation times. The frequency of micronuclei in treated samples demonstrated relatively low ability of ribavirin to induce micronuclei. However, the lowest concentration ofribavirin markedly changed the frequency of mononucleated and multinucleated cells, particularly binucleated ones, which declined significantly. The decline in the frequency ofbinucleate cells was followed with accumulation of greater number of mononucleate cells. Decreased proliferation potential of lymphocytes treated with ribavirin indicates that cells are arrested prior to metaphase. The present investigation showed that ribavirin is capable to induce a delay in cellular proliferation at all the doses assayed. The study demonstrated that CB micronucleus assay is simple and rapid method that can be used to assess toxic effect of drugs in vitro.

Radiosensitivity of human lymphocytes in vitro correlates more with proliferative ability of cells than with the incidence of radiation-induced damages of the genome.

The purpose of this study was the estimate the extent of individual variability in radioresponse of human lymphocytes in vitro and to establish the reasons of variability. Individual variability in radiation-response was evaluated using the cytochalasin B micronucleus test among 82 healthy individuals (36 men and 46 women), of mean age 38 (range 30-48). Blood samples were irradiated with gamma (60Co) rays at a dose of 2 Gy in vitro. The yield of radiation-induced micronuclei (MN), cytochalasin blocked proliferation index (CBPI), fraction of micronucleated binucleate (BN) cells and mean incidence of MN per micronucleated BN cells at a sampling time of 72 hours were scored. Our results brought out a significant effect of gender on the level of spontaneously occurring micronuclei, the lack of statistical differences between gender in the yield of radiation-induced micronuclei and marked variability in radiation response among individuals. Likelihood of expressing hypersensitivity was correlated with ability of cells to proliferate in vitro (beta = 0.41, p < 0.000) more than with the incidence of radiation-induced micronuclei per micronucleated cell (beta = 0.20, p < 0.000).

Divalent cation-activated-ATPase and ecto 5'-nucleotidase activities in chronic tonsillitis.

The activities of two ectoenzymes,Mg(2+)-ATPase (ATPase) and 5'-nucleotidase (5'-NT) from tonsillar mononuclear cells (TMC) and peripheral blood mononuclear cells (PBMC) were investigated in idiopathic tonsillar hyperplasia and recurrent tonsillitis. The ATPase activity of TMC was significantly higher in recurrent tonsillitis than in idiopathic tonsillar hyperplasia, whereas no difference was demonstrated in ATPase activity of PBMC. The activity of 5'-NT was similar in both investigated groups. However, ATPase and 5'-NT activities were significantly higher in TMC compared to PBMC. Such results suggest a possible role of ATPase in the activation of TMC during the course of chronic tonsillitis and indicate a difference in the function of TMC and PBMC.

[Etiopathogenic, diagnostic and therapeutic aspects of stress fractures].

Stress fractures occur with strenuous activity and represent a unique and relatively rare traumatic entity. Their diagnosis is difficult and therapy accompanied with specific problems. The purpose of the presented study is to explain basic characteristics of stress fractures and to approximate possibility of diagnosis and treatment better. Out of 26 fractures, 22 (84.62%) were nondisplaced and managed by bed rest, non-weight bearing or plaster of Paris immobilization for eight to ten weeks. On the other hand, four primarily displaced fractures (15.38%) were successfully treated with rigid internal fixation. Four conservatively managed patients (18.18%), two with fractures of the tibia and two with fractures of the femoral neck, attained a secondary angulation and pseudoarthrosis of the fracture site and, for these reasons, recorded injuries demanded a compensatory surgery management. It is suggested that in case of suspectability of stress fractures it is beneficial to use, parallel to native radiographic study, the bone scan imaging techniques which in the earlier phase of the disease establishes the diagnosis. Treatment of the stress fractures should be, as a rule, conservative. Moreover, "fatigue" fractures of the tibia and femoral neck ask for more continuous observations and a serious access. If non weightbearing regiment and immobilization do not decrease the difficulties; and fracture patterns progress, or if fracture becomes displaced because of delayed diagnosis, open reduction and rigid internal fixation should be done without delay.

[Fractures of the spine in patients with ankylosing spondylitis].

Eight patients with spine fractures in chronic ankylosing spondylitis have been analyzed. The purpose of this article is to determine possibilities of spine injuries accompanied with this specific rheumatic disease and to show difficulties in diagnosis and problems in treatment. Ankylosing spondylitis affects spine in the way that it ossifficate ligaments, synovial joints and other soft tissue structures. Fractures that occur through these areas involve both bones and ligaments, producing an unstable condition, similar to a shearing type of classic spine injuries. The radiologic diagnosis of these injuries is difficult because the bone is frequently osteoporotic and displacement is of minor degree. In this study, 41.67% of the patients were not diagnosed initially and 16.67% were, for this reason, deteriorated neurologically. Therefore, patients known to have ankylosing spondylitis should be examined regarding the possibility of a fracture, and if pains persist after an injury, they should be thoroughly investigated radiologically to rule out a potentially serious problem. Reduction of the displacement and adequate stabilization, preferably by surgery, should be achieved whenever possible.

[Evaluation of long-term prognosis in patients with whiplash syndrome].

We reviewed 50 patients who sustained whiplash injuries of the neck. The mean time of the follow-up was 3 years. Only 16% of observed cases recovered completely. Residual symptoms were severe in 38%. Pain in the neck, headache and paraesthesia were the commonest complaints. Older patients and patients with significantly restricted movements of the neck, sharp local kyphotic deformity, chronic instability and neurologic involvement had a worse prognosis.

[Sexual information and practice in 16-year-old adolescents].

We conducted an anonymous poll among 221 girls and 106 boys from the second grade of secondary schools in Somboru, Crvenka and Ruski Krsturu in order to collect data on some aspects of their sexual knowledge and behavior. By the 16th year of life 8.6% of the girls and 36.8% of the boys experienced coitus, while 48% of the girls and 72% of the boys experienced intimate caressing with the opposite sex. It has been assessed that their knowledge on the physiology of the sexual life and contraception is poor and overestimeted, especially among the boys. The sources of information are mostly inadequate which necessitates an organized approach to sexual education.

[Rupture of the spleen and the surgical approach].

In introduction, the authors present, theoretical considerations about divide of spleen injury, and give appropriate statistical data. In their material consisted of 25 traumatically ruptured spleens during period 1974-1988, they deal, among other questions, with necessity of drainage after splenectomy. As for operative procedure, they point out splenectomy, but also mention other surgical treatments that they are not familiar with. In conclusions they insist on early and precise diagnosis (especially in multi injured patients) and emphasize close relationship between drainage and local intraoperative finding.

[Drainage of the pericystic cavity in echinococcosis of the liver].

The authors emphasize that echinococcosis is cosmopolitan parasitosis and liver is the most often target organ. The most common complications are summarized. At any slightest doubt of the liver infection, the authors recommend modern diagnostic tools (ultrasonography CT) in liver examination. They analyze their lo years experience (20 hepatal). The conclusion is that basic therapeutic procedure is removing of pericyst content with hydatid membrane and residual cavity is treated later. The authors accept updated biac of liver echinococcosis treatment (radical removing of pericyst performing typical or atypical resection, or total lobectomy in case of lobar destruction). However they point out that drainage operations (marsupialization, external drainage with rubber drain, irrigation-aspiration drainage, T-drainage and internal drainage omentopeksia, anastomosis of the cavity with jejunal lumen) still have their place and value in Surgery.

Oligosymptomatic glomus tympanicum and caroticium with two different histological pictures.

Though glomus tumors have been relatively frequently reported, there is very little material about their differential diagnostic problems. The authors present a case of glomus tympanicum and glomus caroticum with only otological symptoms, facial paralysis and traumatic arterio-venous shunts in the neck. These two tumors had few different patterns of glomus tissue.
