[Decreased bone marrow cellularity and hemosiderin in normal and overtrained runners].

The frequency and significance of decreased bone marrow cellularity (BMC) and bone marrow hemosiderin (BMH), and their possible relations to hemoglobin, iron, iron saturation and serum ferritin levels, were studied in 18 normally trained and 18 overtrained competitive distance runners. Decreased BMC was more pronounced in the overtrained runners: mild to moderate hypocellular marrow was found in 25% of them, and severe hypocellularity in another 25%, as compared to 18% and 3.5%, respectively, in the healthy runners. Increased erythrocyte mean corpuscular volume (greater than 95 mu 3) was found in 24% of the runners, but in only 3.5% of the controls. Decreased BMH was found in both the overtrained (1.3 +/- 2.0 hemosiderin granules per 100 normoblasts) and the controls (1.5 +/- 1.9 granules). There were no significant differences in levels of hemoglobin, mean corpuscular volume and serum iron, iron saturation and ferritin. The decreased BMH in the face of normal hemoglobin and serum iron and ferritin suggests that as a group, competitive distance runners do not suffer from overt iron deficiency but that iron might be stored differently, with a lesser proportion in the bone marrow. The more pronounced decrease in BMH of overtrained distance runners may indicate a possible relationship, and decreased BMC may identify runners who may be becoming overtrained.

Cell membrane activities and regeneration mechanisms as therapy mediators in moxibustion and acupuncture treatments: theoretical considerations.

Needling by acupuncture or heating by moxibustion, the major Chinese medicine modalities, include tissue destruction or increased permeability of cell membrane. Both action potential activities and the release of cellular metabolites responsible for regeneration occurs. These phenomena are eventually abolished by local and systemic inhibitory elements being metabolites or neurogenic. The inhibitory effect induced by the acupuncture and moxibustion and directed to the manipulation site of these modalities may affect other anatomical sites and reduce or prevent neoplastic growth and neuromuscular or cardiac membrane activity disturbances.

[Endobronchial tuberculosis].

Endobronchial tuberculosis (EBTB) is not rare. It usually affects the upper lobe bronchi. Bronchoscopic examination may reveal white gelatinous material in the bronchial lumen, mucosal ulcerations, polypoid inflammation, scattered green-blue pigmented areas and narrowed bronchi with post-stenotic dilatation, some of which resemble colored dolomite caves. 2 women, aged 70 and 84, are presented, 1 of whom had been suffering for a year from persistent dry cough and the other from diffuse pneumonia resistant to several antibiotics. Fibreoptic bronchoscopy showed the characteristic picture of EBTB, later confirmed by positive cultures. Recognition of the clinical, radiological and bronchoscopic spectrum of EBTB makes it possible to start treatment before bacteriological diagnosis of tuberculosis from endoscopically recovered material.

The role of acupuncture in asthma: changes in airways dynamics and LTC4 induced LAI.

A cohort of nine extrinsic asthma patients were treated by means of acupuncture. Patients were followed up for changes in medical treatment, spirometry, skin reactivity to immediate type reactions, total serum IgE levels and reactivity of their leukocytes to leukotriene C4 challenge (LTC4 induced leukocyte adherence inhibition (LAI) assay). Our results show that after acupuncture, treated patients were able to reduce bronchodilator and taper completely corticosteroid therapy. No change in skin reactivity or in IgE levels were noted. However, acupuncture treatment was able to negate, in 66.6%, the positive LTC4 induced responses.

[Mammary tuberculosis mimicking breast cancer].

Breast masses in the 7th decade of life should be considered cancerous and immediate diagnosis and treatment are indicated. Mammary tuberculosis is rare, especially in developed countries. It usually results from reactivation of dormant pulmonary and pleural tuberculous foci. We present a 66-year-old woman who was hospitalized for biopsy of a breast mass. 4 years previously there had been trauma to the chest, and several pleurocenteses were required to remove sterile exudate. The past history, recent immigration from an endemic area, physical examination and chest X-ray, suggested tuberculosis. Needle aspiration of the affected breast yielded M. tuberculosis. Specific therapy and corticosteroids resulted in complete cure.

Serratia marcescens infected silk suture rejected by combined acupuncture, moxibustion and low-power laser therapy from the abdominal fascia.

Upper abdominal pains lasting 12 years after cholecystectomy, were improved in an 82-year-old woman following the rejection of indigestable silk surgical sutures induced by combined therapy of acupuncture, moxibustion and low-power laser beam irradiation directed to an old post-cholecystectomy scar. An inflammatory reaction followed by granulation tissue mass was developed. Embedded in the granulation tissue were the above mentioned silk sutures which finally were expelled through the skin at the operation scar. A surgical procedure suggested to the patient, in case of acupuncture therapy failure, was obviously avoided. Serratia-marcescens infection of the expelled material was bacteriologically defined.

Immobilization stress and direct glucocorticoid effects on rat septohippocampus.

The rat septohippocampal cholinergic system to a large extent regulates the adaptive physiological and behavioral response to stress. The mesoseptal dopaminergic (DA) system, one of the converging inputs to the lateral septum, exerts a tonic inhibitory action on the septohippocampal cholinergic neurons. High concentrations of pituitary-adrenocortical hormones in plasma may activate the septohippocampal cholinergic system. We have sought to determine whether this mode of activation may be directly initiated by hormonal action on the cholinergic terminals, or indirectly induced through an alteration in the DA septal inputs. The results indicate that stress initiates rapid and transient changes in DA uptake by septal DA terminals, changes which probably contribute to the initial transient activation of the hippocampal cholinergic system. While the effects of glucocorticoids, observed in vitro, may mimic the enhanced ACh release in stress, they do not mimic the increased choline uptake. Nevertheless, high glucocorticoid concentrations may act directly on septal dopaminergic terminals to reduce their DA uptake capacity. These results imply that the septohippocampal cholinergic activity represents an integrative pathway for neuronal and hormonal signals of stress.

Sheep bronchoalveolar carcinoma: tissue associated protein complex (TAPC) in normal lung tissue and in the tumor differed quantitatively.

Sheep lungs experimentally and naturally affected by bronchoalveolar carcinoma were washed out exhaustively of soluble components by phosphate-buffered saline, pH 7.4 (PBS), followed by glycine buffer, pH 2.8 (GB), and then again by 1M KCl followed by PBS. The tissue matrix (TM) of the tumor-free region and the tumor-affected tissue were analysed separately by sodium dodecyl sulfate (SDS) polyacrylamide electrophoresis. Normal lung tissues obtained from normal sheep served as controls. Several protein fractions and fragments, identified in both the normal and the tumorous lung, have the molecular weight (MW) of 130,000-228,000, as compared with the major soluble tissue associated protein having MW of 70,000. Coomassie blue staining used in the SDS polyacrylamide system and alkaline phosphatase immunoreaction used in the Enzyme Linked Immunosorbant Assay (ELISA) showed tenfold increased concentration of the TAPC in the TM of the tumor tissue and in the blood of tumor-affected animals, respectively. Total concentration of the TAPC in the serum of tumor-affected animals was higher than in the normal. Immunofluorescent antibody test (IAT) detected the TAPC in the cytoplasm of tumor as well as in normal lung cells, and the study suggested that the TAPC reaches the peripheral blood during tissue destruction occurring at the tumor site, as observed by light and electron microscopy (LM and EM). The concentration of each of the TAPC fractions was higher in the tumor-affected sheep lung as compared with normal sheep lung. Antibodies prepared against the TAPC fractions were toxic to sheep lung cells in tissue culture. Tumor cells were more susceptible.

In vitro presynaptic modulation of cholinergic hippocampal activity by pituitary-adrenocortical hormones.

In vivo studies have shown that high blood concentrations of pituitary-adrenocortical hormones can activate the hippocampal cholinergic terminals. Incubation of hippocampal synaptosomal preparations with methylprednisolone, or with ACTH at concentrations comparable to stress-induced high concentrations in plasma, did not have any significant effects on the cholinergic parameters measured under unactivated conditions. In the presence of either high K+ or of ACh, choline uptake was decreased. This decrease was not affected by methylprednisolone. However, methylprednisolone did enhance ACh release both after a previous increase (induced by K+) or a decrease (induced by ACh) of ACh release. In contrast, ACTH had no direct effects on either unactivated or K+-stimulated synaptosomes. Thus, a differential effect was exerted by methylprednisolone on the two presynaptic regulatory mechanisms: choline uptake (no change) and ACh release (increase). We suggest that the activation, observed in vivo, resulted mainly from indirect action of the hormones on the hippocampal cholinergic terminals, in view of the fact that the direct effect in vitro was partial.

Gold-induced ileitis.

We describe a patient with rheumatoid arthritis who developed severe ileitis after being treated with gold salts; the patient responded to elimination of the gold therapy and to BAL.

Rejection of deeply implanted foreign body induced by acupuncture treatment in patient suffering intractable pains lasting ten years following traumatic injury to the left scapular region during a road accident.

Upper back pains lasting ten years were promptly improved in a 63 year old woman following the rejection of a forgotten foreign glass body, through a scar located over the left scapula, and treated by acupuncture. The patient insisted on having this treatment after unsuccessful prolonged and multi-disciplinary formal medical interventions culminating in preparations for exploratory surgery. The acupuncture directed to the scar induced an inflammatory reaction characterized by a sterile granulomatous reaction which surrounded a large piece of glass, apparently implanted in the soft tissue of the suprascapular area during a severe road accident which occurred ten years previously. The surgical intervention was avoided.

Tension headache treated by anti-inflammatory drug injected into GB 20 acupuncture point.

The anti-inflammatory drug Voltaren (0-(36-dichlorophenyl)-amino acetate) is a relatively new substance in the systemic treatment of various pain inducing diseases related to the musculoskeletal elements. In order to achieve effective results, relatively high dosage, applied P.O.P.R. or I.M. should be used for prolonged periods. The acupuncture methods presented the eastern minded western physician with a detailed list of trigger points which may have abrogated certain painful symptoms when appropriately used by acupuncture maneuvers alone. In the present study the possibility of using the appropriate acupuncture point GB 20 for the injection of Voltaren, in an attempt to relieve stable and prolonged tension headaches, has been considered. Thus, acceptable and combined eastern-western treatment method, if found successful, may be evolved. GB 20 point has been selected because of its effective influence among others, on tension headaches, a highly common and most distressing syndrome poorly treated by conventional approaches in most clinics.

The contribution of thermo-moxibustion to surgical treatment in transplanted mouse mammary carcinoma.

BALB/C mice bearing progressive transplanted mammary carcinoma were treated by surgical removal of the tumor mass either with or without thermo-moxibustion, applied to the GV 14-point-equivalent. A surgical removal of the tumor, 14 days post-inoculation resulted in 61% of deaths compared with 90.0% of deaths in the sham operated control. By the addition of the thermo-moxibustion therapy, the mortality rate, in the surgically treated group was reduced to 37.5%. Surgical removal of the tumor mass at day 17 post-inoculation resulted in 70% death while surgery supported by thermo-moxibustion protected the animals to the range of 40% death. Surgical removal of the tumor mass followed by local heterologous immuno-therapy was very effective when applied 14 days post-inoculation (12.5% death only). However, the same procedures applied at day 17 post-inoculation were not effective (80% death). Surgery and moxibustion together were more effective than surgery alone. Surgery and immunotherapy were very effective when applied from day 14 post-inoculation. Thermo-moxibustion as the sole treatment was effective when applied either before or very close to the tumor cell inoculation (35% of death and 33% of death respectively, as compared to 61.7% death in the control). Thermo-moxibustion applied before and after tumor cell inoculation was more effective (15% death rate). Thermo-moxibustion either alone or in conjunction with surgery is a contribution to the protection of BALB/C mice against the early and the late development of mouse mammary carcinoma.

Position effect variegations and brain-specific silencing in transgenic mice overexpressing human acetylcholinesterase variants.

Position effect variegations as well as brain-specific silencing were observed in novel transgenic mouse pedigrees expressing human acetylcholinesterase (AChE) variants. Muscle AChE activities varied between 1.6- and 350-fold of control in these lines, one carrying insertion-inactivated InE6-AChE and two with 'readthrough' I4/E5 AChE, all under control of the ubiquitous CMV promoter. In contrast, brain AChE levels remained within a range of 1.5-fold over control, suggesting an upper limit of brain AChE which is compatible with life.
