Disability days associated with detection and treatment in a hypertension control program.

Labeling individuals as hypertensive has been associated with increased absenteeism from work. Distributions of disability days were compared for the years before and after screening for hypertension among 10,049 participants in a clinical trial of intensive antihypertensive therapy. Stepped care participants attended special clinics and referred care participants returned to their usual sources of medical care. For those unaware of their hypertension at screening, disability days increased between baseline and one year among referred care (n = 1805), while there was no change among stepped care (n = 1842) participants. Neither referred care (n = 1868) nor stepped care (n = 1906) participants who were aware but untreated at baseline reported a change in disability days over the following year. For those aware and treated at baseline, there was no change in disability days in referred care (n = 1275), but a decrease was reported in stepped care (n = 1353). Further descriptive analyses suggest that neither labeling alone nor treatment alone was associated with increased absenteeism. However, detection and treatment were associated with increased absenteeism among newly diagnosed referred care hypertensives, while management of previously treated hypertensives in stepped care clinics was associated with reduced absenteeism. Because these latter comparisons are made between groups stratified on a postrandomization variable, selection bias is possible and interpretations must be made with caution.

Mortality surveillance in collaborative trials.

The Hypertension Detection and Follow-Up Program (HDFP) carried out two pilot surveillances covering the enumerated population to test procedures to be used in assessing the ability of the program to influence life expectancy in the total population. A rigorously sequenced pilot survey of 2,611 households was conducted and carefully monitored through two mailings, telephone contacts, home visits, and communication with "contact" persons. The response rates at each stage varied among the 13 centers. Overall, there was a 42.7% yield from the first mailing; 42.5% of those receiving a second mailing was completed; 78.0% for telephone 61.3% for the home visits and 55.2% from the "contact" persons. Overall, 97.4% of all persons had vital status ascertained. The second phase relaxed the rigorous sequential survey requirements and reduced the reporting requirements from every ten days to monthly. Overall, 93.3% were successfully ascertained. Reduced survey structure, slightly increased mobility (from 12% to 13%), increased workload from 200 to 400 households per center, and a longer time interval between initial enumeration and the mortality ascertainment are among the reasons for performance decline.

[Relation system of electrocardiographic parameters in posterior myocardial infarction].

The polarcoordinates of the electrical heart vector in the groups of posterodiaphragmatic, postero-basal and extensive posterior myocardial infarctions were determined by the triaxicardiometer analogue computer. The ecg leads of Franks' system were numerically and graphically reconstructed from the digitalized polarcoordinates using formulas of connection between spherical polarcoordinates and rectangular Cartesian coordinates. Then the 12 conventional leads, by the use of coefficients were synthetized from the reconstructed leads of Franks' system and of the conventional leads were performed by the ODRA 1204 computer. So was created the connection system of the electrocardiological parameters, where the informative value of the polarcoordinates and the Cartesian coordinates-also mathematically-is exactly alike.

The effect of ethylene glycol and DMSO on fusion of isolated sarcoplasmic reticulum membranes.

Fragmented sarcoplasmic reticulum (FSR) vesicles from rabbit skeletal muscle were suspended in 5-10% ethylene glycol (EG) or in 5, 10, or 15% dimethylsulfoxide (DMSO) and were pelleted onto flat aluminum foil disks. No vesicle fusion o-curs with either 5 or 10% EG treatment and 2-21/2 hours drying. After 4-5 hours drying, 5% EG-treated vesicles have established more areas of close contact and individual vesicles have begun to flatten when compared with the 10% EG vesicles remained round as in the 2-21/2 hour samples of all treatments. Overnight drying also causes extended sheets of bilayer pairs to form in the 5% DMSO-treated samples but, with 10 and 25% treated vesicles, destroys the double bilayers and only occasional dense regions of membrane whorls remain. Both EG and DMSO promote morerapid fusion of FSR vesicles than does glycerol but overnight drying after treatment with 10 or 25% DMSO DESTROYS THE FUSED MEMBRANE.

Effect of lipid modification on fusion of sarcoplasmic reticulum vesicles.

Freeze fracture technique ascertains that sarcoplasmic reticulum vesicles fuse during anisodiametric dehydration which leads to the formation of sheetlike structure of a mean extension of 5000 A. Modification of the membrane lipids by phospholipase A2 digestion or the incorporation of deoxycholate facilitates the coalescence of the vesicles, while it is completely prevented by lipid removal. Membrane fusion during anisodiametric dehydration is considered as resulting from the close contact of highly curved edges from which the membrane proteins have been excluded.

Fusion of isolated sarcoplasmic reticulum membranes.

Fragmented sarcoplasmic reticulum (FSR) vesicles from rabbit muscle were suspended in 1.5-5% glycerol solutions and were pelleted onto aluminum foil disks in a modified centrifuge tube. Examination of these pellets in the electron microscope after drying for 2--2.5, 4--5.5, and 21 hours revealed a progression of changes. First, distances between individual, round vesicles decreases. Next, somewhat flattened vesicles establish limited areas of contact with adjacent vesicles. Finally, vesicle fusion occurs and extended areas of double bilayers are formed. A water loss-time interaction appears to be needed for the fusion process. A Hg-phenyl azoferritin compound was used as a marker to identify intra- and extra-vesicular space in the fused samples. Quantitative measurements of birefringence during imbibition of pellet slices in a graded series (eta-1.41-1.43) is reached. The plateau seen in this part of the curve is again followed by steadily increasing birefringence at higher glycerol concentrations. This interruption in the birefringence curve is presumably due to a matching of the refractive indices of the glycerol solution and a lipid component in the membranes.

Effects of labeling and treatment of hypertension on perceived health.

The effects of labeling a person as hypertensive have important implications for hypertension screening. The Hypertension Detection and Follow-up Program (HDFP) provides an opportunity to examine the effects of labeling, treatment, and study assignment on a large group of hypertensives (n = 10,070). Their answers to questions regarding perceived health and general well-being asked at baseline and again one year later were analyzed. There was no significant change in the perceived health status of persons who were unaware of their hypertension at baseline and remained untreated at one year (labeling alone). The effect of labeling plus treatment was associated with a significant decrease in perceived health. The effect of antihypertensive drug therapy on perceived health status was examined in persons who were aware of their hypertension but not on treatment at baseline, and on treatment at one year. The stepped care group (SC) had a significant improvement in their perceived health and a significant decrease in the amount of time spent worrying about their health. The referred care group (RC) had no change. Program assignment effects were studied in individuals aware of their hypertension and on treatment both at baseline and one year later. Both the SC and RC groups had a significant improvement in their perceptions of their health status. The SC group had a significant decrease in time spent worrying about their health, while the RC group showed no change. These reassuring results fail to support the suggestion that labeling persons as hypertensive is necessarily followed by negative psychological consequences.(ABSTRACT TRUNCATED AT 250 WORDS)
