[Taenia crassiceps in the anterior chamber of the human eye. A case report].

An otherwise healthy 15-year-old girl developed an iridocyclitis in her right eye. A contractable living parasite measuring two millimeter could be detected in the anterior chamber, the other parts of the eye remained uninvolved. The patient had close contact with the young dog of the family. By using a viscoelastic substance the parasite could be removed alive without any complications. The tapeworm was found to be too young for having developed the characteristic scolex. Western-immunoblotting gave indirect evidence for infection with Taenia crassiceps, a dog's tapeworm. During an observation period of two years no evidence for any other ocular or general Taenia crassiceps infestations could be found.

[Retinal lymphoma--simulating uveitis].

A rare form of malignant large-cell non-Hodgkin's lymphoma or reticulum cell sarcoma is confined to the eye and central nervous system (CNS). We report the case of a 62-year-old man, who presented with recalcitrant progressive uveitis in his right eye. Although lymphoma was suspected, the patient refused to undergo vitrectomy. Over one year he developed large blister-like solid scattered lesions of the retinal pigment epithelium (RPE) in the same eye. Repetitive neuroimaging detected no CNS involvement. After developing optic disc-swelling and discrete RPE-changes in the left eye also, the patient agreed to enucleation of his right eye. Histopathology and Immuno-histochemistry revealed malignant intraocular large cell-lymphoma invading all eye-tissues. Liquor cytology showed lymphomatous cells. Suffering from high grade malignant lymphoma with CNS ivolvement, the patient had to undergo radiation therapy of the orbits and neuroaxsis as well as intravenous and intrathecal chemotherapy. Papilledema of the left eye resolved quickly and completely, RPE-infiltrations disappeared and led to discrete punched out lesions with no scarring, the lens remained transparent over the whole observation period of 25 months. the patient has been surviving for more than 52 months after first suspicion of malignant lymphoma, he has retained full vision in his left eye until now.

A film video processor for the viewing of fluorescein angiograms.

A new, commercially available, and inexpensive film video processor facilitates the viewing of fluorescein angiograms on video monitors. The negative film is inserted into a film carrier, read by a charge-coupled device solid state pickup device and presented as a negative or positive image on a black-and-white monitor with the help of a negative/positive conversion feature. Details of the angiogram are studied by zooming into any part of the image, which magnifies the image up to six times. Contrast and brightness adjustment on the video monitor allows examination of under- or overexposed angiograms.

Fluorescein Angiography

[Therapy of parapapillary subretinal neovascularization].

A large variety of diseases may cause a subretinal neovascularization originating from choroidal vessels. Four cases of parapillary choroidal neovascularisation are described, including neovascularization caused by senile and myopic degeneration, by drusen of the optic nerve head, and by angioid streaks. Obliteration of the neovascular membrane with confluent argon laser coagulation reduced metamorphopsia and prevented a progredient loss of visual acuity in all four patients.

The correlation of the results of vitreous fluorophotometry and fluorescein angiography in nonproliferative diabetic retinopathy.

The authors examined 103 eyes of 53 patients with insulin-dependent diabetes mellitus simultaneously by fluorescein angiography and vitreous fluorophotometry and correlated the stage of diabetic retinopathy with the posterior vitreous leakage (PVL) level, calculated from the preinjection-, bolus-, and measurement scans. There was a significant correlation between stage of retinopathy and PVL level, but there was a large variation of PVL readings within each group of retinopathy stages. All eyes were reexamined after a mean period of 11.84 months. In 15 of 16 eyes with progressive retinopathy the first PVL readings were within the distribution range of the respective retinopathy level. A progression of retinopathy was not accompanied by a significant change of the PVL. It was concluded that vitreous fluorophotometry alone is not sufficient for the grading of diabetic retinopathy and has only little prognostic value for the course of the disease.

Long-term changes of the corneal endothelium following intracapsular cataract extraction with implantation of open-loop anterior chamber lenses.

The central corneal endothelium of a consecutive group of 158 patients with senile cataract was examined before intracapsular cataract extraction with implantation of an open-loop anterior chamber lens and 6, 12 and 48 months after surgery. Corneal endothelial morphometry was performed using an automated digital image analysis system. After 6 months of follow-up, there was an average loss of 10.8% in central corneal endothelial cell density, which increased to 14.8% after 12 months and 18.8% after 48 months. The early post-operative endothelial cell loss was correlated significantly with a decrease of cell pattern stability and polymegathism (P less than 0.0001). There was also a significant correlation between the continuous post-operative cell loss and the simultaneous changes of pattern stability and polymegathism and the age at the time of surgery (P less than 0.05).

[A new iridocorneal endothelial syndrome].

Three iridocorneal endothelial syndromes are distinguished: essential progressive iris atrophy, iris nevus (Cogan-Reese) syndrome, and Chandler's syndrome. Obviously, there are patients in whom findings do not correspond fully to any one of the three syndromes; signs and symptoms which justify being considered as a new iridocorneal endothelial syndrome are described.

Atrophy

[Candida retinitis following gastrectomy].

The authors report on a 68-year-old patient who developed disseminated retinochoroiditis and vitritis following extensive gastrectomy for a cardial carcinoma. The clinical diagnosis of an endogenous Candida infection was confirmed by positive identification of Blastomyces from the subclavian catheter tip and the antibody titer. Resolution was achieved with orally administered ketoconazole and intravenously administered miconazole therapy.

Effect of timolol on aqueous humor protein concentration in the human eye.

We performed a double-masked, controlled study investigating the increase in the protein concentration in aqueous humor after topical timolol maleate administration. Forty patients scheduled to undergo intracapsular cataract surgery were included in the study. Either timolol ophthalmic solution or the vehicle of timolol alone (control) were applied topically in a randomized fashion, and aqueous humor was collected during surgery. Protein concentrations were determined by gel-filtration chromatography. The total protein concentration as well as the concentration of high and medium molecular weight proteins were significantly elevated in the timolol-treated group. The relation of the levels of the three different molecular weight classes did not differ significantly between the two groups, indicating that undisturbed protein filtration in the face of reduced aqueous humor production--and not a change in the permeability of the blood-aqueous barrier--is responsible for the increase in concentration.

[Pseudoprecipitates].

As a rule acute primary glaucoma can be diagnosed easily; however, if it is accompanied by pseudoprecipitates (precipitate-like changes of the posterios surface of the cornea), it can simulate a secondary, inflammatory increase in pressure. Pseudoprecipitates are thought to be caused by pressure-induced lesions of the corneal endothelium. They differ from true precipitates by their shape and distribution and appear to be a facultative sign of acute primary glaucoma. In cases with pseudoprecipitates examination of the corneal endothelium with the specular microscope does not reveal any specific findings, only an edema of the corneal endothelium (so-called cornea pseudoguttata).

Acute Disease

[Reversible posterior detachment of the vitreous body].

As a rule, posterior detachment of the vitreous body (even the simple, flat, incomplete type) is irreversible, but in some cases of fresh, moderate, retrovitreal hemorrhage caused by untreated vasoproliferative retinopathy there is evidence of spontaneous reattachment of the vitreous body. Details of this finding are described.

Humans

Fine structure of tapioca melanoma of the iris.

A case of a tapioca melanoma of the iris is studied by light and electron microscopy. Nodular elevations of the tumour were composed of spindle B- and epitheloid cells and of what appeared to be intermediate types. Tiny structures scattered over the entire tumour consisted of smaller epitheloid cells, many of which were found around superficial tumour vessels. Evidently, tapioca melanoma of the iris does not represent any specific histopathological entity. Tapioca melanoma of the iris is a low-grade malignant, locally aggressive tumour with a chronic clinical course.

[Computer-assisted morphometry of the corneal endothelium].

Until recently morphometry of the corneal endothelium depended on the time-consuming technique of manual digitization. Computer-assisted, interactive image analysis with the image processing system makes quick, exact and reproducible processing of endothelial micrograms possible. With the help of this method the homogeneity of the cell pattern and the regenerative activity can be evaluated and changes in the endothelium over time may be compared for statistical significance.

Cornea

[Treatment of venous thrombosis of the retina with prostacyclin].

Prostacyclin (PGI2) was used in the treatment of 10 patients with retinal venous thrombosis. All changes in 6 eyes with the non-ischaemic type of venous obstruction resolved completely, with marked visual improvement. The course of 3 ischaemic venous thromboses was unaffected by the treatment. In one eye ischaemic retinal areas were first noted months after therapy. All ischaemic fundi were subjected to adequate laser coagulation. In spite of the small number of patients it can be concluded that PGI2 does not seem to alter the course of the ischaemic type of retinal venous thrombosis.

Epoprostenol

[Clinical aspects, diagnosis and therapy of ocular Chlamydia infection].

As reported in the recent literature, eye infections caused by Chlamydia trachomatis are increasing in number. Modern laboratory techniques were used in this study to scan a total of 114 patients with unilateral follicular conjunctivitis for these organisms. The authors describe the clinical picture, course, and results of therapy in a total of 6 (5.3%) young patients with chlamydial infections diagnosed by scraping and cell culture.

[Human corneal fibroblasts and a human conjunctival cell line (Chang) produce a thymocyte-activating factor (TAF) in vitro].

A variety of distinct cells such as fibroblasts, epidermal cells, glioblastoma cells and corneal epithelial cells have been shown to produce Interleukin 1-like thymocyte-activating factors. Under in vitro conditions these cytokines influence a multiplicity of different cell types, including lymphocytes, fibroblasts, PMN and hepatocytes. In the present study human corneal fibroblasts, as well as the human epitheloid conjunctival Chang cell line were found to produce thymocyte-activating factors (TAF). The molecular weight of the corneal fibroblasts-TAF was found in the 670 kD region, with minor peaks of activity at 30, 17 and 4 kD. TAF from the Chang-cell line eluted at 15-25 kD. Chromatofocusing revealed the pI of the fibroblast-TAF to be at 6.7 and 5.2 and the pI of the conjunctival cells to be at 7.0, 5.3 and 5.0. The latter cells produced prostaglandin E2 upon stimulation. The immunomodulating properties of these cytokines make an important in vivo role in the pathogenesis of inflammatory corneal and conjunctival diseases probable.

[Corneal epithelial cells and human conjunctival cell line (Chang) produce an interleukin 3-like factor].

Tissue mast cells play a central role in the pathogenesis of allergic eye diseases. In the study reported here the authors investigated whether human corneal epithelial cells and a human epitheloid conjunctival cell line (Chang) can produce an interleukin 3 (IL 3)-like mast cell-activating factor. The activity was detected at a m.w. of 15 and 30 kD, the isoelectric points were located at a pH of 7.85, 7.15 and 6.75. The authors believe that this cytokine, which is produced by epithelial cells of the cornea and conjunctiva, might play an important role in the pathogenesis of allergic ocular diseases.
