[Chemotherapy of high-risk trophoblastic neoplasia].

With recent progress in chemotherapy, the prognosis of patients with trophoblastic neoplasia has greatly improved, but the remission rate of patients with choriocarcinoma remains unfavorable. The Committee for Trophoblastic Disease of the Japan Society of Obstetrics and Gynecology reported the results of hospital registration at 77 institutions throughout Japan in 1987. In this report, the survival rates of patients treated during the 7 years from 1974 to 1980 were described as follows. The 5-year survival rate has approached 100% for patients with invasive mole, while the rate for patients with choriocarcinoma is now approaching 80%. As far as choriocarcinoma is concerned, the survival rate depends on the presence of metastases. The 5-years survival rate for patients without metastases has approached 100%, while that for patients with metastases is barely 60%. Patients with poor-prognosis choriocarcinoma present difficult and challenging problems for the clinician. These patients are best treated on the basis of the prognostic scoring system proposed by Bagshawe. The higher the score, the greater the risk of drug resistance developing during traditional therapy. To date, our experience with the MECA regimen would indicate that it is most effective for patients with high-risk trophoblastic neoplasia. We now also recommend that all high-risk patients should receive at least four additional courses of the regimen after a negative hCG titer has been obtained. After complete remission has been achieved, further follow up should be repeated every month for at least three years.

The lumbosacral plexus of the Japanese serows, Capricornis crispus.

The lumbosacral plexus of the Japanese serow was grossly investigated on 30 fresh specimens. 28 of 30 showed 6 lumbar vertebrae and 2 of the remaining had 5. The communicating pattern among segmental origins from L3 to S5 revealed a variability of the plexus in both types of 5 and 6 lumbar vertebrae. The ratio of prefixed, median fixed and postfixed plexuses in 6 lumbar vertebral type was nearly 2:6:1. The serow's plexus was basically similar to that of domestic ruminants.

Lectin histochemical study on the infraorbital gland of the Japanese serow (Capricornis crispus).

Histology and lectin histochemistry were performed in the infraorbital gland of the Japanese serow. The gland is composed of glandular tissues and a pouch filled with the secretion. The tissues consist of an inner layer of sebaceous glands and an outer layer of apocrine glands. The male sebaceous layer is made up of the ordinary type, whereas the female's layer consists of the ordinary and modified types. In the apocrine gland stained with Arachis hypogaea (PNA), nine different patterns of glandular tubules were distinguished on the basis of staining of the cytoplasm, the Golgi area of secretory cells and secretion. Secretory modes of apocrine secretion and exocytosis were included in these stainings. Myoepithelial cells stained constantly with Glycine max (SBA) except when only the Golgi area of secretory cells was positive. The modified sebaceous gland was stained with PNA, SBA, Ricinus communis I (RCA), Triticum vulgaris (WGA), Canavalia ensiformis (Con A) and Ulex europaeus I (UEA), while the ordinary type was positive in PNA, RCA, SBA, WGA and Con A. The secretion in the pouch was stained with PNA, RCA, SBA, Dolichos biflorus (DBA), WGA and Con A. These findings suggest that the modified sebaceous gland contains large amounts of glycoconjugates and the apocrine gland shows a cyclic secretory process of apocrine secretion and exocytosis.

The brachial plexus of the Japanese serow (Capricornis crispus).

The brachial plexus of the Japanese serow was macroscopically studied. It was composed of nerves derived from the sixth (C6), seventh (C7) and eighth (C8) cervical nerves and the first (T1) thoracic nerve (98.1%). Rarely, the second T2) thoracic nerve joined it (1.9%). C6 joined C7 and made up the cranial trunk. C8 and T1 (+T2) united to form the caudal trunk. Each trunk split into dorsal and ventral divisions. The dorsal divisions from C6-T1 formed the dorsal cord which gave off the thoracodorsal, axillary and radial nerves. The manner of union of ventral divisions and their divergence was classified into 4 types. Type I was that the ventral cord was formed by cranial and caudal ventral divisions and sprouted the musculocutaneous, median and ulnar nerves. It was found in 67.3%. Type II was that the proximal muscular ramus of the musculocutaneous nerve emerged from the median nerve and the ulnar nerve appeared from the caudal ventral division from C8-T1. It was observed in 19.2%. Type III was that the proximal muscular ramus and ulnar nerve were derived from the cranial and caudal ventral divisions, respectively. It was seen in 11.6%. Type IV was quite different from type I to III. The cranial ventral division from C6-C7 was not observed. The caudal ventral division from C8-T1 alone formed the ventral cord giving off the nerves. It was encountered in one case (1.9%) and probably represented rare variation. These findings indicate that the brachial plexus of the Japanese serow is chiefly similar to that of other ruminants.

Immunohistochemistry and effect of vinblastine on intracisternal microtubules of the canine neurons.

The immuno-histochemical and drug-binding properties of intracisternal microtubules (IMs) in the RER of canine neurons were studied with antitubulin IgG and vinblastine. Specific fluorescence against antitubulin IgG was not detected in the RER in which the IMs were aggregated but was observed in the cytoplasm outside the RER. Injection of vinblastine into the sympathetic ganglion did not change the size, shape or arrangement of IMs in the RER, although typical honeycomb crystals were formed near IM-containing RER. These findings show that the IM does not possess antigenicity to antitubulin IgG and has no vinblastine-binding site. Thus it is suggested that the IM may be a defective or modified microtubule.

Non-response of rough endoplasmic reticulum containing intracisternal microtubules to axotomy in the dog.

Responses of the RER containing intracisternal microtubules in the canine ganglion nodosum to axotomy were observed at 3, 7, 14, 30, 50 and 99 days after section of the vago-sympathetic trunk. At 3 days, neurons showed no changes. From 7 days to 50 days, a retrograde reaction occurred in the cell bodies; nuclei were irregular in outline and eccentric in position and flattened RER disintegrated and dispersed. On the other hand, dilated RER containing intracisternal microtubules, which was detected as cytoplasmic inclusions under the light microscope, was present at the opposite pole to the eccentric nucleus. The ratio of areas of cytoplasmic inclusion to cell body significantly decreased. However, there were no changes in the size and arrangement of intracisternal microtubules within the RER. At 99 days, axotomized cell bodies had recovered and the RER containing intracisternal microtubules was indistinguishable from the contralateral controls. These findings indicate that the RER containing intracisternal microtubules is not disrupted by axotomy, though it is compactly packed at the periphery of cell bodies during the retrograde reaction. It appears, therefore, that the RER containing intracisternal microtubules does not respond to axotomy.

Ameloblastoma accompanied by prominent bone formation.

A case of ameloblastoma accompanied by prominent bone formation is described. Although the clinical and radiographic appearance suggested an ossifying fibroma or fibrous dysplasia, microscopic examination showed an ameloblastoma of the follicular type with prominent bone formation.

Polyglucosan bodies in the digestive tract of the aged dog.

In our recent studies on aging phenomena in animals, polyglucosan bodies (PGB) were found within the smooth muscle in the digestive tract of aged dogs without neurologic signs. PGB were basophilic, round, or oval bodies which appeared to have a homogeneous or concentric shape. Their histochemical properties were characterized by the presence of glucose polymers (polyglucosan). Electron microscopy revealed that PGB were composed mainly of irregularly clustered, short branching filaments measuring about 90 A in width. PGB were histochemically and ultrastructurally identical to the previously reported Lafora-like bodies in the CNS of aged dogs. PGB were found in all aged dogs and were disseminated throughout the digestive tract, especially in the cecum.

The migration of macrophages after phagocytosis in chick bone marrow.

The migration of macrophages in chick bone marrow was studied by histoplanimetry after an intravenous injection of colloidal carbon or iron-dextran. In the initial stages after the injection, carbon-laden macrophages were found only in the sinus, where erythrocytopoiesis took place. The carbon-laden macrophages then passed through the sinus wall and gradually migrated from the sinus to the extravascular area (i.e., the granulocytopoietic region) peaking at 1 to 3 days after injection. After 7 days, almost all the carbon-laden macrophages accumulated to form "macrophage islets" mainly around the lymphatic nodules in the extravascular area. In the cases injected with iron-dextran, the movement of iron-laden macrophages was similar to that of the carbon-laden ones. The similarity suggests that the direction and time of migration of macrophages are independent of ingested materials.

A case of cementifying fibroma growing into the right maxillary sinus is reported. Clinical and histopathological findings are presented. The differentiation from central fibro-osseous lesions in jaw bones is discussed.
