[A case of food-dependent-exercise induced anaphylaxis possibly induced by shellfish (Sulculus Supertexta and Turbo Cornutus)].

A 17 years old girl experienced an anaphylactic reaction of urticaria, dyspnea, syncope and hypotension while riding a bicycle 55 minutes after eating shellfish Lapas shellfish which was a-like Sulculus Supertexta (SS). She recovered within several hours after the emergency treatment. Another attack occurred 3 months later while she was running with a dog 30 minutes after eating shellfish (Turbo Cornutus; TC). RAST scores were 4 for Lapas and 2 for TC. RAST inhibition test by ELISA showed a high crose-reaction between keyhole limpet hemocyanin (KLH) and Lapas, and between KLH and TC, while the cross reaction between Lapas and TC was low. Gel chromatography with sephacryl G-200 revealed that both Lapas and TC had several allergens with different molecules which were detected by ELISA. Exercise challenge produced an immediate fall of FEV1 and a significant increase in plasma histamine levels for 45 minutes.

Hypersensitivity pneumonitis induced by toluene diisocyanate: sequelae of continuous exposure.

A 41-year-old automobile paint sprayer showed the clinical features of hypersensitivity pneumonitis 1 week after he had begun to work with paint materials containing toluene diisocyanate. His symptoms began 6 to 8 hours after exposure to the agent and spontaneously disappeared by the next morning. He had diffuse, fine reticulonodular shadows on a chest roentgenogram and a restrictive impairment of pulmonary function. Immunoglobulin G antibody to toluene diisocyanate-human serum albumin was present in bronchoalveolar lavage fluid and sera: IgA antibody was present only in bronchoalveolar lavage fluid. Also, the patient had sensitized bronchoalveolar lymphocytes to toluene diisocyanate-human serum albumin. The histologic findings suggested hypersensitivity pneumonitis. The results of bronchoalveolar lavage, which was repeated on four separate occasions, showed lymphocytosis and a predominance of suppressor-cytotoxic T cells. The findings from serial determinations of humoral antibodies showed no changes consistent with the results of clinical and laboratory studies. In contrast, blastogenic responses of bronchoalveolar lymphocytes to toluene diisocyanate markedly decreased, and the patient showed clinical improvement despite continued exposure to the agent.

Allergens of the house dust mite Dermatophagoides farinae. II. Immunological characterization of four allergenic molecules.

In a previous study, we detected four major allergens from the house dust mite extract prepared from the body of Dermatophagoides farinae, employing radioimmunoelectrophoresis. The four allergenic proteins were designated as Me1, Me2, Me3 and Me4. The most prevalent allergen was found to be Me1 and its molecular weight was determined to be 17,000 daltons. In this study, the molecular weight of the second prevalent allergen, Me2, was determined to be 27,000 daltons using the immunoblotting method. The molecular weights of the other two major allergens, Me3 and Me4, were also estimated.

Spontaneous regression in murine hypersensitivity pneumonitis: lack of immunological tolerance.

We have reported a murine model of hypersensitivity pneumonitis (HP) by transnasal administration of Thermoactinomyces vulgaris (Tv) antigen and shown a potential usefulness in the study of the immunopathogenesis of HP. Since we observed spontaneous regression (SR) of the lung lesions with continuous administration of Tv antigen in our model of HP, attempts were made to study if SR is caused by immunological tolerance. The titers of anti-Tv IgG antibody in serum and bronchoalveolar lavage fluid (BALF) did not show any decrement during the period of SR, at the 6th and 9th weeks after the first administration of the antigen. A delayed-type hypersensitivity reaction, i.e. footpad swelling with Tv antigen, did not change significantly during the SR period. Analysis of BALF cells disclosed that Lyt-2-positive cells, i.e. suppressor/cytotoxic T lymphocytes, did not increase to predominate Lyt-1-positive cells. The lymphocyte proliferation test with Tv antigen showed that the lung lymphocytes reacted to Tv antigen as well as concanavalin A during the period of SR in a similar magnitude as in the development period (3 weeks after the first treatment with the antigen). Treatment with cyclophosphamide before the period of SR resulted in no delay of SR. Finally, the adoptive cell transfer of spleen T cells obtained from mice during the SR period did not manipulate the development of granulomatous pneumonitis in the recipient mice. We concluded that the events causing SR in our murine model of HP may not involve the induction of immunological tolerance in humoral or cellular immunity.

[Study on the clinical usefulness of RAST using insect and mite allergen discs].

In order to examine the usefulness of mite and insect RAST, RAST was carried out on 79 asthmatic patients using mite, cockroach, mosquito and larval chironomid midge (Chironomus thummi thummi, CTT) allergen discs supplied from Pharmacia AB, and discs of two species of adult chironomid midge (Tokunagayusurika akamusi, TA and Chironomus yoshimatsui, CY in Japan) which were prepared in our laboratories. The results were as follows. 1) The positive rate of RAST in 65 randomly selected asthmatic patients was 62% in the case of mite, 25% for TA, 20% for CY, 14% for cockroach, 11% for CTT, 8% for silk and 6% for mosquito. The positive rates of silk and mosquito RAST were low. However, these RAST seemed to be clinically useful when patients were positive to these RAST. 2) The rate of coincidence between skin test and RAST was 86.5% in the case of mite and 30% in the case of silk. The rate of coincidence between prick test using adult CY and RAST using larval CTT was 46.5%. 3) The positive rate of larval CTT RAST was about half that of adult TA or adult CY. However, larval CTT RAST seemed clinically useful when patients were positive to the RAST, because all the patients who were positive to larval CTT RAST were also positive to both adult TA and CY RAST. 4) A significant correlation was found among the three kind of chironomid midge RAST and also between cockroach RAST and any of the three kinds of chironomid midge RASTs.

[A case of dermatomyositis which rapidly developed to respiratory failure in the presence of pneumomediastinum].

A 49-year-old woman was admitted in February 1987, with a six-month history of joint pain, maculopapular and erythematous rash, proximal muscle weakness and a two-month history of skin ulceration and dyspnea on exertion. Physical examination showed Gottron's papules on her fingers and a faint heliotrope rash. Biopsy of erythematous skin lesions on the shoulder and the back of the hand revealed perivascular inflammatory cell infiltration and tiny ulcerative lesions of the cutaneous tissue. Biopsy of the right quadriceps muscle showed type II fiber atrophy and slight perivascular lymphocytic infiltrate, whereas serum CPK level was within normal range. Chest X-ray film showed granular infiltrates in both lower lung fields. Based on the current findings the case was diagnosed as dermatomyositis associated with interstitial pneumonia. The administration of prednisolone, 30 mg/day resulted in the improvement of the skin lesions and muscle weakness, while the intensity of lung infiltrates was little affected. Three months after steroid therapy, the patient was readmitted because of increasing dyspnea on exertion and multiple skin ulcers. Chest X-ray revealed a small amount of gas in the mediastinum, in addition to slight deterioration of interstitial lung disease. In spite of various treatments for pneumomediastinum, including bed rest, administration of analgesics and oxygen inhalation, it developed rapidly, complicated severe subcutaneous emphysema and right-sided pneumothorax. Although high-dose prednisolone therapy and mediastinal drainage were performed, the pneumomediastinum was not resolved and she died from respiratory failure. At autopsy, predominant histological features of the lungs were acute interstitial pneumonia with hyaline membrane and edematous granulation formation in the alveoli.(ABSTRACT TRUNCATED AT 250 WORDS)

Comparison of total IgE and anti-mite IgE antibody levels in the sera of patients with atopic dermatitis and/or atopic bronchial asthma.

In the present study characterization of serological features of AD and/or BA patients were attempted. Nearly all of the patients (23 out of 24) with AD with or without episodes of BA had total IgE levels higher than 1,000 IU/ml. Conversely, 14 out of the 15 BA patients showed total IgE levels less than 1,000IU/ml.9 out of the 14 AD patients with BA(AD + BA) had histories of childhood asthma but required no current treatment for BA. The rest of the AD + BA patients required medication for BA but they were easily controllable with conventional bronchodilators such as beta 2 stimulators and/or xanthine derivatives. It was shown that AD patients (n = 6) with extremely high titers of anti-mite IgE antibodies (more than 110 PRU/ml up to 820 PRU/ml) remained free from BA episodes in the presence of hyper IgE immunoglobulinemia (1,818 IU/ml to 47,300 IU/ml). The results indicated that hyper IgE immunoglobulinemia in atopic patients might prevent the development of severe BA, but on the other hand, increase the possibility of developing AD.

[Study on the clinical significance of pharmacia rast RIA in animal allergy].

To study the usefulness of Pharmacia animal allergen discs, the specific IgE antibodies in the sera of patients with allergy to pet or laboratory animals were measured with the use of those discs. The coincidental ratio between skin test to cat hair and RAST to cat epithelium was 73.3%, however between skin test to dog hair and RAST to dog epithelium or dog dander it was 66.7% or 60.0%, respectively. The rate of RAST positive do dog dander was higher than to dog epithelium, and so it seems to be the optimum allergen for screening. The correlation between RAST value to dog epithelium and dog dander was good, but there was no relationship between cat and dog epithelium, it seems that cross-antigenicity is low. In laboratory animals, the rate of RAST positive was in decreasing order: guinea pig, mouse, rabbit, rat, hamster. The rate of RAST positive to serum proteins of rat and mouse was lower than to epithelium and urine proteins. It seems not to be useful.

Dogs hypersensitive to house dust mite.

Active intradermal skin reaction, respiratory resistance and respiratory patterns were examined in five dogs after intradermal injection or inhalation of house dust mite (Dermatophagoides farinae) extract and the results obtained were as follows: 1. Three out of the five dogs exhibited positive skin reactions against house dust mite extract solution at concentrations of 0.1 micrograms/ml to 10 micrograms/ml. 2. Inhalation of mite extract at a concentration of 1 mg/ml by the dogs that had positive skin reactions produced a marked increase in respiratory resistance and changes in spirogram. These results suggest that the dogs that exhibited positive skin reactions were spontaneously sensitized to D. farinae.

Experimental hypersensitivity pneumonitis in the mouse: histologic and immunologic features and their modulation with cyclosporin A.

A murine model of hypersensitivity pneumonitis (HP) was established with transnasally administered Thermoactinomyces vulgaris (Tv) bacilli. Bronchoalveolar lavage (BAL) of experimental animals revealed marked increase in the total cell, lymphocyte, and macrophage numbers; the findings were similar to those in human HP. The BAL lymphocytes were mostly Thy 1.2 positive. Lyt-1 positive cells predominated Lyt-2 positive cells. Anti-Tv IgG antibodies and delayed-type hypersensitivity footpad reactions against Tv were detected in animals with HP. Cyclosporin A (CyA), a potent immunosuppressive drug, had marked effects on the development of HP in this model. When CyA was administered throughout the course of Tv inoculations, the granulomatous pneumonitis was markedly suppressed, and an increase in BAL lymphocytes, Thy 1.2 positive cells, was suppressed. When CyA was administered only during the first half period of the Tv treatment, suppression of the disease was minimal; when CyA was administered in the latter half, both the HP lesions and the increase in BAL cell lymphocyte numbers were significantly suppressed. These results indicate that a series of transnasal administration of Tv in mice may provide a good model for human HP.

Granulomatous lung disease by Thermoactinomyces vulgaris antigen in C5 deficient and sufficient mice.

Hypersensitivity pneumonitis (HP) is a granulomatous lung disease with pulmonary lymphocytic infiltration supposed to be caused by type III and type IV allergic mechanisms. In type III allergy, compliment activation plays an important role. In this study, we adopted the intranasal administration of the ultrasonified Thermoactinomyces vulgaris(Tv) antigen to mice, which causes granulomatous lung lesions. We used C5 (the fifth component of compliment) deficient B10D2/oSnJ mice and C5 sufficient B10D2/nSn control mice for the above murine model of HP to see if late components of compliment (C5-9) are concerned in the granuloma formation of HP. The result was that both C5 deficient and sufficient mice formed histologically similar granulomatous lung lesions with lymphocytic infiltration. Our data indicate that C5 is not necessary for the granulomatous lung lesions of HP by the Tv antigen.

Preventive effects of cyclosporin on diabetes in NOD mice.

Non-obese diabetic mice aged 30 to 60 days were treated orally with Cyclosporin at doses of 25, 15 and 2.5 mg/kg every 2 days until 160 days of age. Diabetes developed in 12 out of 18 oil-treated mice (67%), with partial to complete Langerhans' islet destruction associated with lymphocytic infiltration. The non-obese diabetic mice showed a plasma glucose concentration of 6.62 +/- 0.92 mmol/l (mean +/- SD) at 50 days of age. The plasma glucose level of oil-treated non-obese diabetic mice gradually increased after 130 days of age and reached 14.0 to 19.0 mmol/l at 160 days of age, while Cyclosporin-treated non-obese diabetic mice showed neither clear increase of plasma glucose levels nor development of insulitis. The cumulative incidence of diabetes in Cyclosporin-treated mice was significantly lower than that in oil-treated mice (p less than 0.01). Subsequently, Cyclosporin treatment was started after development of glucose intolerance. Twenty-five mg/kg of Cyclosporin was administered every 2 days for 35 days. Cyclosporin appeared to have little therapeutic effect on diabetes in non-obese diabetic mice.

Antigen-nonspecific and specific suppression of lymphokine production in desensitized guinea pigs.

Doubly immunized guinea pigs may be desensitized with respect to delayed hypersensitivity reactions against both antigens (anergy) by injection of large doses of either one. This anergic response therefore has both a specific and nonspecific component. The specific component of desensitization persists longer than the nonspecific one. In the present study, we have explored the mechanism of both antigen-specific and antigen-nonspecific suppression during the later stages of desensitization. Guinea pigs immunized with two antigens, DNP-KLH and DNP-EA, were desensitized with DNP-EA. The lymph node cells obtained from the animals 1 day after desensitization were unable to produce MIF in the presence of either antigen. The cells obtained 3, 5, and 7 days after desensitization were able to generate MIF when stimulated with the non-specific antigen (DNP-KLH), but not with specific antigen (DNP-EA). It was shown that both T- and non-T-cell fractions obtained 1 day after desensitization had the capacity to antigen-nonspecifically suppress MIF production. In contrast, if the cells were obtained 3 or 5 days after desensitization, T cells could inhibit only the antigen-specific production of MIF, while non-T cells were still capable of suppressing antigen-specific and nonspecific MIF production. Interestingly, when these two populations were mixed back again, it was now only suppressive to the specific antigen-induced MIF production. This latter observation indicates that nonspecific suppressor non-T cells may themselves be regulated by suppressor T cells. Furthermore, antigen-specific suppressor T cells were shown to produce soluble factor(s) which inhibited the production of MIF.

Interstitial pneumonitis in autoimmune MRL/lpr mice and its treatment with cyclosporin A.

Age-associated changes of anti-double-stranded (ds) DNA antibodies, anti-single-stranded (ss) DNA antibodies, and serum immune complex concentrations were studied in MRL/lpr mice. All anti-ds DNA antibodies, anti-ss DNA antibodies, and immune complexes began to be detected in the sera of MRL/lpr mice aged 8 to 13 weeks and increased remarkably after 17 weeks of age. Almost no pathological findings were observed histologically in the lungs of MRL/lpr mice aged 8 weeks but interstitial pneumonitis became evident at 14 weeks of age. Peribronchial and perivascular lymphocyte infiltrations were seen in the lungs of 14-week-old MRL/lpr mice and became more severe at 21 weeks of age. Oral administration of cyclosporin A to 15-week-old MRL/lpr mice markedly prolonged their life span. The lungs of 44-week-old MRL/lpr mice given cyclosporin A showed few pathological findings except for minimal perivascular lymphocyte infiltration.
