Toxic epidermal necrolysis with extensive mucosal erosions of the gastrointestinal and respiratory tracts.

Toxic epidermal necrolysis with epidermal shedding over almost the entire body occurred in a patient with classical rheumatoid arthritis treated with sulindac, penicillamine and a combination analgesic containing paracetamol and chlormezanone. Erosions in the lower respiratory tract and the intestine contributed to a lethal outcome of the disease and showed a microscopical picture similar to that of the skin involved. The histopathological picture of these extracutaneous lesions have been only briefly reported previously.

Absent latissimus dorsi muscle and anhidrotic axilla in Poland's syndrome. Case report.

The case of an 18-year-old female with absence of the right breast, sternocostal part of the major pectoral muscle and total absence of the minor pectoral muscle and the latissimus dorsi muscle, is presented. She also had decreased sweating capacity and absence of terminal hairs in the central part of her right axilla. Histological examination of skin specimens from this area of the axilla demonstrated hypoplasia of the apocrine sweat glands, but normal eccrine sweat glands, indicating an apocrine dependent axillary sweating function. The breast was constructed using a direct subcutaneous prosthesis implant, giving a final result that was good in regard to shape and softness. This provides grounds for reconsidering the use of the latissimus dorsi muscle flap in breast constructions in Poland's syndrome.

Ceftazidime and renal function.

Glomerular filtration rate (GFR) as measured by 51Chrome-EDTA clearance, decreased with a mean of 10 ml/min during therapy with ceftazidime 2 g bid in 16 patients with initial GFR of 30 to 110 ml/min. A significant increase in the excretion of urinary alanine aminopeptidase was also found. In another three patients with initial GFR of 17-22 ml/min increases in serum creatinine during therapy were noted. These observations indicate that ceftazidime should be used with caution in patients with impaired renal function and not be combined with nephrotoxic drugs until the safety of such combinations has been studied.

Acute radiorenography during attacks of renal colic.

Acute radiorenography was compared to emergency excretory urography to evaluate its diagnostic accuracy in 22 patients with clinical signs of renal colic, including erythrocytes in the urinalysis. All 16 patients with delayed excretion and/or dilated renal pelves on excretory urography had delayed excretion of radioactivity on renography. Furthermore, 6 patients with reduced renal function judged by renography had marked or slight delay in excretion on excretory urography. Small stones not causing stasis were found on excretory urography in 2 patients with normal renograms. It is concluded that acute radiorenography is simple, rapid and safe, and identifies accurately patients with abnormal renal function during attacks of colic, with less risks than excretory urography of causing renal damage or aggravating the pain of renal colic.

Bone scintigraphy in the diagnosis of skeletal involvement and metastatic calcification in multiple myeloma.

The contribution of bone scintigraphy to the diagnosis of skeletal involvement in multiple myeloma was evaluated in a consecutive, unselected series of 25 previously untreated patients. Definite scintigraphic abnormalities were found in 11 patients (localized in 4, generalized in 7) (44%). In the majority of patients the clinical value of the information gained from scintigraphy was roughly equal to that found by radiography. However, 2 patients with normal and 2 with questionable bone scans had X-ray evidence of skeletal involvement, and the extent of bone destruction in 4 cases was seriously underestimated in the bone scans. Small osteolytic lesions were as a rule not detected by scintigraphy. On the other hand, in 8 patients the bone scan added information of involvement of ribs, pelvis and vertebrae, not clearly visualized by X-ray. In 2 patients, both with IgA myeloma, hypercalcemia and uremia, a massive extraskeletal uptake of the bone-seeking isotope was observed. Autopsy verified in one case the presence of a disseminated metastatic calcification.

Penile blood pressure in erectile impotence following cystectomy.

Penile systolic blood pressure and systolic and diastolic brachial blood pressure were measured in 17 men who had undergone cystectomy and prostatectomy for cancer of the bladder. After the operation, 13 patients reported erectile impotence, while 4 retained sexual potency. Comparisons were made with 7 healthy, potent controls. The systolic penile blood pressure was significantly lower and the systolic pressure gradient between brachial and penile pressure was significantly higher in the men with erectile impotence than in the potent men. Reduced penile blood pressure after such operations may arise from vascular trauma. A surgical technique that protects penile blood vessels may conceivably reduce the risk of erectile impotence following cystectomy.

Basal cell carcinoma in children. Case report.

Clinical and pathoanatomic features of two children with basal cell carcinoma, an extremely uncommon disease in childhood, are reported. Clinically these lesions had a benign appearance, similar to a foreign body granuloma and an intradermal naevus, respectively, but histological examination showed a picture typical of basal cell carcinoma. The tumours were radically treated and no recurrence occurred during an observation time of 12 and 42 months, respectively.

The exchangeable splenic platelet pool in response to intravenous infusion of isoprenaline.

8 healthy volunteers and 4 asplenic subjects, in whom autologous platelets had been labelled with radioactive sodium chromate, received i.v. infusions of isoprenaline in a dose of 0.03 microgram x kg-1 x min-1 over a period of 6 min. In the former group these infusions caused a significant decrease in the concentration of labelled as well as unlabelled platelets in the peripheral blood. Body surface countings showed that a significant increase in the count rate over the spleen occurred concomitantly with the decrease in the circulating platelet-bound radioactivity (PBR). In the group of asplenic subjects no change in PBR occurred. It is concluded that adrenergic beta-receptor stimulation causes a transitory trapping of platelets in the exchangeable splenic platelet pool.

Transplantability and metastasibility of an MCA-induced sarcoma in nude mice. Influence of transfer of thymus cells and adoptive immunity.

Challenge of C57(nu/nu) mice with an MCA-induced sarcoma, MCG101, induced in C57BL/6J mice, disclosed a higher resistence subcutaneously and intravenously than in syngenic C57/(+/+) mice. Transfer of 5 X 10(7) thymus cells to the nude mice brought their resistance to subcutaneously injected tumour cells back to the level of the C57/(+/+) mice, but only with the highest tumour cell doses tested, i.e. 5 X 10(4) and 10(5) cells. Against intravenously injected tumour cells a similar effect by the thymus cells was only suggested. Spontaneous metastases from the same tumour transplanted to the tail did occur in the nude mice and were possibly facilitated by specifically sensitized spleen cells. The results indicate that the resistance to subcutaneously injected tumour cells in nude mice may well be dependent on their incapability to develop thymus-dependent immuno-responses and that spontaneous metastases can be facilitated by a weak immuno-response.

Tumour metastasis in mice with reduced immune reactivity. III. Studies with three weakly antigenic tumours in thymectomized and/or sub-lethally irradiated mice.

A previous observation that increased spontaneous metastasis formation from the highly antigenic MCA-induced sarcoma MCG101 occurred in immunosuppressed C57BL/6J mice prompted the present study to establish whether or not tumours capable of inducing only weak transplantation resistance would behave similarly. One MCA-induced sarcoma, MCG12, one epidermoid carcinoma, EpCa1 in CBA mice and a spontaneous mammary carcinoma, MaCa1 in C3H mice, were transplanted into adult thymectomized and/or sub-lethally irradiated mice at different intervals after irradiation. The effects of the treatments on the immunoreactivity were monitored in separate groups by determining the primary and secondary responses to SRBC and the survival of first set allogeneic skin grafts. The treatment resulted in a significantly increased incidence of mainly the lymph-node metastases with MCG12 and of only the lung metastases with EpCa1. No increase in metastases was noted with MaCa1. Thymectomy potentiated the increase in metastases from MCG12 in irradiated mice but unexpectedly so only for a limited time after irradiation. Local growth of MCG12 and MaCa1 at the transplantation site was more rapid in females than in males. The changes in tumour growth and spread induced by the treatments varied with tumour-host system, sex and time of transplantation after irradiation and did not conform well with any of the tested parameters of immunoreactivity. There was no close parallel between the effects of the treatments on growth and spread of the tumours.

Tumour metastasis in mice with reduced immune reactivity. IV. Restoration of relative resistance to metastasis formation in thymectomized sub-lethally irradiated mice by spleen cells.

Adult thymectomy and/or sub-lethal irradiation increased spontaneous metastasis formation in several syngeneic murine systems (MCA-induced sarcomas MCG101 in C57BL/6J mice, MCG12 in CBA mice and MCA-induced epidermoid carcinoma EpCa1 in CBA mice). In order to differentiate between immunological and non-immunological effects of these treatments on tumour spread the effects of presumed selective restoration of immune competence by transfer of syngeneic spleen cells from normal donors shortly after irradiation (1.5 months before tumour transplantation) was perfomed. This procedure brought the resistance of thymectomized irradiated mice to tumour spread back to the level seen in untreated mice, providing further evidence for the conclusion that the indrease of tumour spread in this experimental model to a major part was immune-mediated. In thymectomized irradiated mice adoptive transfer of concomitant immunity by spleen cells from tumour-bearing mice soon after tumour transplantation completely inhibited the spread of the highly antigenic MCG101 and reduced that of the weakly antigenic MCG12 to control level. In contrast, adoptive transfer after tumour resection had no significant effect of the spread from either to the tumours.

Regional distribution of common and dysplastic naevi in relation to melanoma site and sun exposure. A case-control study.

The frequency of melanoma (CMM), and of common and dysplastic naevi (CN and DN) in areas of skin chronically, intermittently and rarely exposed to UV light was investigated in 121 melanoma patients (30-50 years) and 310 controls. Both cases and controls had significantly more CN in intermittently exposed areas than in areas chronically or rarely exposed. The ratio of observed to expected number of CMM was also highest in intermittently exposed skin (1.3 compared to 0.8 in chronically exposed and 0.5 in rarely exposed areas). Thus, intermittent UV exposure seems to have the most potent 'naevogenic' as well as carcinogenic effect on melanocytes. Nineteen per cent of controls and 56% of cases had naevi fulfilling the clinical criteria for DN. The distribution pattern of DN was clearly different from that of CN and does not accord with the idea that UV light is a major aetiological factor for DN. The probability of CMM significantly increased with the degree of relative clustering of CN (p less than 0.05) and of DN (p less than 0.01). This co-variation of naevi and CMM over the body surface might be the result of the local insults to the melanocyte system caused by UV light and/or to the fact that naevi are precursor lesions of CMM.
