Altered immune responses and susceptibility to Leishmania major and Staphylococcus aureus infection in IL-18-deficient mice.

IL-18, formerly designated IFN-inducing factor, is a novel cytokine produced by activated macrophages. It synergizes with IL-12 in the induction of the development of Th1 cells and NK cells. To define the biological role of IL-18 in vivo, we have constructed a strain of mice lacking IL-18. Homozygous IL-18 knockout (-/-) mice are viable, fertile, and without evident histopathologic abnormalities. However, in contrast to the heterozygous (+/-) or wild-type (+/+) mice, which are highly resistant to the infection of the protozoan parasite Leishmania major, the IL-18-/- mice are uniformly susceptible. The infected IL-18-/- mice produced significantly lower levels of IFN-gamma and larger amounts of IL-4 compared with similarly infected +/- and +/+ mice. In contrast, when infected with the extracellular Gram-positive bacteria Staphylococcus aureus, the IL-18-/- mice developed markedly less septicemia than similarly infected wild-type (+/+) mice. However, the mutant mice developed significantly more severe septic arthritis than the control wild-type mice. This was accompanied by a reduction in the levels of Ag-induced splenic T cell proliferation, decreased IFN-gamma and TNF-alpha synthesis, but increased IL-4 production by the mutant mice compared with the wild-type mice. These results therefore provide direct evidence that IL-18 is not only essential for the host defense against intracellular infection, but it also plays a critical role in regulating the synthesis of inflammatory cytokines, and therefore could be an important target for therapeutic intervention.

The vav proto-oncogene product (p95vav) interacts with the Tyk-2 protein tyrosine kinase.

The vav proto-oncogene product participates in the signaling pathways activated by various cell-surface receptors, including the type I IFN receptor. During engagement of the type I IFN receptor, p95vav is phosphorylated on tyrosine residues, but the kinase regulating its phosphorylation has not been identified to date. Our studies demonstrate that p95vav forms a stable complex with the IFN-receptor-associated Tyk-2 kinase in vivo, and strongly suggest that this kinase regulates its phosphorylation on tyrosine. Thus, p95vav is engaged in IFN-signaling by a direct interaction with the functional type I IFN receptor complex to transduce downstream signals.

High degree of occult tumor contamination in bone marrow and peripheral blood stem cells of patients undergoing autologous transplantation for non-Hodgkin's lymphoma.

Twenty patients with non-Hodgkin's lymphoma (NHL), primarily intermediate-and high-grade, were evaluated for evidence of bone marrow (BM) or peripheral blood stem cell (PBSC) lymphoma contamination using tumor-specific oligonucleotide-polymerase chain reaction (TSO-PCR). Patients were enrolled in a single-institution study comparing PBSC and bone marrow transplantation (BMT) for relapsed NHL. A molecular marker (CDR3 rearrangement, T cell beta receptor [TC beta R] rearrangement, or BCL-2/IgH rearrangement) was identified from analysis of the diagnostic tissue in 17 of 20 patients. Prior to undergoing BMT, 14 of 17 patients had PCR evidence of lymphoma involvement of either BM (11/17) or PBSCs (9/11). No decrease was found in the frequency of contamination of PBSCs compared with BM. In one patient, quantitative competitive PCR (C-PCR) identified a three- to tenfold greater quantity of contamination in the BM compared with PBSC. All evaluated patients (6/6) with contamination prior to BMT had persistence of marrow contamination following BMT. Our data demonstrate that TSO-PCR can generate a molecular marker for the majority of patients with intermediate- and high-grade NHL. In addition, we identified a high rate of occult lymphoma involvement in both BM and PBSC. As demonstrated by C-PCR, however, quantitative differences may exist in contamination of BM and PBSCs.

Altered glucose transporter mRNA abundance in a rat model of endotoxic shock.

To better understand molecular mechanisms of glucose transport in shock, we studied glucose transporter isoform mRNA abundance after injection of S. enteritidis endotoxin (40 mg/kg) or saline. Six to 8 hours after injection, endotoxin-treated animals compared to controls became hypoglycemic (44 +/- 6 vs. 111 +/- 4 mg/dl) and lactacidemic (5.9 +/- 0.5 vs. 1.3 +/- 0.1). At such times, tissue RNA was isolated and hybridized to Riboprobes for GLUT1 (erythrocyte), GLUT2 (liver), and GLUT4 (muscle/fat) glucose transporter isoforms and expressed as percent of control. GLUT1 mRNA abundance was increased in fat (660%, p less than .05), soleus muscle (314%, p less than .05), and liver (871%, p less than .001) of endotoxin-treated rats. Soleus muscle GLUT4 mRNA levels were increased (+33%, p less than .02), while liver GLUT2 mRNA levels were markedly decreased (-58%, p less than .01). The overall increase in GLUT1 mRNA abundance accompanied by lowered liver GLUT2 mRNA levels may either cause or reflect profoundly altered glucose transport.

A community of buddies: support groups for veterans forced into early retirement.

Ongoing support groups for veterans who have made a poor adjustment to involuntary early retirement are offered by a mental hygiene clinic in a Veterans Administration hospital. Most of the veterans have been forced to retire for medical reasons. The groups are intended to strengthen their support systems and to reduce the negative psychosocial consequences of involuntary early retirement, such as isolation, boredom, depression, and impaired social relationships. A survey of group members indicated their functioning had improved after group participation. They reported better mental health, improved social relationships, higher activity levels, and greater satisfaction with retirement.

Percutaneous renal biopsy in infants and young children.

Records of 55 percutaneous renal biopsies performed in 53 children aged 5 days to two years were reviewed. Radiographic or ultrasound imaging techniques were not utilized during the biopsy procedure. Forty-nine of 55 biopsy attempts yielded sufficient tissue for histologic evaluation (89.1%). Transient gross hematuria not requiring intervention was the sole complication and was noted in 6 of 55 biopsy attempts (10.9%). Percutaneous renal biopsy can be performed safely in very young children and the success of the procedure is comparable to that in older children.

Medicine, the media and monetary interests: the need for transparency and professionalism.

Emerging evidence suggests that media coverage of medicine is increasingly promotional in nature. Recent Australian examples include misleading newspaper articles on an experimental cancer vaccine and a high profile television current affairs segment on a new influenza drug, which failed to disclose the industry ties of a key expert featured in the report. There are widening concerns that this problem in medical journalism may be exacerbated by the growing commercialisation of medical and scientific research, and the increasing ties between researchers, doctors and pharmaceutical or biotechnology companies. Closer links between industry and medicine are being explicitly encouraged both in academia and the health care sector for the mutual benefits they bring. However, these partnerships are the cause of growing unease within medicine. In the United States, rigorous legislation governing research protocols is being proposed, and in Australia new ethical guidelines covering industry-profession relationships are being promulgated. If one of the media's roles is informing the community about the business of health and medicine in a fair and accurate way, a cultural change in medical journalism is required.
