Can women with gonadotropin levels diagnostic of polycystic ovarian syndrome benefit from therapy with dopamine agonists?

On the basis of LH and FSH data the incidence of PCOS in a series of 33 infertile women with hyperprolactinemia due to differing causes was found to be 39%. Where the elevations of prolactin were intermittent (spikers), the incidence was 44%, compared to 33% in a control population and 23% (9% histologically confirmed) in a 1991 Dublin general infertility clinic population. Androgen levels were uninformative, as were TRH stimulation test results. All 33 infertile women were treated with dopamine agonists. Four pregnancies occurred in the PCOS group. Two were hyperprolactinemic spikers on dopamine agonists plus antiestrogens. Four of the 10 pregnancies in the non-PCOS group were also on dopamine agonists plus antiestrogens. Two of these were spikers. The use of dopaminergic drugs to lower circulating prolactin is established. The addition of an antiestrogen in a concomitant PCOS situation is rational. There appears to be little justification, however, to use them in any anovulatory situation, including PCOS in the absence of hyperprolactinemia.

An outbreak of Salmonella mikawasima associated with doner kebabs.

During October 1992 an increase in the number of isolates of Salmonella mikawasima, a rare serotype, was noted including a cluster of nine cases in the South West Thames region. A case control study was conducted and univariate analysis showed a statistical association between illness and eating at take-away A for cases compared with household controls (P = 0.003) and with neighbourhood controls (P = 0.0245). Cases were also more likely to have eaten kebabs than were controls or average take-away A customers, implicating doner kebabs as the most likely vehicle of infection. Plasmid profile analysis of the nine cases' isolates showed them to be indistinguishable and to be characterized by a single plasmid of approximately 60 MDa. The original source of the Salmonella mikawasima contamination was not determined, but food preparation practices for kebabs at take-away A were insufficient to protect against illness if contaminated. This outbreak was only recognized because of the unusual serotype, but could be an indication of a more widespread problem with doner kebabs.

Preparation and immunogenicity of vaccine Ac NFU1 (S-) MRC towards the prevention of herpes genitalis.

A subunit antigenoid vaccine, Ac NFU1 (S-) MRC, was used to prevent primary herpes genitalis in 60 subjects considered to be at risk of this infection. There was no evidence of serious local or general side effects. Neutralising antibody responses were detected in 59% and 90% of subjects receiving the low and high doses of vaccine respectively; immunoprecipitating antibody was detected at a lower frequency, namely in 23% and 43% of subjects receiving the low and high doses respectively. After a mean follow-up period of 18 months none of the vaccinated subjects contracted herpes genitalis after completing the vaccination course.
